Basic Documentation: 1. PROOF OF IDENTITY (mandatory) 2. PROOF OF BILLING ADDRESS (mandatory) 3. RECENT PHOTOGRAPH (mandatory) PROOF OF IDENTITY - MANDATORY Any one of the following can be submitted as proof of identity
1. Valid Driving license (permanent or learning) issued by Govt. of India only 2. PAN ID Card 3. Passport (Should be issued by Govt. of India only) 4. Passbook of any Bank, along with photograph 5. Ration Card with photograph 6. Credit Card with Photograph 7. Voters ID Card 8. Arms & ammunition license 9. Govt. ID Card mentioning residence address/signature 10. ESIC Cards mentioning ID /Address /Signature 11. Sr. Citizen ID Card mentioning Address/ID/Signature issued by competent Govt. authority 12. CGHS Card 13. Student Identity Card as per the policy note appended at the bottom of this doc (**) 14. Pensioner's Card /Army Canteen Card 15. Govt. Registered Sales Deed/other documents mentioning Photo /Address/Signature

PROOF OF ADDRESS - MANDATORY Any One of the following should be submitted along with the application
1. Valid Driving license (permanent or learning) issued by Govt. of India only 2. Passport issued by Govt. of India 3. Ration Card 4. Latest Bank statement showing last 3 months transactions / Passbook of any Bank from nationalized / non-nationalized banks 5. Vehicle Registration Certificate 6. Voters ID Card 7. Electricity/Water/MTNL/BSNL bill - last 3 months. 8. Govt. ID card mentioning residence address /signature 9. House tax payment receipt 10. LIC of India Policy Certificate 11. Arms & ammunition license 12. Student Identity Card as per the policy note appended at the bottom of this doc (**) 13. Bankers Verification on letter Head only with Stamp & Signature in Original 14. ESIC Cards mentioning ID/Address/Signature 15. Sr. Citizen Cards mentioning Address /ID/Signature 16. LPG Delivery Receipt (Should be computerized and within last 3 months) 17. CGHS Card 18. Valid Insurance Policy /Premium Receipt from LIC only 19. Pensioner's Card/Army Canteen Card. 20. House / Property Allotment Letter from Govt. 21. Sales / Lease / Partnership deeds, Rent agreements with Govt. registration or notarized on valid stamp papers 22. Latest Bill from any other Telecom Operator in India, along with any of the above address proofs (point 1 to 24), having a different address within India 23. ID cards of private/affiliated/recognized institutions such as BAR Council of India, ICWAI, Medical Association of India - as address proof , if supported by any of the above listed proofs (point 1 to 24) with a different address within India 24. Latest credit card statement / bill from credit card issuing bank, if supported by any of the above listed proofs (point 1 to 24), with a different address within India
