Kepada seluruh Karyawan,

Sebagai salah satu upaya pencegahan terjadinya peningkatan dan penyebaran kasus di lingkungan
Perusahaan, Manajemen senantiasa memperbaharui Protokol Kesehatan yang sudah diterapkan agar dapat
melindungi kesehatan dan keselamatan seluruh karyawan secara maksimal.
Sehubungan dengan hal tersebut di atas, Manajemen akan memberlakuan Protokol Kesehatan sebagai
berikut :

1. Bagi Karyawan yang melakukan perjalanan di luar area Jabodetabek dan Banten, wajib melakukan hal-
hal sebagai berikut :
- Mengisi Personal Travel Decralation sebelum keberangkatan dan Health Declaration setelah kembali
dari perjalanan dengan jujur;
- Mengisi Self Screening dengan jujur;
- Melaporkan kepada dokter perusahaan setelah kembali dari perjalanan atau 1 (satu) hari sebelum
jadwal masuk kerja mengenai aktivitas selama perjalanan dan kondisi kesehatan;
- Wajib menjalani karantina mandiri selama minimal 5 (lima) hari atau maksimal 14 (empat belas) hari
dan melakukan test antigen atau test PCR sesuai arahan Dokter perusahaan berdasarkan hasil
interview terkait aktivitas perjalanan dan kondisi kesehatan karyawan tersebut;

2. Jika karyawan yang baru kembali dari luar kota dan tinggal bersama dalam satu kost atau rumah
kontrakan dengan rekan kerja sesama karyawan PT CAP, maka karantina mandiri harus dilakukan di luar
kost atau kontrakan dimaksud untuk mencegah terjadinya penularan COVID 19 kepada rekan kerja
tersebut. Tempat yang akan ditempati selama masa karantina harus diinformasikan kepada HR
(Pak Dedi Mulyadi) pada saat mengisi Self Declaration sebelum melakukan perjalanan;

3. Biaya karantina mandiri menjadi tanggung jawab karyawan yang bersangkutan.

Protokol Kesehatan ini merupakan bagian tak terpisahkan dari Protokol Kesehatan sebelumnya yang sudah
diumumkan pada tanggal 28 Juni 2021 yang lalu.

As one of the efforts to prevent the increase and spread of cases in the Company, Management is constantly
updating the Health Protocol that has been implemented in order to protect the health and safety of all
employees to the maximum.
In connection with the matter mentioned above, Management will implement the following Health Protocols
as follows :
1. For Employees who travel outside the Jabodetabek and Banten areas, it is mandatory to do the
following :
- Fill out a Personal Travel Declaration prior to departure and a Health Declaration after returning from
the trip honestly;
- Fill self-screening honestly;
- Report to the company's doctor after returning from travel or 1 (one) day before the work schedule
regarding activities during the trip and health conditions.
 - Must undergo self-quarantine for a minimum of 5 (five) days or a maximum of 14 (fourteen) days and
conduct antigen tests or PCR tests in accordance with the direction of the Company Doctor based on
the results of interviews related to travel activities and health conditions of the employees;
2. If the employee who has just returned from out of town and lives together in a boarding house or rented
house with a co-worker of a fellow PT CAP employee, then self-quarantine must be carried out outside
the boarding or contract intended to prevent the transmission of COVID 19 to the co-worker. The place
to be occupied during the quarantine period must be informed to HR (Pak Dedi Mulyadi) at the time of
filling out the Self Declaration before traveling;
3. The Self-quarantine costs are the responsibility of the employee concerned.
This Health Protocol is part and parcel of the previous Health Protocol that was announced on June 28, 2021.
