Sekitar 50% orang dewasa di Amerika Serikat (AS) menggunakan kacamata atau lensa kontak.
Seperempat dari mereka menggunakan lensa kontak terdiri dari 90% pengguna lensa kontak lunak(Soft
Contact Lens/SCL) dan 10% pengguna jenis Rigid Gas-Permeable (RGP) yang kaku.

Bahan Lensa Kontak

Bahan lensa kontak yang digunakan harus memiiki kriteria antara lain bahan yang dapat dibentuk sesuai
permukaan kornea, tahan lama, mudah ditangani, nyaman, dan transparan. Permukaan pada lensa
kontak akan terbasahi dan harus tetap dalam keadaan basah di antara kedipan mata. Lensa kontak juga
diharapkan tidak menimbulkan endapan yang menempel pada lensa kontak yang nantinya akan menjadi
media bakteri atau jamur untuk berkembang. Dan pada lensa kontak diharapkan dapat mentransmisikan
oksigen dan karbon dioksida serta dapat menjaga pertukaran air mata disetiap kedipann untuk
mempertahankan metabolisme dan membuang debris yang terbentuk. Pada lensa kontak tidak
diharapkan menyerap air mata yang banyak pada kasus gangguan air mata dimana produksi air mata
yang sedikit sehingga akan membuat mata menjadi lebih kekurangan air mata karena terserap oleh
kontak lensa.

Dk, Dk / t, dan sudut pembasahan adalah beberapa istilah yang digunakan untuk menggambarkan
permeabilitas oksigen dan keterbasahan suatu lensa kontak. Dk adalah suatu istilah yang mengacu pada
permeabilitas oksigen dari bahan lensa, di mana D adalah koefisien difusi untuk oksigen dalam material
ke kornea, dan k adalah konstanta kelarutan oksigen dalam material. Dk / t mengacu pada

“Hard” contact lenses were introduced in the 1940s and were made of polymethylmethacrylate
(PMMA), the same plastic used subsequently for the rst intraocular lenses. These lenses were very
durable, but oxygen passed only around them, not through them. Soft contact lenses are made of a
hydrogel polymer, hydroxyethylmethacrylate or, more often now, a silicone hydrogel. Hydrogels have
more oxygen permeability when they have higher water content, but the higher-water-content lenses
tend to cause dryness of the cornea if they are made thin, and they may form deposits and require
frequent replacement. Modifying the hydrogels yielded “silicone hydrogels,” used for daily and
extended wear. These lenses achieve their oxygen permeability with less water content, using pores
induced by the presence of silicon atoms, rather than high water content. Some patients are more
comfortable with the hydrogels than the silicone hydrogels. Available lenses dier in chemistry
throughout the lens or at its surfaces with diering wettability, exibility, clarity, resistance to deposit
retention, and suitability for those with dry eye. They also dier in their longevity and should be replaced
at recommended intervals. RGP lenses were developed in the latter 20th century, using the architecture
of large-polymer plastics to allow passage of oxygen. RGP lenses allow oxygen to pass through and allow
more tear exchange around the lens with each blink than soft lenses allow, so that oxygen-depleted
tears are not trapped between the lens and the cornea. The addition of uorine to the silicone/acrylate
material increases oxygen permeability and encourages the coating of the lens with mucin, which
improves wettability. There are various chemistries, with diering advantages.

Patient History and Examination

Factors that increase the risk of complications with contact lens use include diabetes mellitus, especially
if poorly controlled; immunosuppression; long-term use of topical ocular medications, such as
corticosteroids; and occupational chemical or foreign body exposure. Other relative contraindications to
contact lens use include an inability or history of failure to care for contact lenses, monocularity,
abnormal eyelid function such as Bell palsy, severe dry eye, and corneal neovascularization. The eyes
and adnexa are examined for abnormalities of eyelid and lash position, tear lm, and ocular surface.
Eyelid movement should be observed. The cornea and conjunctiva should be evaluated carefully for
signs of allergy, scarring, symblepharon, or other signs of conjunctival scarring diseases, such as ocular
cicatricial pemphigoid and giant papillary conjunctivitis or abnormal vascularization resulting from
previous lens wear. Through refraction and keratometry, the ophthalmologist can determine whether
there is signicant corneal, lenticular, or irregular astigmatism. The identication of irregular astigmatism
may suggest other pathologies, such as keratoconus.

