Appointed person the one who carries out a task

We know that safe workplaces dont just happen, and that the
challenges require every individual to play their part. Those of you
who work offshore must therefore think HSE in every work situation. This means that you must take responsibility for safety and
health, and ensure that your workmates do the same.
Working on one of Statoils offshore installations involves many
obligations, and I require the following of you where HSE is concerned:
You must:
be familiar with and obey the relevant regulations and
procedures
evaluate all risk factors before you carry out a task
check that you have all the necessary documentation before
starting a job
take your time to carry out all work in a safe manner
always use the mandatory protective equipment
ensure that your workplace is clean and tidy
contribute to a good working environment
be familiar with the procedures and special duties in an
emergency situation
report accidents, near misses and hazardous conditions
Checklists have proved to be an efficient tool in ensuring that important safety considerations are not forgotten or overlooked when
planning and carrying out work. This booklet does not cover all
aspects of some of the operations described - it contains important
checklists for a number of selected operations only. Everyone who
carries out/prepares for a task must have this book on them at all times and use it as a final check before/during/after a job. The complete requirements are described in governing documents.
This booklet is available on all EPNs offshore installations, and I expect
you to use it and go through the checklists whenever necessary.

Barriers, signs

Breathing apparatus

I can confirm that:

2 no-entry barriers are clearly indicated through
red/white chain/band and a red sign with white
text
3 warning barriers are clearly indicated through
yellow/black chain/band and a yellow sign with
black text (in exceptional cases, barrier tape or
yellow and black tape may be used instead)
4 radioactivity is clearly indicated through
yellow/black chain/band. When radioactivity
warning signs are also in place, this means no
entry
5 scaffolding is used to cordon off an area - e.g.
hazardous openings, height differences, etc.
6 signs are standard, e.g. for:

7 the need for barriers both above and below the

Chemicals
I can confirm that:

1 the requirements specified on pages iv-viii of


this booklet have been met
2 an OLF-approved YPD is available, has been
registered with the nurse in HAMS, has been read
and is understood
3 the nurse has been notified of work with
chemicals (type/quantity/use/who/what/where)
when the work requires a work permit
4 the labelling on chemical packaging is in
accordance with the regulations. If in doubt, I will
contact my supervisor
5 I know where the nearest eyewash station/emergency shower is located and I have checked
that there is water in it
6 I will wear the necessary personal protective equipment
7 if chemicals are transferred to another approved container, I will ensure that this is also labelled
in accordance with the regulations
8 containers for toxic waste are available
9 the area is cordoned off and signs are in place
10 when the work is finished, the remaining

Personal safety when working on low-voltage
plant
A.1 Rotating equipment driven by low-voltage
motors

carried out logging
