This QuickGuide is not intended as a replacement for the SPECTRALIS Manuals. Do not use the Spectralis without reading the full Operating Instructions. Please read the Cautions and Warnings in the Hardware Operating Instructions and always follow their contents. The Spectralis HRA is a confocal scanning laser ophthalmoscope (cSLO) for retinal angiography and reflectance imaging. Available imaging modes are fluorescein and ICG angiography, infrared and redfree reflectance, and fundus auto-fluorescence. The imaging modes can be used separately or simultaneously. The Spectralis OCT is a dual beam spectral domain optical coherence tomograph (SD-OCT) and cSLO system that acquires transectional images or volume scans of the retina and a fundus image simultaneously. A real time eye tracker couples cSLO and SD-OCT scanners to position and stabilize the OCT scan on the retina. Spectralis OCT and Spectralis OCT Plus are two version of the Spectralis OCT that differ only by the set of accessories. Camera head and power supply of Spectralis OCT and Spectralis OCT Plus are identical. Spectralis OCT Plus uses a different head rest and joystick than Spectralis OCT. Spectralis OCT has no control panel, no footswitch and is not upgradable to Spectralis HRA+OCT. The Spectralis HRA+OCT combines the features of the Spectralis HRA and the Spectralis OCT Plus. All six acquisition modes are available, separately or simultaneously with one other mode.

Eye Tracking, Heidelberg Noise Reduction All Spectralis devices obtain a high cSLO and SD-OCT image quality through Heidelberg Noise Reduction technology. Image Acquisition III


Preparations Pull the camera to the most backward position, ensure head support and camera objective are clean. Adjust the height of table and chair for each patient. Pupil dilation is recommended to increase the image quality. It is recommended to perform the Spectralis scan before other diagnostic measures are taken that irritate or dry the cornea. Explain the procedure and the fixation targets to the patient. Adjust the focal plane to the refraction (spherical equivalent) of the examined eye using the focus knob at the back of the camera. Start the software (see page 8). Create or open an existing patient file. Start a new examination. The Acquisition window will open automatically (for further information on the Acquisition window, please see pages 11 and 16). Control Panel Settings1 Start the laser by pressing the yellow button at the bottom right corner of the control panel screen. The main menu will open (see page 9). Control Panel Settings1 (continued) The following settings are typically used initially: Select the Infrared reflection mode (IR) on the control panel), turn the filter lever to R for the reflection mode. Select a scan angle of 30 degrees on the control panel. For the examination of highly myopic patients, an internal myopic lens may be used for fundus imaging in the More menu of the control panel.

Image Acquisition IV
Preparations (cont.) It is recommended to pre-set the default Acquisition Parameters. In the Acquisition window, open the menu Setup and choose the option Acquisition Parameters. The window shown on the right will open. Select high speed or high resolution (high speed recommended for standard applications). The Cyclic Buffer Size is important for movie acquisition available for all fundus imaging modes: Once the Acquire button on the control panel has been pressed, the system will save several additional fundus images from the cyclic buffer before the acquisition button was pressed and add them to the beginning of the movie. The number of cyclic buffer images saved is dependent on the selected cyclic buffer size and the scan resolution. The Automatic Image Brightness Control1 is recommended for users new to the device. It automatically increases/decreases the detector sensitivity so that the image is not too dark or overexposed. This feature can be enabled/disabled any time via the control panel2 (menu More). Manual control gives experienced users the best control over the image quality. Ask the patient to place his/her chin on the chinrest and to place their forehead against the headrest. Adjust the chinrest height so that the patients eyes are at the same level as the red canthus marks on the headrest posts. Use the central internal fixation target if appropriate, or select a different fixation target using the Select Target sub-menu of the control panel2 or via the Acquisition window3. Slowly bring the camera towards the patients eye, move the camera (up, down, right or left) to the center of the pupil and adjust the distance between the objective and the examined eye to approx. 14 mm between the front edge of the objective and the cornea. As soon as the laser beam enters the pupil, you will see the image of the fundus on the screen (Acquisition window, see also pages 11 and 16).