Contact Lens Selection

The main advantages of soft contact lenses are their comfort and shorter period of adaptation (see
Table 5-3). They are available with a wide range of shapes and materials and are usually easier to t than
rigid lenses. A lost or damaged lens is readily replaced without much cost. Soft lenses are designed for
monthly, daily, and 1–2-week replacement. Very few soft lenses dispensed now are intended for longer-
duration duty cycles. Daily-wear lenses have been favored in the United States ever since reports in the
1980s showed increased incidence of keratitis with extended-wear lenses. Improved materials that have
greater oxygen permeability (Dk = 60–140) have been approved for extended wear. Use of these
materials may decrease the risk of infection compared with the risk associated with earlier materials,
but patients who want extended-wear lenses should understand the increased risk of bacterial keratitis
and the signs and symptoms that require the attention of a physician. Additional risk factors for
extended-wear complications include previous eye infections, lens use while swimming, exposure to
smoke, dry eye, eyelid margin disease, and allergy. Daily-wear users are exposed to the same risks, only
less so. About 10% of contact lenses t in the United States are RGP lenses. Some with high-oxygen
permeability are approved for extended wear. They are typically replaced yearly. The main advantages
of RGP lenses are the high quality of vision they oer, including the correction of corneal astigmatism (see
Table 5-3), durability, and simplicity of lens care. The main disadvantages of RGP contact lenses are
initial discomfort, longer period of adaptation, greater knowledge and eort required for tting, and the
greater cost of replacing a lost or damaged lens. People who use contacts only sporadically are generally
more comfortable with a soft lens.

Keuntungan utama

Contact Lens Fitting

The goals of lens tting are good vision that does not uctuate with blinking or eye movement, comfort for
the entire day, and low risk of complications. Terms used to describe lens geometry, illustrated in Figure
5-4, include the following: The apical zone of the cornea (corneal apex) is the steepest part of the
cornea, normally including its geometric center. The base curve is the curvature of the central posterior
surface of the lens, which is adjacent to the cornea, described by its radius of curvature. The diameter
(chord diameter) is the width of the contact lens from edge to edge. The optical zone is the central area
of the lens; curvature of its front surface is chosen to achieve the desired power of the lens. Peripheral
or secondary curves of the posterior lens surface farther from its center are designed to follow the
shape of the cornea, which is normally atter toward its periphery. Sagittal depth or vault is the distance
between the center of the posterior surface to the plane of the edges of the lens.

Contact lenses are described by their rear vertex power, which can be checked for rigid lenses by placing
the back of the lens on the nose cone of a lensmeter (done less frequently for soft lenses, similarly or in
a liquid chamber, correcting for index of refraction). The required contact lens power may dier slightly
from the power of the appropriate spectacle correction, to adjust for the dierence in position of the
correcting lens. This disparity is known as the correction for the eectivity of lenses, and is discussed in
Chapter 1. The magnitude of the correction depends on the strength of the lenses, and can be readily
obtained from standard tables (Table 5-4).

Soft Contact Lenses

Soft contact lenses are comfortable, thanks to the thin edges encountered by the eyelids. The lenses
extend beyond the cornea to the conjunctiva overlying the sclera. A good soft contact lens t is often
described as having a “light 3-point touch,” the lens touching the surface of the eye at the corneal apex
and at the limbus on either side of the cornea. A soft or rigid lens can be made to t tighter by either
choosing a smaller radius to steepen the base curve or by increasing the lens chord diameter without
changing the radius. Either way, we are increasing the “sagittal depth,” the height of the lens’s rear
surface, if it were placed edge-down on a table (Table 5-5 and Fig 5-5).

Lensa Kontak Lembut

Evaluating the soft lens t, the clinician should observe the lens movement and centration after the lens
has been in the eye for a while, until the t has stabilized. In a good t, the lens will move about 1 mm with
upward gaze or blink, or with gentle pressure on the lower eyelid. A tight lens resists movement, and a
loose lens will move too much. By evaluating a patient’s vision and comfort, slit-lamp ndings (eg, lens
movement, lens edge, limbal injection), and keratometry mires, the clinician can determine whether the
lens ts well (see Table 5-5). Once a t is deemed adequate, an overrefraction is performed to check
whether the power needs to be adjusted. When the initial tting process is complete, the nal lens
parameters should be clearly identied (Table 5-6). The clinician should teach the patient how to insert
and remove the contact lenses, how to care for them, and how to recognize the signs and symptoms of
eye emergencies. Follow-up appointments are scheduled depending on the lens and patient.

