I. Mission Statement (Purpose) The mission statement establishes the purpose of a teams existence. A well-written mission sets or reaffirms the teams belief as to its purpose and links to higher-level organization mission statements. II. Vision Statement

III. Goals and Objective After the team develops its mission statement, it can identify high level goals that support the teams mission. From the teams agreed upon goals, it will develop shorter-term objectives that are critical to team success and that will be used to guide the team in fulfilling its goals. Team objectives must be written so that they are measurable. Monitoring and evaluations, which each team will eventually develop and monitor, will be linked directly back to the teams objectives. IV. Composition and Roles The purposes of defining roles and responsibilities for both the team and individual members are: 1) to enable the team to identify required functional areas, 2) to match them with specific team members, and 3) to highlight any gaps between the skills/abilities required by the team and those available within the current membership. Teams are most effective when: The team has members with the skills and experience needed to do the job. Team members can bring experience and approaches from a range of different backgrounds. The team has enough people to do the job, but not so many that people get bogged down in communication. The team has representation from involved functions and divisions. Look to the mission and objectives to determine who is needed on the team to make sure its goals can be accomplished. The best way to go about this is to list each team member and define the roles and responsibilities of each. Who will be the team leader? Who is responsible for what duties and outcomes? Authorities and Boundaries With the roles defined, you now need to look at what team members can and can't do to achieve the mission: What are the specific duties and responsibilities of the team? How much time should team members allocate to the team mission, and what priority do team activities have relative to other ongoing activities? How should team members resolve any conflicts between their day jobs and the team mission? What budget is available, in terms of time and money? Can the team recruit new team members? What can the team do, what can it not do, and what does it need prior approval to do? Note: Since empowerment involves trust between the parties involved, as a team progresses and proves itself over time, one would expect trust levels to increase and team empowerment boundaries to widen. VI. Resources and Support This section lists the resources available to the team to accomplish its goals.

This includes budgets, time, equipment, and people. In conjunction with the performance assessments, changes to the resources required should be monitored regularly. As well as this, it details the training and coaching support available to the team to help it to do its job.

VII. Operations This section outlines how the team will operate on a day-to-day basis. This can be as detailed or as minimal as the situation warrants. It may be comprehensive and detailed for a long-duration team, or limited to a few bullet points in a team that is expected to have a short life. General Team Decision-Making Process Identify how the team will make decisions. Consensus decision making is necessary for all decisions in which team stakeholders have an interest. However, there may be times when other decision-making approaches are more appropriate. Indicate in this section what type of decision-making approaches the team will use and when. If applicable, teams are encouraged to use illustrations or flowcharts when describing decision-making processes. In addition, team decision-makers must be clearly identified in this section if not identified previously Process Improvement Procedures High performing teams continually improve. Explain how the team will monitor, implement, and track improvements to team processes and provide improvement suggestions to process owners outside the team. Changes in Team Membership Identify the process the team will use to address changes in team membership. Include recruitments, reductions, and replacements to team membership.

Meetings Team Decision-Making Process Identify how the team will make decisions. Consensus decision making is necessary for all decisions in which team stakeholders have an interest.