Camera Alignment - Fundus Image Turn the camera so that the structure of interest is visible on the screen. Move the camera slightly up/down and sideways until the image appears brightest and most evenly illuminated. Then, move the focal plane (using the focus knob) until the image is brightest. For less experienced users, we recommend to use the Automatic Image Brightness Control1. It is activated either via the Auto button in the More menu on the control panel2, default setting (see page 5), or in the Acquisition window3. Aim for even illumination, minimal artifacts and a centered macula, unless otherwise indicated. Manual Sensitivity Control1 If the image is overexposed (white) reduce the sensitivity using the sensitivity button on the control panel (see page 9). In order to optimize sensitivity, increase the sensitivity to the point just before saturation / blooming (i.e. white areas or white dots with colored specs) occurs using the Sensitivity / ART knob on the control panel2.

Camera Alignment - OCT Image4 First, align the camera using the fundus image as described above. For Spectralis OCT, the OCT live image is automatically opened. For Spectralis OCT Plus and Spectralis HRA+OCT, the OCT window is opened by pressing the OCT button on the control panel.

The optimum camera position is reached when no dark corners and overexposed areas are visible in the live image (see image to the right).

Then, move the camera towards the patient until a cross sectional image of the retina appears in the Acquisition window. If no OCT image becomes visible, adjust the scan depth by selecting a different eye length using the buttons in the Acquisition window shown on the right.

Camera Alignment - OCT Image (cont.) The bar above the OCT image will appear red if the OCT image touches the upper border. Move the camera further away from the patient if the OCT image is shown mirrored (upside down). If the OCT image is tilted in a horizontal direction, move the camera slightly left/right (if capturing a horizontal scan) or up/down (if capturing a vertical scan). The blue Quality bar in the lower part of the image indicates the signal strength1. The quality score range is 0 (bad quality) to 40 (excellent quality). If the score is 15 or less, the quality bar turns red. A long bar represents a high signal strength. To achieve optimum image quality, position the OCT image in the upper half of the Acquisition window. Move the camera slightly up/down and sideways until the fundus and OCT image appear brightest and most evenly illuminated. Fine tune brightness and sharpness of the fundus image using the focus knob. The optimum camera position is reached when no dark corners and overexposed areas are visible.

Acquiring and Saving Images To acquire images with the settings highlighted in blue, press the foot switch2, the Acquire button on the control panel2 or the button on the joystick3.

After acquiring images, save them using the Save images option in the top left corner of the Acquisition window. To end the acquisition session, exit the Acquisition window. The camera will automatically turn off. For Angiography, the timer is started using the Inj. button on the control panel4. When the Acquisition window is closed, the Show Timer window displays the duration since the last injection.

Patient Database, Starting an Acquisition


The Heidelberg Eye Explorer (HEYEX) is a common software platform for all Heidelberg Engineering imaging devices. Each device features image acquisition and Analysis modules, called plugins for the HEYEX. Starting the HEYEX Turn on the power supply (the power button is located on the front panel of the power supply), the external hard drives (the power button is located on the back side of both the Data and Archive drives) and the computer. Start the application by left double-clicking on the HEYEX icon on the desktop: or by using the windows start menu: Start Programs Heidelberg Eye Explorer Explorer. The Database window will appear. Heidelberg Eye Starting an Acquisition Create or Open a Patient File - To open an existing patient file, select the patient name and click on the New Examination button to start the new examination. Confirm that you wish to reexamine the patient with Yes. - To create a new patient record, click on the New Patient button. In the Patient File window, enter last name, first name, date of birth and sex. All other data is optional. Examination Data Window The Examination Data dialog opens before each exam, but can also be opened at any later stage using the Examination button in the patient file. The respective Device Type for the examination must be selected from the dropdown menu; all other data is optional. Eye Data Window On the left side, all patients of the patient database are listed. The right part of the window serves to select patients for new examinations and review or for batch processing (e.g. export). Double click on a patient name to select the file into the right part of the window. Closing the HEYEX window will shut down the program. This window enables the entry of the eye parameters for both eyes. For the Spectralis, only the cornea curvature is important; it will affect the measurement of distances and areas.

