This application form is for international student applicants (non-U.S. citizens and non-resident aliens). If you are a U.S. citizen or
resident alien, you will only need to submit the program application form. Program application forms are available on-line at
http://www.une.edu/admissions or by contacting the Office of Admission, University of New England, 11 Hills Beach Road, Biddeford,
Maine 04005-9599, USA or at admissions@une.edu.

Please note: information requested on this form will be used to complete the Certificate of Visa Eligibility for Nonimmigrant F-1 Student Status,
commonly known as the I-20 form. This is the document issued by a school through the internet-based Student and Exchange Visitor Information
System (SEVIS) for presentation at a U.S. consulate abroad to apply for an F-1 Student visa; it must also be presented to an immigration official upon
entry into the U.S.

Your passport and I-20 form MUST match in order for you to be issued an F-1 visa stamp and be admitted into the United

To ensure reasonable success in the classroom, all students applying as undergraduate or graduate degree-seeking students to the University of New
England must demonstrate proficiency in the English language. International students generally show proficiency by taking the Test of English as a
Foreign Language (TOEFL). To be accepted into a full-time University degree program, a minimum score of 213 (computer test), 550 (paper test), is
required.

STATEMENT OF FINANCIAL RESOURCES
Department of Homeland Security regulations require institutions to obtain evidence stating that applicants have adequate financial
resources to provide for their expenses while studying in the United States. Funds may come from a variety of resources, including
personal savings, scholarships, fellowships, sponsoring agencies, continuance of salary, the applicant’s family or any dependable
source. All international applicants, those applying from outside and those currently living in the United States, must fill out the
Declaration of Finances and submit supporting evidence of funding.

On a separate piece of paper, please provide a list of each of the sources that will be used fund your costs at UNE. This should
include separate information for 1) personal funds, 2) family funds, 3) scholarships funds, 4) government funds, and 5) other funds.
Please include name, relationship, address, occupation, guarantee statement, and signature with date of individuals that will help
support your educational expenses.

Read the following instructions very carefully. Please note that support may come from one or several sources.
• Enter the amount of funds that will be available for each year of study next to each applicable source of funds.
• Enter amounts in U.S. dollars.
• Enter the information requested about each applicable source of funds.
• Enter the total amount of funds that will be available for each year of study for a 12-month period.
• Remember to enclose evidence of funds for each indicated source. Statements from banks or savings, employer or government must
be on an official letterhead, must include the original signature of an official representative, and must specify amount of money
available in U.S. dollars.

IMPORTANT NOTE FROM THE U.S. GOVERNMENT:
All applicants for a non-immigrant student visa are now required to pay a $100 SEVIS fee. Please visit the U.S. Immigration website for more
information. This fee must be paid before the applicant arrives at the consulate for the visa interview. Visit on-line at https://www.fmjfee.com/index.jhtml.
Please complete both sides of the application and return to the Office of Admission. Attach an affidavit of support documenting ability to pay all
educational and living expenses while studying in the United States. The affidavit may include a notarized letter from your sponsor and current bank
statements. The I-20 will be issued when this form is returned with the appropriate affidavit of support.
