1. Pastikan anda datang maksimal 30 menit sebelum waktu batas registrasi atau pemeriksaan anda akan
dibatalkan.
Make sure you arrive a maximum of 30 minutes before the registration limit or your check will be
canceled.

2. Nomor registrasi ini digunakan untuk pengambilan nomor antrian.

3. Pemohon sertifikat kesehatan diwajibkan mengikuti Surat Edaran Kepala Balai Kesehatan Penerbangan
nomor : SE 028 Tahun 2018 tanggal 02 Oktober 2018 tentang Prosedur Permohonan Sertifikat
Kesehatan.
Health certificate applicants are required to follow the Circular of the Head of Aviation Health Center
number: SE 028 Year 2018 dated October 2, 2018 concerning Procedure for Application for Health
Certificate.

4. Jika anda akan melakukan pemeriksaan laboratorium, pastikan anda melakukan beberapa hal berikut :
If you are going to do a laboratory check, make sure you do the following:

1. Puasa selama 10-12 jam sebelum pemeriksaan.

Fasting for 10-12 hours before examination.

2. Tidak merokok.
Do not smoke.

3. Tidak minum obat.

Do not take any medicine.

4. Tidak berolah raga atau melakukan pemeriksaan Treadmill sebelum melakukan tes
laboratorium.
Do not exercise or do a Treadmill examination before doing laboratory tests.
