I would like to apply for Schengen visa to cover my intention to travel in Schengen Areas for 11 days from October 7-17, 2016 with
France as my Point of Entry on October 7, 2016 and the total number of days in France is longer than other countries we’ll be visiting. While
Amsterdam, Netherlands will be my point of Exit on October 17, 2016 (Amsterdam to London, UK).

My purpose of travel is tourism and primarily claiming a prize of Europe trip for 2 of Tour Radar “My summer Moment” photo contest last July
2015 announced by TourRadar-Topdeck Travel, Cinderella Ocao, the primary winner and I, Gib Sam G. Salak, being the second passenger.

The trip includes coach transport, ferry crossings (where applicable), accommodations, meals, sightseeing tours of all major stops
(including some with local city guides) and 'Bucket List Inclusions' activities as specified on each individual trip page.

I am applying through your honorable consulate because Paris, France will be the city of my First Entry of the Trip. We’ll be visiting in the
Schengen Areas as indicated in the following itinerary:

Day 8 | 14 October 2016 | Munich to Rhine Valley
Day 9 | 15 October 2016 | Rhine Valley to Amsterdam, Netherlands
Day 10 | 16 October 2016 | Amsterdam
Day 11 | 17 October 2016 | Amsterdam to London, UK (To catch my return flight to Philippines)

1. Application Form duly dated and signed with passport size pictures
2. Passports showing my travel experience and approved UK Visa
3. Winning Photo as seen on Tour Radar official website
4. Proof of Hostel Accommodation in St. Christopher’s Inn Gare du Nord, Paris, Detailed Day to Day Itinerary, and Travel
Confirmation from Topdeck Travel with booking Reference no. TU140407
5. Round-trip Airline ticket via Qatar Airways for Clark-London (October 6, 2016) and London-Clark (October 19, 2016)
6. Official Business Permits and Registration Documents (DTI, Mayor’s Permit, BIR Certificate of Registration, Contract of Lease)
and Business Contracts as proof of income
7. Bank Certification and Statement of Account
8. Photocopy of Birth Certificate
9. Travel Insurance with a coverage of €40,000
