OPERATIVE OR PERIODONTAL EXAMS:

WREB staff will give you your Candidate Packet when you turn in your Dental Candidate Limitation of Liability
Agreement/Radiograph Affidavit (on back). Endodontic candidates will also need to turn in their sextants, radiographs, and
Endodontic Worksheet at this time (see below).

The Operative and the Periodontal Treatment procedures may be completed anytime during the exam. Candidates taking
the Endodontics exam in conjunction with either the Operative and/or Periodontics exams will need to adjust their clinic times
accordingly. All patients must be in line for grading by 4:30 pm on Clinic Days 1 and 2 and by 11:00 am on Clinic Day 3.
If your patient is not in line by these times, you will be assessed a late penalty. All work on any procedures must be
completed by 5:00 pm on Clinic Days 1 and 2 of the exam, including taking impressions. Candidates working on any
procedure after 5:00 pm will be assessed a late penalty. After 5:00 pm, only placing a temporary restoration, dismissing your
patient or cleaning your operatory unit and leaving the clinic are allowed.

All Candidates and patients must be out of the clinic by 5:30 pm on Clinic Days 1 and 2. The exam ends at 11:00 am
on Clinic Day 3. No procedures will be allowed after 11:00 am on Clinic Day 3.

ENDODONTIC EXAM:

Endodontic sextants and the Dental Candidate Limitation of Liability Agreement/Radiograph Affidavit form will be collected on
Orientation Day any time between 12:30 pm and 1:30 pm. You were provided a one-gallon ziploc bag with a white
1” x 2” label affixed. Use a black permanent marker and write your Candidate I.D. number on the label. Use this bag to
turn in your sextants, radiographs and Endodontic Worksheet.

When turned in, the bag should contain:


One sextant with the mounted Anterior Tooth with your Candidate I.D. number inscribed on the lingual of the sextant.
One sextant with the mounted Posterior Tooth with your Candidate I.D. number inscribed on the lingual of the sextant.
For each tooth: Two preoperative radiographs (one buccal view and one proximal view) in a four-film mount labeled
only with your Candidate I.D., the tooth number, and “A” (for anterior) or “P” (for posterior). The two empty slots in the
mount will be for your postoperative films.
Endodontic Worksheet with your Candidate I.D. number, anterior tooth number, posterior tooth number and the
posterior canal you plan to treat. If submitting radiographs via computer, check box “Radiographs submitted on
computer” on Endodontic Worksheet.

Any Candidates who have an Endodontic tooth rejected by the Grading Examiners will be notified at the end of Candidate
Orientation and given an opportunity to turn in an alternate submission. There is no deadline for re-submissions. However,
after 7:50 am the first clinic morning, it may take two hours or more for the Grading Examiners to approve or reject a tooth.
You may lose treatment time if a tooth has not been approved by the Grading Examiners prior to your scheduled Endodontics
Exam. There will be no extension of exam time due to this approval process. All submitted models become the property of
WREB and are not returned, except for those rejected due to undiagnostic radiographs.

Submitting conventional films (2.0mm sphere not required): For each tooth you will submit two preoperative
radiographs (one buccal and one proximal view) in four-film mount labeled with your Candidate I.D., tooth number,
and A for anterior or P for posterior. Your name, school name, or location should not appear on the mounts.
Submitting printed digital files (2.0mm sphere required): For each tooth, you will submit a sheet with two
preoperative radiographs (one buccal and one proximal view) labeled only with your Candidate I.D. number, the tooth
number A for anterior and P for posterior. Your name should not appear on the sheets.
Submitting digital radiographs in the computer (2.0mm sphere required): Check box “Radiographs submitted on
computer” on Endodontic Worksheet. The file name for each tooth should include your Candidate I.D. number, tooth
number, and either Anterior or Posterior. A sample file name for an anterior tooth would be: B160 #8 Anterior. The
films should be saved in a manner that will allow you to add the post treatment films, and submit the final films in a
four mount template.

COMPUTER SIMULATION DENTAL EXAMS:

# Information for Pearson VUE Test Centers. The Periodontal Assessment and Prosthodontics (CSW) exam and
Patient Assessment and Treatment Planning (PATP) computer simulation examinations must be completed any time
between January 11 and February 25, 2011 through Pearson VUE. Your Pearson VUE eligibility number is
required when scheduling your exam.

WREB recommends calling as soon as possible to allow arranging the most convenient time for your schedule. If you delay,
you may not be able to take the exam at your preferred time or on your preferred day. Please remember that you may only
take the exam between the scheduled dates listed above. You may contact Pearson VUE by telephone at 1-888-344-7924.
In the event that you cancel after completing the CSW exams, please understand that your score is non-transferable.
