ANTIEMETIK

Terapi pada kemoterapi
Antiemetic Treatment of
CINV (Chemotherapy-Induced
Nausea and Vomiting)
Introduction
Penggunaan antiemetik yang tepat, mencegah CINV pada
70% hingga 80%
Guideline dari
- MASCC (Multicational Association of Supportive Care
in Cancer)
- ASCO (the American Society of Clinical Oncology)
- NCCN (the National Comprehensive Cancer Network)
STEROIDS, DEXAMETHASONE
Dexamethason mengambil peranan dalam pencegahan
CINV baik fase akut dan delayed
Prevention Fase akut (MASCC, ASCO, NCCN):
highly, moderately dan low emetogenic
Prevention Fase delayed:
Highly emetogenic -- Dexa kombinasi Apprepitant
(MASCC, ASCO, NCCN)
Moderately emetogenic -- Dexa kombinasi Apprepitant
(NCCN)
Agent Route Recommended dose
5-HT3-Receptor Antg
Ondansetron Oral 24 mg (high), 16 mg (moderate)*
IV 8 mg (0.15 mg/kg)
Granisetron Oral 2 mg
IV 1 mg (0.01 mg/kg)
Tropisetron Oral 5 mg
IV 5 mg
Dolasetron Oral 100 mg
IV 100 mg (1.8 mg/kg)
Palonosetron IV 0.25 mg
Steroid
Dexamethason Oral/IV 12 mg (highly emetogenic, with
apprepitant), 20 mg without
aprepitant; 8 mg (moderately
emetogenic); 8 mg
(high/moderate) days 2 and 3
NK1-receptor antg
Aprepitant Oral 125 mg on day 1, 80 mg on day 2
and 3
* 8 mg twice daily is recommended
Antiemetik
Metochlopramide
Pencegahan delayed emesis
Efektivitas metochlopramide kombinasi dengan steroid
sebanding dengan 5-HT3RAS pada prevention delayed
CINV namun tidak direkomendasikan pada guideline baru
Metochlopramide digunakan pada kondisi tertentu pada
intoleransi 5-HT3RAS atau steroid
Suatu studi meta analysis membandingkan 5-HT3RAS
dengan methoclopramide pada fase delayed untuk
emetogenic moderate memberikan keuntungan
Other antiemetic
Olanzepine
Atypical antipsychotic drug
Mekanisme aksi pada multiple reseptor yang
mengatur nausea dan vomiting
Studi trial fase II:
Kombinasi Olanzepin, dexamethason dan
granisetron efektif sebagai prevention CINV
akut dan delayed pada pasien dengan highly
dan moderately emetogenic
Other antiemetic
Benzodiazepin
Sering ditambahkan pada regimen antiemetik
pada kondisi khusus:
- anxiety
- risk reduction of anticipatory CINV
- refractory and breakthrough emesis
Antihistamin
Studi diphenhydramine atau hydroxyzine pada
prevention CINV tidak memperlihatkan aktivitas
antiemetik
Prevention of CINV
Highly Emetogenic Chemotherapy
Acute CINV
Kombinasi 5-HT3 RA, dexamethason dan aprepitant pada
24 jam pertama
Delayed CINV
Cisplastin mengakibatkan 60 hingga 90% pasien
mengalami delayed emesis jika tidak diberikan prevention
antiemetik
Kombinasi dexa dan apprepitant
Contoh regimen antiemtik
CISPLATIN DAN 5-FU
Premedikasi
a. Injeksi Vomceran 8mg/12jam, iv hari I
b. Injeksi Dexametasone 8mg(2 A)/12 jam, iv hari I-V
c. NaCl 0,9% 1 liter+20 mmol KCl + 2 MgSO4 habis dalam
2 jam
(keterangan:Tiap NaCl 500cc dalam 60 menit)
Protokol
Cisplatin…mg dalam NaCl 500cc habis dalam 60 menit
NaCl 3 liter (6plabot) +5cc KCl + 2,5ccMgSO4 habis dlm 1jam
(tiap NaCl 500cc+7,5cc KCl+3cc MgSO4 habis dalam 4 jam)
5-FU…mg dalam dextrose 5% 500cc infuse continue dalam 24
jam (hari I-V)
Contoh regimen antiemtik
Ny W
Doxorubicin – Siklofosfamide
Premedikasi
Ondansetron 8 mg/iv
Dexa 2A iv
Acute CINV
Kombinasi 5-HT3 RA, dexamethason dengan atau
tanpa aprepitant pada 24 jam pertama
Kombinasi antracycline dan cyclophosphamide:
Kombinasi 5-HT3 RA, dexamethason dan apprepitant
Delayed CINV
Dexamethason
5-HT3 RA
Prevention of CINV
Low Emetogenic Chemotherapy
Steroid (dexamethason) pada 24 jam pertama
Tidak ada prophylaxis setelah 24 jam
Prochlorpherazine atau methochlopramide sebagai
alternative dexamethason
Cancer pain:
somatic, usually from tumor or mass effect, bone
metastastases
visceral, infiltration, compression, extension, or
stretching of the thoracic, abdominal, or pelvic
viscera
neuropathic, CA compressing or infiltrating
nerves/nerve roots/blood supply to nerve, Post-
radiation
Physiological effects of Pain
Increased catabolic demands: poor wound
healing, weakness, muscle breakdown
Decreased limb movement: increased risk of
DVT/PE
Respiratory effects: shallow breathing,
tachypnea, cough suppression increasing risk
of pneumonia and atelectasis
Increased sodium and water retention (renal)
Decreased gastrointestinal mobility
Tachycardia and elevated blood pressure
Physiological effects of Pain
Negative emotions: anxiety, depression
Sleep deprivation
Existential suffering
AVOID: meperidine,
agonists/antagonists, combo agents,
propoxyphene