OCT Acquisition Window Scan Patterns1


Section Scan Select the scan pattern and orientation of the linear scan (vertical, horizontal line) by clicking on the buttons in the Acquisition window. Position and length of linear scans can be modified by draging the end of the scan line with the mouse. Circle Scan Activate the single scan pattern by clicking on the respective button in the Acquisition window. The circle is 12 in diameter2. For RNFL measurements, the respective fixation light is automatically activated. Move the circle so it is centered perfectly around the nerve head. The distance from the outer rim of the nerve head to the circle should be the same all around the circle. Volume and Star-Shaped3 Scans (cont.) The size of a volume or star-pattern scan can be changed using the corresponding arrows in the Acquisition window or the right/left cursor keys ( / ) of the keyboard. For volume scans, the up/down ( / ) cursor keys may be used to set the vertical extension of the scan. The number of scan lines can be selected via the scroll-wheel of the mouse, or by pressing the Shift and up/down cursor keys at the same time ( + or + ) Fast Volume Scans3 Volume scans are acquired in ART mode by default. However, ART may be switched off to acquire images in the Fast volume scan mode. To select this mode, press the the ART button in the Acquisition window. This will deselect the ART button (as shown on image to the right). Preset These buttons offer the possibility to select preset scan settings (e.g. scan angle, fixation light, density): Fast, HighResolution, HD-ROI, Post. Pole, ONH, RNFL, 7 Lines. By pressing on one of the Custom buttons (to the right) for several seconds, the current settings are saved.

Volume and Star-Shaped Scans Size and number of sections in the selected patterns are displayed in the Acquisition window. Furthermore, for volume scans, the distance between the sections is displayed. Star-shaped scans are always acquired in ART mode. In this mode, the vertical scan is used for targeting.

The OCT mode must be active.(see page 6). Not available for Spectralis HRA. The circle diameter in mm is dependent on image settings and the corneal curvature. For example, for corneal curvature of 7.7 mm, the circle is approximately 3.5-3.6 mm in diameter. . The Fast volume scan mode should only be used when strong eye movements make it impossible to acquire good quality ART volume scans. OCT Acquisition Window1 ART MEAN


With ART (Automatic Real Time) MEAN, a selected number of B-scans is averaged at each scan position. Activate the ART-MEAN mode by clicking on the Sensitivity/ART button on the touch panel or by pressing the joystick button for several seconds (Spectralis OCT). In ART mode, the live images are always displayed as small images in the lower part of the screen.

Blue pattern indicates the location of the OCT section in the fundus image. The blue arrow indicates the scan direction. Click in the image with the mouse to move the scan pattern (see previous page) .

ART Slider Set number of images (frames) that are used for averaging (maximum 100) with the slider. The chosen value is indicated below the slider. The blue part of the bar represents the current number of averaged frames processed.

The top and bottom of the OCT live image are marked by black lines to make it easier to adjust the camera position.

OCT Acquisition Window Follow-up1 I


It is possible to acquire a repeat of an OCT scan at the same location that was selected during a previous exam. Definitions Baseline image: The image of a series which was acquired first. Follow-Up image: All subsequently acquired images of a series. Reference image: Scan to which all other images of the series are compared. Initially, this is the baseline image. Later, this can be any other image of the series. The thumbnails of images in the patient file that are part of a progression series are marked with a special symbol in the bottom left corner. In the Analysis window (see page 20), the same symbol is displayed Preparing a Follow-Up Acquisition To be able to perform a follow-up acquisition, a previous OCT image from the HEYEX database must be defined as baseline or reference scan. To define a reference image, right click on the desired thumbnail in the Patient File and select Progression, Set Reference from the context menu. To start the follow-up acquisition, start the camera. Pressing the Follow-up button in the Acquisition window will bring up a work list window with previously selected reference images of the examined eye. Click on the reference image desired and press OK. Note: More than one reference scan can be defined for every patient, for example if different scan orientations are used. It is also possible to select a reference image from an image series (as volume scans or star-pattern scans; see page 39 Extracting Single Images).

OCT Acquisition Window Follow-up1 II


To capture the follow-up image, press Acquire on the control panel or press the footswitch or the joystick button (Spectralis OCT). To cancel a follow-up acquisition, press on the Follow-up button again. A change of the scanning parameters (e.g. scan angle) will also automatically cancel the follow-up acquisition. Changing or Removing the Reference Image In the Analysis window, go to the menu bar and select Progression Set Reference. In the Patient File (Image Viewing) window, right-click on any followup thumbnail image and select Progression Set Reference from the context menu. All thickness graphs in a progression series will now be compared to the new reference image. Note: On printouts, the new reference image is identified with a red border in the Recent Follow-Ups or All Follow-Ups report (standard layout or large OCT). To remove a reference image, use the option Remove Reference from the Progression menu in the Analysis window2.

