I. Latar Belakang
Obat tradisional saat ini merupakan salah satu yang menjadi aspek fokus
menjanjikan dalam perkembangan obat baru karena aktivitas biologinya (Newman &
Cragg, 2012; Pandit, Kim, Kim, & Jeon, 2011). Kunyit dalam kedudukan tata nama
tanaman termasuk kedalam kategori sebagai berikut : Kingdom: Plantae;
subkingdom: Viridiplantae; Infrakingdom: Streptophyta; Superdivision:
Embryophyta; Division: Tracheophyta; Subdivision: Spermatophytina; Kelas:
Magnoliopsida; Superorder: Lilianae; Order: Zingiberales; Keluarga: Zingiberaceae;
Genus: Curcuma L; Spesies: Curcuma longa. Kunyit juga terekenal di tempat lain
dan memiliki nama yang berbeda beda seperti di Inggris (Saffron), India (Turmerik),
Bengali (Haldi, Halud), Gujarati (Halada, Haldar), Hindi (Haldi, Hardee), Kashmiri
(Lidar), Punjabi (Haldi, Halja), Malayalam (Mannal, Manjal), Marathi (Halad,
Halede), Sanskrit (Anestha, Bahula), Tamil (Manjal, Mancal) (Chakraborty et al.,
2011). Kunyit memiliki berbagai aktivitas farmakologi seperti anti malaria (Nelson
et al., 2017), anti inflamasi, anti neoplastik (Fadus, Lau, Bikhchandani, & Lynch,
2017), pengobatan alzheimer (Shen, Liu, An, & Ji, 2016), immunomodulator
(Catanzaro, Corsini, Rosini, Racchi, & Lanni, 2018), anti kanker (Vemuri et al.,
2017), gastroprotective (Kwiecien et al., 2019) dan nephroprotective (Hosseini &
Hosseinzadeh, 2018).

Senyawa kurkumin dapat larut dalam aceton, methanol dan ethanol (Goel,
Kunnumakkara, & Aggarwal, 2008). Peningkatan kelarutan kurkumin selain dapat
juga digunakan teknik dispersi padat. Teknik ini dilakukan dengan polyethylene
glycol (PEG) 4000 dan 6000 pada perbandingan (1:1 , 1:4, dan 1:8) (Modasiya &
Patel, 2012), serta dengan penggunaan CO2 + ethanol (Cunico, Acosta, & Turner,
2017). Isolasi kurkumin lainnya adalah dengan menggunakan pelarut dichlormethane
kemudian direflux selama 1 jam sampai didapatkan filtrat, lalu dipanaskan
menggunakan water bath dan residu padatan dilarutkan menggunakan hexane
(Amalraj, Pius, Gopi, & Gopi, 2017). Perlindungan terhadap cahaya harus dilakukan
untuk mencegah terjadinya degradasi pada kurkumin (Prasad et al., 2014).
Efek penghambatan tumor pada kurkumin juga dilakukan pada Wilm’s tumor
(WT-1) yang berperan terhadap ekspresi gen leukemic K562 yang di mediasi oleh
signalling protein kinase C (PKCa) sebagai factor transkripsi WT-1. Peran kurkumin
adalah dengan memberikan efek anti proliferasi terhadap pengikatan WT-1 dengan
protein promoter, dan menurunkan WT-1 dengan mRNA (Semsri, Krig, Kotelawala,
Sweeney, & Anuchapreeda, 2011). Menurut (Jiang et al., 2012) terdapat hubungan
struktur dan aktivitas anti tumor dari senyawa kurkuminoid dari kunyit dengan
menggunakan MTT (3-4,5-dimethyl thiazol- 2-yl)-2,5-diphenyl tetrazolium bromide
assay. Peran dari kurkumin dalam penghambatan lipolitik pada berbagai stimulasi
adiposan 3T3-L1. Efek anti lipolitik pada kurkumin juga dapat menurukan kadar asam
lemak bebas dalam plasma dan memperbaiki sensitivitas insulin (Xie, Kong, Wu, Li,
& Li, 2012). Penghambatan efek kardiotoksik pada kurkumin akibat dari penggunaan
daunorubicin dikarenakan pengikatan spesifik dengan carbonyl reductase (CBR1)
yang berperan sebagai kofaktor pembentukan daunorubicinol (Hintzpeter, Hornung,
Ebert, Martin, & Maser, 2015).
