he General Information test items shall include the following areas: Philippine Constitution, Code of Conduct and Ethical Standards for Public Offic mployees (R.A. 6713), Peace and Human Rights Issues and Concepts, and Environment Management and Protection.

ASSING GRADE

Have not been convicted by final judgment of an offense or crime involving moral turpitude, or disgraceful or immoral conduct, dishonesty, exa irregularity, drunkenness or addiction to drugs; Have not been dishonorably discharged from military service or dismissed for cause from any civilian positions in the government;

Applicant with pending administrative and/or criminal cases may take the examination and shall be conferred the eligibility if they successfully p same. However, this is without prejudice to the outcome of their pending cases. If they are found guilty of grave offenses, their eligibility shall be based on the penalties stated in the decision and pursuant to Section 58 (a) of the Uniform Rules on Administrative Cases in the Civil Service Resolution No. 010659, dated March 22, 2001; and Have not taken the same level of CS Examination in less than three (3) months.

Those who have already passed the same level of examinations or a comparable level of examination given by the Professional Regulation Commission, as well as passers of the Bar examination and those who graduated with honors need not apply.

PPLICATION REQUIREMENTS Applicants must accomplish properly the prescribed examination application form (CS Form No. 100, Revised 2008). Four (4) copies of identical I.D. pictures with specifications, as follows: a. b. c. d. e. f. Must show the subject applicant in standard close-up shot (from shoulder level up). Applicants should not wear eye glasses or any access may cover their facial features when they are having their pictures taken. Taken within the last three months prior to filing of application With full name tag The name tag must show the applicant's First Name, Middle Initial, Last Name, and Extension Name (if any)

The name tag should not be computer-generated, meaning, the applicant should have his/her picture taken while holding his/her name

Any valid ID such as Drivers License, SSS ID, GSIS ID, current Office ID, Postal ID, BIR ID, Barangay ID, Voters ID, Valid Passport or Police Clearanc contain the applicants picture, signature, birthdate, and the signature of the authorized head of the issuing agency. Photocopy of the same shall be to the application form to serve as reference for Room Examiners on the day of the examination. Examination fee of Php 500.00.

Failure to take the examination on your scheduled date shall mean forfeiture of your examination fees. No request for rescheduling is allowed.

pplication forms may be reproduced in 8.5 in. x 13 in. (21.6 cm x 33 cm) size bond paper provided the same format is followed. Applicants may plications in any of the CSC Regional/Field Offices.

XAMINATION PERMITS

ualified applicants shall be notified of their places of examination (school assignment) through a Notice of Assignment. If applicants still do not know the signment one (1) week before the examination date, they may inquire from the CSC Regional/Field Office where they filed their application.

HAT TO BRING ON EXAMINATION DAY

pplicants should bring the following on examination day: Notice of Assignment or Examination Permit Lead Pencil/s (No. 1 or 2) and eraser/s Ball Pen (Blue or Black)

The same valid ID cards presented during application stage (i.e. Drivers License, SSS ID, GSIS ID, current Office ID, Postal ID, BIR ID, Barangay ID ID, Valid Passport or Police Clearance)

WARNING The Civil Service Commission uses a highly reliable system to detect cheats. Any form of cheating in any civil service examination shall be considered a violation of Republic Act

and any person found guilty shall be administratively and criminally liable.

Those found ADMINISTRATIVELY LIABLE shall suffer the following penalties: For those in theGOVERNMENT SERVICE, dismissal from the service with all the accessory penalties of forfeiture of government benefits, cancellation of eligibility, bar from taking any government examination, and perpetual disqualification from re-entering government service. For PRIVATE INDIVIDUALS, perpetual bar from entering government service and from taking any government examination. Those found CRIMINALLY LIABLE shall be meted out the penalty of imprisonment of not less than six (6) years and one (1) day but not more than twelve (12) years and a fine of not less than Fifty Thousand Pesos (Php50,000.00) with the appropriate accessory penalties.