Acquiring a Follow-Up Image The reference images will now be visible as small images in the Acquisition window below the live images. The scan type, orientation of scan and fixation target chosen for the baseline exam are automatically activated. Move the camera so that the live fundus image is showing approximately the same area on the retina as the baseline image. As soon as the overlap between live and reference image is sufficient, a green frame appears on the reference image, indicating the borders of the reference image. Optimize the camera position for maximum overlap between live and reference image . The blue scan line and green box will automatically follow the eye movement. However, if there is too much eye movement and the live image can not be matched to the baseline image, the blue line will appear as a dotted red line.

To guarantee the safety of the user and the patient at all times, a limit has been imposed on the maximum period of time for which the laser beam can be switched on for a single patient. If this limit is exceeded, the screen shows the message: 'Laser Safety; laser timed out'. Image acquisition for this patient is automatically interrupted and can only be continued after a specified waiting period (about 3 hours). Activate ART Composite mode by selecting Composite in the control panel menu and pressing the sensitivity button of the control panel. Start the acquisition by pressing the Acquire button on the control panel or by using the foot switch. It is possible to acquire multiple images during the same ART COMPOSITE session. To switch the ART COMPOSITE mode off, press the Sensitivity / ART button again. Patient File, Opening Images


Patient File Viewing Images (Analysis Window) To open an image, double-click on the thumbnail or select Display from the context menu. The selected image will be displayed in a separate Analysis window with its own menu and tool bar. Open Analysis windows (from one or several patients) can be grouped or closed automatically. Use the Window menu with the options Cascade, Tile and Close All in the Analysis window or in the Patient File. In the Analysis window, the black arrow buttons shown on the right may be used to browse through images of a visit tab or lightbox*.

Lightbox The Patient File contains the data and images of a patient. Each thumbnail represents an image or scan of the right or left eye. Images can be viewed, analyzed, printed, imported, exported and archived. All images from one acquisition session are stored under the same visit tab. To open the Context Menu, click on a thumbnail image with the right mouse button. Choose the desired menu item by selecting it with either the left or right mouse button. From the patient file, patient data regarding diagnosis, referring physician, operator, eye data, etc. can be accessed via the buttons Patient, Examination and Eye Data.

Examination Data / Comments In order to write a comment, click on the Examination button in the Patient File to open the Examination Data window and select the Diagnosis tab. The text in the Comment field will be automatically inserted on the printout if the respective option is ticked in the Print Spectralis Report window (see page 36). If the text is too long, it is automatically truncated.

The third button1 is used to compare a reference B-scan to another B-scan within a progression series1. This can be from two volume scans or section scans.

Zoom & Pan Modes The zoom factor represents the ratio of monitor pixels to image pixels. If the Auto option is selected in the drop-down list, the image is automatically scaled to fit the window. In this case, if the window size is changed, the image is re-scaled. If a fixed zoom factor is set (e.g. 100%), changing the size of the window will change the image frame and not the scale. When the image is not shown in full size, scroll bars enable viewing of the full image and the button for the Pan mode is active. Preferences Open this window by using the option Preferences from the Options menu in the OCT Analysis window. The tab Analysis Center Options offers selection of general default settings. Select the default tab for the Analysis window using the drop-down menu. Info bubbles are activated by moving the mouse over a button or fuction. To turn off the info bubbles, remove the checkmark for the option Display info bubbles. The tab Default Image Settings offers the possibility to set the default color settings for the OCT and HRA image.

Viewing OCT Volume Scans For volume scans, a green frame in the fundus image indicates the borders of the scanned area. The total number of B-scans and the number of the currently displayed B-scan are shown in the upper left corner of the fundus image. The green arrow indicates the direction of the OCT scan. If the scans in this area are densely, spaced only the active OCT scan is indicated. Choose a different scan from the series by left-mouse clicking on the fundus image within the green frame. If the scans of a volume scan are less . densely spaced, the inactive scans are indicated in dark green. Left-click on these lines to view the corresponding OCT scans

For the 3D View tab, the option Automatically adjust vertical position may be selected. In this case, the fundus image is automatically positioned slightly below the lowest segmentation line.

