The goal was for the patient to establish or return to normal bowel functioning patterns within 8 hours of nursing interventions. Interventions included determining stool characteristics, auscultating bowel sounds, encouraging fluid intake and a high-fiber diet, and discussing stool softeners or laxatives as needed. The rationale was to identify causes, improve stool consistency, and enhance defecation. After 8 hours the patient was able to establish normal bowel patterns.