Rigid Gas-Permeable

Contact Lenses Rigid gas-permeable lenses have smaller chord diameters than soft lenses, and allow
more circulation of fresh tears under the lens with each blink. Central and peripheral curves, edge
shape, and central thickness can be tailored for stable vision, comfort, and tear exchange. Unlike a soft
contact lens, an RGP lens maintains its shape when placed on the cornea, and a tear lens lls the space
between the lens and the cornea. The power of the tear lens is determined by the dierence between the
curvature of the cornea (K) and that of the base curve of the contact lens (Fig 5-6). The t is described as
apical alignment (on K), when the base curve matches that of the cornea; apical clearance (steeper than
K), when the base curve has a steeper t, with radius shorter than that of the cornea; and apical bearing
(atter than K), when the base curve is atter than the cornea.

Gas-Permeable yang Kaku

To fit an RGP lens of a particular design and material, one can consult the laboratory fashioning the lens,
a tting guide for the lens, or use trial lenses. In designing the lens, consideration is given to pupil size and
lid tension, as well as refractive error, keratometry, and possibly topography. Figure 5-7 The most

Gralapp.) The most common type of RGP lens t is an apical alignment t with the upper edge of the lens
under the upper eyelid (Fig 5-7). This “lid attachment” t allows the lens to move with each blink,
enhances tear exchange, and decreases lens sensation, because the upper eyelid does not strike the lens
edge with each blink.

Tipe paling umum dari lensa RGP t adalah penyelarasan apikal t dengan tepi atas lensa di bawah kelopak

Alternatively, a central or interpalpebral t is achieved when the lens rests between the upper and lower
eyelids. The diameter of the lens is smaller than with a lid-attachment t. There may be greater lens
sensation, because the eyelid strikes the lens edge with each blink. This type of t may be preferable for
patients who have high upper eyelids, hyperopia, or Asian eyes. To evaluate the t of a contact lens, the
clinician looks for stability of vision and appropriate lens movement. The uorescein pattern is evaluated
at the slit lamp (Fig 5-8). If there is apical clearing of the cornea, pooling or a bright green area will be
observed centrally; if the RGP lens is touching the cornea, this area will appear dark. Over-refraction
determines whether a power change is needed, when the t is satisfactory. Table 5-7 summarizes RGP
lens parameters and the range of normal values.

Power

The power of the tear lens is approximately 0.25 D for every 0.05-mm radius-of-curvature dierence
between the base curve of the contact lens and the central curvature of the cornea (K), and this power
becomes somewhat greater for corneas steeper than 7.00 mm. Tear lenses created by rigid contact
lenses with base curves that are steeper than K (smaller radius of curvature) have plus power, whereas
tear lenses formed by base curves that are atter than K (larger radius of curvature) have minus power
(Figs 5-9, 5-10). Therefore, the power of a rigid contact lens must account for both the eye’s refractive
error and the power introduced by the tear lens. An easy way of remembering this is to use the rules
SAM (steeper add minus) and FAP (atter add plus). Clinical Example 5-3 illustrates these calculations.

Because the refractive index of the tear lens (1.336) is almost identical to that of a cornea (1.3765), it
masks the optical eect of the corneal surface. If the back surface of a contact lens is spherical, then the
anterior surface of the tear lens is also spherical, regardless of the corneal topography. Thus, the tear
layer created by a spherical rigid contact lens neutralizes more than 90% of regular and irregular corneal
astigmatism. This principle simplies the calculation of the tear lens power on astigmatic corneas: the
powers of the steeper corneal meridians can be ignored, and only the attest meridians need to be
considered. The refractive error along the attest meridian is represented by the spherical component of
refractive errors expressed in minus cylinder form. For this reason, it is simpler to use the minus cylinder
format when dealing with rigid contact lenses (Clinical Example 5-4). When a spherical contact lens is in
place, there may still be “residual” astigmatism, which may be caused by the eye’s natural lens and the
shape of the posterior surface of the cornea. There are “back surface toric” gas permeable lenses with
toric back surfaces, chosen to match the shape of the cornea, stabilizing the lens position, with a
spherical front surface. There are also “bitoric” RGP lenses, whose back surface matches that of the
cornea, while the front toric surface is chosen to correct the residual refractive error (Clinical Example 5-
5).

Gas-Permeable Scleral Contact Lenses A scleral lens has a central optic that vaults over the cornea and a
peripheral haptic that rests on the scleral surface (Fig 5-15). The shape of the posterior optic surface is
chosen to minimize the volume of the uid compartment while avoiding corneal contact. The posterior
haptic surface is congured to minimize localized scleral compression; the transitional zone that joins the
optic and haptic surfaces is designed to vault over the limbus.