Menurut (Chen et al., 2010) melaporkan mengenai aktivitas anti virus influenza
dari kurkumin sebesar 90% pada hasil kultur sel dengan pemberian dosis 30 µM.
Kurkumin mengganggu proses perkembangabiakan virus yang pada akhirnya
menghambat proses terjadinya influenza. Proses penghambatan pada virus
digambarkan melalui hasil dari hemaglutinasi pada virus H1N1 dan H6N1. Pada
Candida albicans, kurkumin memiliki efek yang membuat struktur morfologi dari
C.albicans menjadi rusak, hal ini dikarenakan oleh efek anti adhesi pada ALS1 dan
ALS3 yang berkaitan dengan proses pembentuka biofilm (Alalwan et al., 2017).
Infeksi pada sistem reproduksi merupakan kondisi yang dapat terjadi pada
wanita maupun pria yang akan berdampak pada kesehatan reproduksi. Pada wanita
lebih berisko terjadinya infeksi ini, hal ini dikarenakan saluran pada sistem urinary
wanita lebih pendek dibandingkan dengan pria sehingga terjadinya resiko infeksi lebih
tinggi. Selain itu juga pada penggunaan obat kontrasepsi yang dapat mempengeruhi
dari kelenjar pada vagina, sehingga lebih rentan terserang infeksi. Saat ini sedang
dikembangkan produk kontrasepsi yang berasal dari tanaman yang memiliki zat aktif
kurkumin berupa sediaan gel (Patel, Thakkar, Moradiya, Gandhi, & Gohel, 2015). Gel
ini bersifat biodegradable yang terbuat dari Poloxamer dan HPMC K4M yang bersifat
non hormonal, memiliki efek dan durasi panjang, dan diaplikasikan dengan cara
intravaginal. Optimasi dosis digunakan dengan tiga factor yaitu Box Behken Design
(BBD), dimana setiap batch berisikan formula 19.96% Poloxamer 407, 3.83%
Poloxamer 188, 0.91% HPMC K4M yang sudah di optimasi waktu dan efek yang
dapat memberikan nilai tertinggi. Kurkumin juga dapat digunakan pada pengobatan
polycystic ovary syndrome (PCOS) yang dibandingkan dengan clomiphene citrate.
Kurkumin mampu memperbaiki profil hormone, lipid, antioksidan dan status glikemik
pada ovarium model hewan uji yang dibuat menjadi PCOS dengan membuat disfungsi
sel ovari, mengganggu pematangan sel telur yang mempegaruhi pada fertilitas (P. S.
Reddy, Begum, Mutha, & Bakshi, 2016). Menurut (Akinyemi et al., 2015) kunyit
memiliki efek mencegah hipertensi dari hasil biomarker reproduksi pada tikus jantan.
4.9 Aktivitas anti angiogenik dan anti proliferative
Menurut (Grace G.L. Yue et al., 2010) fraksi polar pada kunyit memiliki efek
immnuomodulator pada human peripheral blood mononuclear cells (PBMC). Efek ini
dihasilkan dari proliferasi pada PBMC melalui uji penggabungan (methyl-3H)-
thymidine. Berdasarkan hasil ini kunyit memiliki potensi untuk dignakan sebagai terapi
adjuvant pada pasien dengan kemoterapi yang mengalami penurunan daya tahan tubuh.
Selain Ar turmerone terdapat juga α-turmerone yang berasal dari kunyit juga memiliki
aktivitas sebagai anti proliferative pada sel kanker HepG2, MCF-7 dan MDA-MB-231.
Kedua turmerone tersebut memiliki efek stimulatory pada proliferasi PBMC dan
produksi sitokin. Anti proliverative dari α-turmerone dan kurkuminoid memiliki
potensi sebagai pengobatan kemoterapi (Grace G L Yue et al., 2010).
4.11 Aktivitas anti inflamasi
Anti inflamasi merupakan efek yang dimiliki dari BDMC yang merupak derivat
kurkuminoid. Efek ini dihasilkan dari jalur signalling pada heme oxygenase-1 yang
dimediasi peningkatan ion Ca2+ intraseluler, yang kemudian menurunkan aktivitas
calmodulin dependent protein kinase II, regulasi signal ekstraseluler kinase 1 / 2 dan
NF-E2-related factor-2. Menurut (Cooney et al., 2016) kurkumin memiliki kemampuan
aktitivitas anti inflmasi pada kolon di hewan model tikus yang dibuat inflamasi bowel
disease.
