SURVEY- HOW AWARE ARE PEOPLE OF THE HEALTH HABITS THAT WILL KEEP
THEIR BODY SYSTEMS HEALTHY

Dear Survey-taker

Firstly, we would like to thank you for kindly taking time off to assist us. We greatly
appreciate it. We would respect your privacy and would like to assure you that all information,
particulars and responses would be kept in strictest confidence and be used solely for the
purpose of the results analysis.

We are conducting a survey on the topic, 'how aware are people of the health habits
that will keep their body systems healthy?' We have some questions below on your diet habits,
your everyday habits and your knowledge of the health habits which would keep your body
systems healthy. We hope that you would not be offended by any of the questions set.

We have prepared 7 main topic questions in this survey, although the survey itself
consists of a total of 19 questions. Please read each of the questions carefully and try to answer
as many as possible. A number of questions in this survey are questions with specific correct
answers, and they are made in this way to test your knowledge on good health habits.
