Anchor bolt projection above the top of concrete is computed as follows:

Single Nuts
Thickness of grout + thickness of base plate (or height of anchor bolt
SABP Q-001
B10 chair above the top of grout) + 1.5 x diameter of anchor bolt
Sec. 6.2.4
Double Nuts
Thickness of grout + thickness of base plate (or height of anchor bolt
chair above the top of grout) + 2.5 x diameter of anchor bolt

The thread length required at the top of the anchor bolt must be sufficient
to accommodate two nuts and about ½ of the anchor bolt diameter
SABP Q-001
B11 projecting above the top nut. Normally a thread length of about 3 bolt
Sec. 6.2.5
diameters of will be sufficient to provide some tolerance for errors in the
elevation of the anchor bolt placement in the field.
The accumulated variation in dimension between centers of anchor bolt
groups along the established column line through multiple anchor bolt SAES-Q-005
B12
groups shall be equal to or less than 2mm per 10,000 mm, but not to Sec. 4.8.2
exceed a total of 25 mm. (AISC 7.5.1d)
Variation in dimension from the center of any Anchor-rod group to the
SAES-Q-005
B13 established column line through that group shall be equal to or less than
Sec. 4.8.2
6mm. (AISC 7.5.1e)
Welded wire fabric shall be located as shown on IFC Drawings. SAES-Q-001