Thickness Profile Tab RNFL


Classification The pie chart displays classification results for the 6 sectors of the optical disc (T, TS, TI, N, NS, NI), together with a global classification. The black value gives the average RNFL thickness measurement value for each sector and the global average (G). The green value below in brackets indicates the average value from the normative database. If the measured average thickness in a sector is within the 95% confidence interval of the normal distribution (p>.05), the sector is colored green to indicate it is within the normal range. If the average thickness is outside the 95% confidence interval but within the 99% confidence interval of the normal distribution (.01<p<.05), the sector is colored yellow to indicate borderline. If the average thickness is outside the 99% confidence interval of the normal distribution (p<0.01), the sector is colored red, indicating outside normal limits. An outside normal limits classification means that less than 1% (1 out of 100) of all normals from the database have thickness values this low, indicating a high probability of abnormality. The colored bar below the pie chart indicates the overall classification. The bar is red ("outside normal limits") if one or more sectors or global is outside normal limits, and yellow ("borderline") if one or more sectors or global is classified as borderline but none as outside normal limits. If all sectors and global are classified as within normal limits, the bar will appear green ("within normal limits").

In the Thickness Profile tab1, the RNFL thickness profile can be displayed. From the dropdown menu in the center of the window, select the option RNFL to view classification results. Thickness Profile The profile on the lower right of the window presents the RNFL thickness detected along the circular scan, compared to values from the normative database. The normative database comprises 170 eyes of 169 Caucasian subjects (Mardin et al., University of Erlangen, Germany, publication in preparation). The black curve indicates the measured values. For followup examinations, the values of the reference exam are indicated as gray line. The green area represents the 95% normal range. The green line within the area indicates the average thickness values from the normative database. The yellow area represents values outside the 95% confidence interval but within the 99% confidence interval of the normal distribution (.01<p<.05), indicating borderline classification, and the red area represents values outside the 99% confidence interval of the normal distribution, indicating outside normal limits.

Modify the Segmentation Line (cont.) Click the segmentation line you would like to modify with the left mouse button. The color of the selected segmentation line turns to green and red drag marks are visible. To modify the segmentation line, click one of the red drag marks with the left mouse button and drag it to the desired position. The segmentation line will follow the position of the dragged mark. If necessary, you can insert additional drag marks. Open the context menu by right clicking on the line and then click on the option Add point. Alternatively, double click on the line with the left mouse button to create a new drag mark. To delete a drag mark, select Delete Point from the context menu. It is recommended to save your changes before selecting the second segmentation line. To discard the previous changes applied to the segmentation line, click the Undo Changes button. To save the modifications, click the Save changes button under the OCT image. To exit the edit mode, click the Enter/exit Segmentation line edit button again.

Automatic Segmentation To reset a previously modified segmentation line back to its automatic segmentation, enter the segmentation line edit mode, select the appropriate segmentation line, right-click on the segmentation line, and select Set to Auto-Segmentation from the context menu.

Manual Segmentation If the software could not create a segmentation line, the Create Segmentation Line button appears under the OCT image when the segmentation line edit mode is started. To add the missing line, click on the arrow button and select the type of segmentation line you would like to add from the appearing context menu. A horizontal segmentation line will be created in the OCT image. Since it initially contains three drag points, it is recommended to add more points to the line before dragging these points to their final positions. Currently three options are available depending on the missing segmentation line: Create ILM Segmentation (Internal Limiting Membrane), Create RNFL Segmentation (lower boundary of Retinal Nerve Fiber Layer) and Create BM segmentation (Bruchs Membrane). A manually modified segmentation line is marked with an m at the left end. When you move the mouse over a manually modified segmentation line, the last edit date is displayed in the status bar at the bottom of the window and in the info bubble (if activated; see page 22).

To segment sharp angles, try adding additional drag marks (shown by red arrows in the above image) to the corners where you do not want the segmentation line to be affected. This will help to stabilize the segmentation line that is beyond this sharp angle.

Fundus Image Analysis Window II


Menu Image Overlay Tools Overlays are stored in addition to the image contents. They can be copied and pasted to corresponding images of the same eye. Overlays can be removed at any time by selecting the option Remove all Overlays from the menu Overlay.

