Domini,(The Manager,CBS TK) This is in regards to your complaint registered with us for your Classic Life Premier 2006 Plan, policy 000905962 and 000901266. Policy no 000905962:- We regret to inform you that the captioned policy got terminated due to non-receipt of renewal premiums on March 10, 2011. Kindly note that the fund value on date of termination was Rs.1,04,180.32 and amount paid after deduction of surrender charges (Rs.15,000/-) to you was Rs.89, 180.32 and the same was refunded by cheque no. 939171on March 14, 2011 which was delivered to you on March 16, 2011 by Blue dart vide pod no. 44222058473. However, as per our records the premium amount paid before the policy got terminated was Rs.2,00,000/- and the amount received after the termination was Rs.1,00,000/-. Also note that the same was refunded to you vide.cheque no. 930513 dated February 28, 2011 which was delivered to you on February 28, 2011 by Blue dart vide pod no. 44221473096. Please note that an amount of Rs 1,00,000/-was refunded due to non receipt of BSL-13, CBC, COI Form [ ceritifcate of insurability form ], ECG, FMR, NRI/ Passport copies of entry/exit [ Foreign Questionnaire duly signed by Life Insured]. We would also like to inform you that we are not in receipt of any other premium through demand draft. However, if the same has been deducted from your account, we request you to kindly provide us with the bank statement of the same at any nearest branch of Birla Sun Life Insurance Co Ltd/ Head Office or alternatively, you can also send us an email with a scanned copy of the same, duly signed by you, on customerservice@birlasunlife.com. We will do the needful on receipt of the requisite details. Policy no 000901266:- We regret to inform you that the captioned policy got terminated due to non-receipt of renewal premiums on March 21, 2011. Kindly note that the fund value on date of termination was Rs.1,13,653.01/- and amount paid after deduction of surrender charges (Rs.15,000) to you was Rs.98,653.01/-. Also note that the same was refunded by cheque no.953872 on March 25,2011 which

was delivered to you on March 28, 2011 by Blue dart vide pod no. 13122528344. However, as per our records the premium amount paid before the policy got terminated was Rs.2,00,000/- and the amount received after the termination of policy was Rs.1,00,000/- and the same was refunded to you vide cheque no.930293 on February 02, 2011 which was delivered on February 28, 2011 by Blue dart vide pod no. 44221472142. Please note that an amount of Rs 1,00,000/-was refunded due to non receipt of COI Form [ ceritifcate of insurability form ], FMR, NRI/ Passport copies of entry/exit [ Foreign Questionnaire duly signed by Life Insured]. We would also like to inform you that we are not in receipt of any other premium through demand draft. However, if the same has been deducted from your account, we request you to kindly provide us with the bank statement of the same at any nearest branch of Birla Sun Life Insurance Co Ltd/ Head Office or alternatively, you can also send us an email with a scanned copy of the same, duly signed by you, on customerservice@birlasunlife.com. We will do the needful on receipt of the requisite details. Further, note that we tried calling you at your registered no04692730230 and 09961717342; however we were unable to establish contact. Please register your updated number by submitting fresh Policy service request form (PSRF) along with a KYC proof at any nearest branch of Birla Sun Life Insurance Co Ltd/ Head Office and on receipt of the same needful shall be done.