These RGP lenses, with or without channels, must vent tears in and out to prevent buildup of suction
onto the cornea and allow ample circulation of fresh tears beneath the lens. Improved RGP chemistry
has enabled these lenses to be well tolerated. They have 2 primary indications: (1) correcting abnormal
regular and irregular astigmatism in eyes that cannot be t with corneal contact lenses, and (2) managing
ocular surface diseases that benet from the constant presence of a protective, lubricating layer of
oxygenated tears, such as pellucid degeneration, Terrien marginal degeneration, keratoconus, Ehlers–
Danlos syndrome, elevated corneal scars, and astigmatism following penetrating keratoplasty. These
lenses can be helpful with complications of Stevens–Johnson syndrome, graft-vshost disease, tear layer
disorders, and ocular cicatricial pemphigoid, protecting the fragile epithelium of these corneas from the
abrasive eects of keratinized eyelid margins associated with distichiasis and trichiasis and from exposure
to air. They have promoted the healing of some persistent epithelial defects, where other strategies
have been unsuccessful. Semi-scleral lenses, extending not as far onto the sclera, have diameters 15–18
mm; scleral lenses have larger diameters, 18.1–24 mm.

The classic definition of myopia 10 considers a refractive error of -6.00 DS to -10.00 DS as "high myopia"
and above -10.00 DS as "very high myopia." Pathological or congenital myopia is generally assumed to
be caused by developmental failure of one ofthe ocular components, resulting in elongation of the
anterior-posterior diameter of the eye continuing into maturity. 11 There may be ophthalmoscopically
visible signs such as chorioretinal thinning at the posterior pole associated with a posterior
staphylomata. This may include a choroidal crescent at the optic nerve head and occasional retinal
colobomata. The clinician may even observe gross protrusion (exophthalmos) of the eyebalL which can
be mistakenly confused with Grave's disease (Figure 33-8, A and B). Because of the retinal distension,
best corrected visual acuities are often less than 20/20 (6/6). Because the risk of retinal detachment is
higher, it is important to educate patients as to the signs and symptoms of a detachment, as well as the
appropriate patient response and subsequent treatment. Macular lesions (e.g., macular holes, macular
degeneration, and Fuch's spots) may occur with time, leading to an even more profound loss of
achievable central vision. Special refractive considerations with high myopia involve the minification or
decrease in apparent (perceived) size of an image in relation to the object. This is due to the fact that
minus lenses minifY the retinal image compared with the size in an uncorrected eye. This effect is
covered in detail in Chapters 26 and 32. Visual acuity with high myopia is, therefore, usually somewhat
less than expected because of minification of the correcting spectacle lenses. If a large refractive shift
into the minus direction is measured, it is important to verify a correlating improvement in visual acuity

It is simply easier for the eyes to converge to a near target because the demand for convergence has
been lessened through the minus spectacle lenses. As also noted in Chapter 26, accommodative demand
at near has also been lessened, and the reduction of accommodative convergence at near tends to
offset the lesser demand for convergence in accommodating myopes. Hence, there are only occasional
binocular vision problems associated with the near vergence and accommodation demand changes that
occur as a result of wearing highminus spectacle lenses. The lessened accommodative demand is
actually a positive factor, especially for prepresbyopes and incipient presbyopes, though this relaxed
demand with spectacles can prove to be a problem when contact lenses are prescribed (see Chapter 26).
Conventional high-minus spectacle lenses are both heavy and have thick edges that are cosmetically
unappealing. The optics industry has dealt with these disadvantages by using various methods of
decreasing both thickness and weight (Figure 33-9): 1. High-index lens materials, which allow the same
refractive powers to be achieved with less thickness and weight 2. Lighter lens materials, which reduce
the weight of spectacle lenses 3. Reduced eye size of selected frames that allow highminus lenses of
lesser diameter and therefore decrease both thickness and weight 4. Minus lenticular lens designs, such
as the "myodisc," which decrease peripheral lens thickness and weight at the expense of cosmesis and
field of view 5. Aspheric surfacing, which also allows for decreased peripheral lens thickness and weight
at the expense of field of view

Several annual publications list and describe the various high-index materials and specialty lens designs
currently available. Several guides are published as supplements to professional publications and
provide the most up-to-date information, because new materials and designs are constantly being
developed and introduced. With the availability of purer polycarbonate (PCB) and other optically pure
polyurethane materials, there is no reason not to prescribe high-index materials for high-prescription
patients. The higher the index of refraction, the thinner and lighter the resultant lens. However, higher
index also increases the intensity of light reflections and decreases light transmission. This can be
minimized by prescribing high-index materials with antireflective coatings. I2 The indices of coating
materials are more suitable for high-index materials. I3 There may still be some chromatic aberration,
especially with peripheral viewin~ but this is usually not clinically significant (Table 33-1). The frame
selection process is a crucial one and should not be influenced solely by the patient's cosmetic concerns.
A small eye size and symmetrical shape with adjustable nose pads is usually the best option. If the frame
is wider than the patient's face, the sides of the face will appear compressed behind the frame because
of minification with high-minus lenses and both magnified and distorted with plus lenses. The lens edge
thickness also increases as the decentration increases. Thus, zero decentration is optimal for both vision
and cosmetic appearance. The optical center of the lens should also be at or near the geometric center
of the frame for the same reason. Adjustable nose pads allow the lenses to be set as close to the
corneas as the eyelashes allow. In hypermetropia or aphakia, this significantly reduces magnification. 6
The field of vision is also increased and peripheral aberration decreased by this maneuver.

