The undersigned who consents to :
1.2.1. Transfusi darah dan produk-produk lain dari darah, bila dirasa perlu oleh dokter.
The transfusion of blood and other blood derived products as the surgeon or physician feels necessary.
1.2.2. Ahli bedah yang bertugas mencari konsultasi atau mendapat bantuan dari dokter lain yang berkaitan jika dirasakan perlu,
selama pelaksanaan tindakan atau operasi berlangsung.
The attending physician / surgeon seeking consultation or assistance from other relevant specialist when it is needed
during the course of this operation / procedure
1.2.3. Pasien dapat dirawat di ruang intensif setelah operasi sesuai kondisi pasien. Untuk pelaksanaan perawatan intensive dan
tindakan medis yang lazimnya dilakukan di perawatan intensif
Patient may be admitted to the Intensive Care Unit based on patient’s condition. For any medical care and treatment
that is normally conducted in the Intensive Care.
1.2.4. Bila staf kamar bedah mengalami luka tusuk / terpapar cairan tubuh pasien, pasien setuju untuk diperiksa darahnya
In the event of staff receiving a needlestick / splash injury, patient agree to laboratory testing of my blood.
