TINJAUAN SPESIFITAS DAN KESESUAIAN KODE DIAGNOSA

GASTROENTERITIS DENGAN ICD-10 PADA PASIEN RAWAT INAP DI RSUD
dr.GUNAWAN MANGUNKUSUMO AMBARAWA TAHUN 2022

Berdasarkan survei awal pada sepuluh rekam medis pasien rawat inap kasus
gastroenteritis di RSUD dr.Gunawan Mangunkusumo Ambarawa tahun 2021, 50%
kode diagnosa tidak spesifik atau idak sesuai. Penelitian ini bertujuan untuk
mengetahui spesifitas dan kesesuaian kode diagnosa gastroenteritis yang tertulis
pada dokumen rekam medis pasien rawat inap. Penelitian deskriptif dilakukan
dengan mengobservasi 84 dari 531 berkas rekam medis pasien rawat inap dengan
penyakit gastroenteritis tahun 2022. Teknik sampel dengan random sampling. Hasil
penelitian menunjukan 32 diagnosa tidak spesifik. Kode diagnosa pada 12 (15%)
dokumen tidak sesuai ICD-10. Faktor yang mempengaruhi spesifitas diagnosa dan
kesesuaian kode diagnosa adalah kelengkapan data pemeriksaan penunjang.
Ketidakspesifikan diagnosa terjadi karena dokumen rekam medis hanya
melampirkan pemeriksaan darah saja tanpa melampirkan hasil pemeriksaan feces
atau lainya yang dapat mengetahui penyebab penykit gastroenteritis. Faktor lain
yang berpengaruh adalah saat mengkoding coder hanya melihat diagnosa utama
tanpa melihat data anamnesa dan pemeriksaan penunjang. Unit rekam medis
sebaiknya mengembangkan pengetahuan petugas terkait audit koding dengan hasil
pemeriksaan penunjang atau laboratorium. Desain formulir perlu diubah dengan
menambahkan kolom untuk menunjukan tingkat derajat penyakit yang
mempengaruhi spesifitas diagnosa.

REVIEW OF SPECIFICITY AND ACCURACY OF GASTROENTERITIS

DIAGNOSIS CODE WITH ICD-10 AMONG INPATIENTS AT RSUD dr.GUNAWAN
MANGUNKUSUMO AMBARAWA YEAR 2022

Based on an initial survey of ten medical records of inpatients with gastroenteritis

cases at RSUD Dr. Gunawan Mangunkusumo Ambarawa in 2021, 50% of the
diagnosis codes were not specific or inappropriate. This study aims to determine the
specificity and accuracy of gastroenteritis diagnosis codes written on inpatient
medical records. Descriptive research was conducted by reviewing 84 of 531
medical records of inpatients with gastroenteritis in 2022. Sampling technique with
random sampling. The results showed that 32 diagnoses were not specific.
Diagnosis codes in 12 (15%) documents were not ICD-10 compliant. Factors that
influence the specificity of diagnoses and the accuracy of diagnosis codes were the
completeness of the supporting examination data. Diagnostic non-specificity occurs
because medical records only include blood tests and did not attach results of stool
tests or other tests that could determine the cause of gastroenteritis. Another
influencing factor was that the coder only observed the principal diagnosis to
determine the diagnosis code without looking at the history and supporting
examination data. The medical records department should improve staff knowledge
of coding audits with supporting examination or laboratory results and modify form
design by adding a column to indicate the degree of illness which affects the
diagnosis specificity.
