>Assisted on Operating Room Table >Connected to Cardiac Monitor with vital signs of RR-20, Bp120/80, HR-90 >Signed consent attached to chart 5:08 pm>General Anesthesia administered by Dr. Gomez >Skin preparation done on the anterior neck by OR nurse >Initial count of instruments sponge done with the presence of OR nurse >Draped aseptically by Dr. Dominguez 6:50 pm>Operation finished 5:20 pm>Operation Started whereby a horizontal Incision on the anterior neck initiated by Dr. Dominguez assisted by Dr. Almazan and Dr. Pagalilauan >Deepening on incision >Platysma muscle incised and retracted >Strap muscle incised and divided >Blunt and sharp dissection employed to expose and mobilize the thyroid gland >aftercare rendered >vital signs of: SP O2-100% , BP-110/60 , CR-102 >Oral secretion suctioned until clear with intermittent oxygen by mask 7:05 pm>Transferred to Recovery Room with IVF of D5LRS 1L @650 cc level infusing well @ right hand >Anesthesia record and OR sheet attached to chart 6:15 pm>1st specimen out characterized as reddish, globular soft mass 6:18 pm>2nd specimen out characterized as reddish soft tissue >Luke warm saline washing and suctioning done >Inspection and sponging done 6:40 pm>Specimen placed on a sterile container and labeled >Wound sutured layer by layer >Final count of instruments, needles and sponge complete and verified by OR nurse