Various filters are available that may be beneficial to patients with high refractive errors. An ultraviolet
(tN) filter is essential for aphakes and pseudophakes without tN-absorbing intraocular lens implants.
Many high-index materials provide such tN protection, and the additional filter may not be necessary.
Yellow filters are available to decrease the glare of atmospheric haze ("shooting glasses"), which may be
particularly appreciated by patients complaining of reduced contrast. It is important to consider lens-
design options beyond high-index materials if the spectacle prescription is greater than -10.00 to -15.00
O. A biconcave lens (e.g., some of the power is ground on the lens front surface) helps to minimize
overall edge thickness. In higher powers, a lenticular design such as the myodisc lens may be more
practical (see Figure 33-10). The myodisc has a small (35-40 mm) concave disk or "lenticule" ground on a
plano carrier. The power ofthe lens is in the lenticule; thus, the stronger the power, the smaller the
lenticule. A lenticular G design also has a lenticule, but this carrier has a plus curve that creates a thinner
edge. New lens designs include a blended myodisc (which has a series of optically unusable aspheric
curves ground from the carrier edge to the lenticule edge) to reduce the edge thickness further (Figure
33-10). These lens designs, available in both glass and plastic, must be fit carefully with special attention
paid to split IPO measurements and vertical centration.

Aspheric lens designs, which were first employed with high-plus prescriptions, are both thinner and
lighter than spherical lenses of the same optical powers. An aspheric front curve also compensates for
the peripheral aberration a patient would normally experience if a spherical lens is ground on a base
curve flatter than indicated on the corrected curve chart. If an aspheric design is combined with a high-
index material, the resultant lenses are 10% to 40% thinner and lighter than the same prescription in CR-
39, depending on the exact prescription. 13 Aspheric lenses are also flatter on the back surface than
standard lenses, so ifthe spectacles are fit too close for a patient with very long lashes, these may rub
the back of the lens, causing increased soilage. Some manufacturers therefore offer two different back-
surface curvatures. Frame adjustment is also obviously important in such instances. In certain cases, it is
possible to make center-thickness modifications in order to reduce the overall edge thickness of
spectacle lenses of high-minus prescriptions. The standard for CR-39 is 2.5-mm or 2.2-mm center
thickness, so that the lenses pass the "drop-ball" test (see Chapter 23). The high-index materials are
stronger than CR-39. For example, a polycarbonate lens can be ground at 1.0-mm center thickness and
still pass a drop-ball test. Polyurethane lenses can be ground at 1.5-mm center thickness and pass the
test. However, if the center thickness becomes too thin (e.g., 0.6 mm), these lenses may eventually
warp.

rata di permukaan belakang daripada lensa standar, jadi jika kacamata itu terlalu dekat untuk pasien
dengan bulu mata yang sangat panjang, lensa ini dapat menggosok bagian belakang lensa, yang
menyebabkan bertambahnya daya tahan tanah. Oleh karena itu beberapa produsen menawarkan dua
lengkungan permukaan belakang yang berbeda. Penyesuaian frame juga jelas penting dalam hal ini.
Dalam kasus-kasus tertentu, dimungkinkan untuk melakukan modifikasi ketebalan tengah untuk
mengurangi keseluruhan ketebalan tepi lensa kacamata dari resep minus tinggi. Standar untuk CR-39
adalah ketebalan tengah 2,5 mm atau 2,2 mm, sehingga lensa lulus uji "bola jatuh" (lihat Bab 23). Bahan
indeks tinggi lebih kuat dari CR-39. Misalnya, lensa polikarbonat dapat digiling dengan ketebalan pusat
1,0-mm dan masih lulus uji jatuh-bola. Lensa polyurethane dapat di-ground pada ketebalan tengah 1,5-
mm dan lulus uji. Namun, jika ketebalan tengah menjadi terlalu tipis (mis., 0,6 mm), lensa-lensa ini
mungkin pada akhirnya melengkung.
