 Role play menerima pasien baru

Tangga 2 januari 2020 pukul 09.00 WIB di UGD RSUD Dr.Soetijono Blora datang
seorang pasien Ny.Lilis dengan keluhan panas 3 hari, dan merasakan nyeri kepala,
kemudian seorang perawat UGD yang berjaga pada hari itu memeriksa dan mengkaji
tentag keluhan pasien.

Perawat UGD : selamat pagi Bu

Pasien : pagi juga sus

Perawat UGD : selamat datang di RSUD Dr.Soetijono Blora, saya perawat kiki yang
bertugas pada pagi hari ini , maaf dengan ibu siapa ?

Pasien : dengan ibu lilis sus

Perawat UGD :baik,bagaimana kondisi pagi ini ibu, apakah ada yang dikeluhkan ?

Pasien : sudah 3 hari badan saya terasa panas, nyeri kepala ,dan pada saat
malam hari keluar keringat dingin

Perawat UGD : baik bu, saya akan memeriksa kondisi ibu mari bisa berbaring disini,
saya akan memeriksa tekanan darah, nadi , suhu, dan respirasi ibu

Pasien : baiklah sus

Perawat UGD : permisi ya bu, saya akan memeriksa ibu

(perawat melakukan ttv),terlebih dahulu saya akan mengukur suhu

tubuh ibu ya, bisa tolong alat ini taruh di ketiak ibu.

Pasien : iya bisa sus

Perawat UGD : (termometer sudah berbunyi)ibu , termometernya sudah

berbunyi,tolong bisa diambil .

Pasien : ini sus termometernya

Perawat UGD :lalu saya akan mengukur tekanan darah ibu ya, biasanya tekanan
darah ibu berapa ?
Pasien : biasanya tekanan darah saya 120/70 sus, saya mengecek 1 bulan
yang lalu.

Perawat UGD : oh begitu ya bu, baik saya akan mengecek dahulu, permisi bu, bisa
tolong naiakan baju lengan kiri/ kanan bu ?

Pasien : iya sus , tangan kiri saja.

Perawat UGD : (Mengecek tekanan darah pasien ) sudah selesai bu

( melepas dari tangan pasien )

Pasien : berapa hasilnya sus?

Perawat UGD : hasilnya 90/80 mmHg bu

Pasien : apakah itu termasuk normal/tidak sus ?

Perawat UGD : dibawah normal bu atau bisa disebut hipotensi

Pasien : lalu bagaimana sus apakah saya akan dirawat inap atau bisa rawat
jalan saja nurse ?

Perawat UGD : dari hasil pemeriksaan didapatkan TD 90/80 mmHg , nadi 90 kali
permenit, pernafasan 24 kali permenit, dan suhu 38 ˚C, nanti akan
diperiksa lebih lanjut oleh dokter ya bu.

Pasien :baiklah.

( beberapa saat kemudian dokter datang dan memeriksa keadaan pasien lalu, setelah
15 menit doter mengintruksikan bahwa pasien bisa dirawat jalan, dan memberikan catatan
resep obat kepada perawat yang akan diberikan ke pasien untuk rawat jalan )

Perawat UGD : ibu bisa rawat jalan, seperti tadi apa yang telah dijelaskan oleh dokter,
ibu tunggu sebentar saya akan mengambilkan obat untuk imrs.

( perawat mengambil obat yang telah diresepkan oleh dokter ), ini obatnya ya bu.

Pasien : iya sus, terimaksih , pembayaran bagaimana sus ?

Perawat UGD : dikasir saja ya bu

Pasien : baiklah sus,saya permisi dulu.

Stairs January 2, 2020 at 09.00 WIB at UGD RSUD Dr. Soetijono Blora came a patient
Ny. Lilis with a 3-day hot complaint, and felt a headache, then a nurse who vigil guard on
the day of examining and reviewing the complaints about the patient.

Nurse UGD : Good morning Mrs

Patient : Morning also nurse

Nurse UGD :Welcome to the HOSPITAL Dr. Soetijono Blora, I'm a nurse Kiki
who served in the morning today, sorry with the mrs who?

Patients : With mrs. lilis nurse,

Nurse UGD : Okay good, how the condition this morning mrs, is there any
complained?

Patients : Already 3 days my body feels hot, headache, and at night out cold
sweat

Nurse UGD : Good mrs, I will check the condition of the mrs, let's mrs can lie
down here, I will check blood pressure, pulse, temperature, and mrs
respiration

Patient : Alright nurse

Nurse UGD : Excuse ya mrs, I will check the mrs

(Nurse do TTV ), first I will measure the temperature of the mrs body

Patient : Yes can nurse

Nurse UGD : (thermometer has already sounded) mrs, Termometer new already
read, please can be taken.

Patient : This is the Termometeric

Nurse UGD : Then I will measure my mrs blood pressure, usually what is mrs
blood pressure

Patients : usually my blood pressure 120/70 nurse, I checked it 1 ago.

Nurse UGD : Oh so, well I will check first, excuse me, can you please increase
sleeves left/right ?

Patient : Yes nurse, left hand only.

Nurse UGD : (Checking the patient's blood pressure) is done mrs

(Remove from patient's hand)

Patient : What is the result nurse?

Nurse UGD : result 90/80 mmHg mrs

Patient : Does it include normal/not nurse?

Nurse UGD : Under normal mrs or can be called hypotension

Patient : Then how will I be hospitalized or can be outpatient in nurse?

Nurse (UGD) : From the results of the examination obtained by TD 90/80 mmHg,
maternal pulse 90 times per minute, respiratory mrss 24 times per
minute, and the maternal temperature 38 °c, then mrs will be examined
further by the Doctor ya Mrs.

Patients : All right.

(A few moments later the doctor came and examined the condition of the patient
then, after 15 minutes the Doter instructed that the patient could be treated to the road, and
give a prescription medication record to the nurse to be given to the patient for outpatient)

Nurse UGD : Mrs can outpatient, as before what has been explained by the doctor,
mrs wait a minute I will get medication for mrs.

(The nurse takes the medication that has been prescribed by the doctor), this is her cure.