ETDRS Grid Overlay The grid is composed of four concentric circles with radius 360m (dashed line), 600m, 1800m and 3600 m. The two bisecting lines are indicated. The window size can be adjusted with the mouse It can be moved by drag and drop. When the cursor is within the center of the inner circle and the left mouse button is pressed, the cursor symbol will turn into a cross and enables movement of the grid. Similarly, the grid can be rotated by drag and drop on any of the concentric circles using the left mouse button. The ETDRS context menu is opened by a right mouse click on any of the grid lines. The option Overlay Properties enables the change of the color of the grid or the font. Note: The ETDRS Grid Overlay function is dependent on image scaling. Please make sure that the cornea curvature is entered correctly for the relevant eye in the Eye Data window (see page 8).

Image Settings, Screen Printing


Fundus Image Analysis Window Open the control window by selecting the shortcut icon or the option Brightness & Contrast from the Image menu. Brightness and Contrast can be adjusted together by moving the red dot in the grey square (up and down = brightness, left and right = contrast) or separately using the respective edit field. The Reset button resets all controls to standard values. Additionally, image Sharpening and Noise Reduction can be modified in the respective tabs. OCT Analysis Window (cont.) Image settings can also be adjusted without opening the Brightness & Contrast window, press the Alt-key when the mouse is on one of the respective images to change contrast (left/right) and brightness (up/down, fundus image only). The OCT image can be shown in Grayscale (Black on White or White on Black) or Color (Spectrum, Heat, High Frequency). The Reset button resets all controls to standard values.

OCT Images Open the control window by selecting the shortcut icon or the option Brightness & Contrast from the Image menu. For the OCT image, contrast can be adjusted using the slider. For the fundus image, brightness and contrast can be adjusted together by moving the black dot in the white square (up and down = brightness, left and right = contrast) or separately using the respective slider below the white square.

During Acquisition, the OCT Color Scale can also be changed by selecting this option from the Setup menu.

Printing I
Printing Reports To print one or multiple fundus or OCT images on a report, select the desired thumbnail images into the lightbox of the Patient File, select them all and select Print from the context menu. The Print Spectralis Report window will open. The report types available in the Print Spectralis Report window depend on the types of images selected for printing, as well as the context from which the print function was started. In the section Reports, select the type of Report or number of images per page2, report language, and information to be printed on the printout and page layout (see section Options below). Printouts can be previewed using the button Preview in the Print Spectralis Report window. Options Display Page Header: Title of the report and custom header (if implemented, see Software Manual) are displayed. Display Patient Information: Patient name and other patient data. If all printed images are from the same examination date, this date will be displayed together with the patient data. Display Diagnosis and Commen: If this Option is selected and if all printed images are from the same examination date, text entered in the Comment field! of the Patient Data window will be displayed in the Notes section on the bottom of the report. On progression printouts, the information of the reference exam will be shown. Provide Space for Notes: Shows or hides the Notes box at the bottom of the printouts. Add Page Margins: Remove this option to avoid additional software-generated margins. Automatic Page Orientation: Page orientation (portrait or landscape) is automatically set according to the report type (e.g. Selected Image Only reports are always printed in the landscape orientation, Single Visit Reports are always printed in the portrait orientation).

Printing II
OCT or Fundus Images Overview Reports When printing multiple images from the Patient File (or lightbox), an Overview Report will be printed. The number of images per page can be selected in the Print Spectralis Report window (1, 2, 4, 6, 8, 9,12,16 images/page, OCT scans or HRA simultaneous images count as 2 images each).The best setting for the number of selected images will be chosen automatically. If the Automatic Page Orientation check mark is set, the orientation for the 2 and 8 images per page layout will be landscape, all other layouts will be printed in portrait mode. OCT Reports Single Visit Report The Single Visit Report is only available for single OCT B-scans. For volume or star pattern scans, a single scan must be selected in the Analysis window or by using the Expand option in the Patient File.

Follow-up Reports (Retinal Thickness Change Analysis Reports) The report All Follow-ups is available for any OCT B-scan that is part of a progression series. Recent Follow-ups is only available when the selected image is a follow-up image acquired after the reference image (see page 14). The reference image is highlighted with a red frame. Both report formats are available with enlarged OCT images (see lower printout).

Thickness Map Report Open the Thickness Map tab and use the Quick Print button or the menu option Image / Print Report. This report is only available for volume scans. RNFL Basic Report OU (cont.) values for the respective sectors and global (G). In the middle section, the Thickness Profiles are displayed. The green area represents the 95% normative range. The green line indicates the average thickness values from the normative database. The yellow areas represent values outside the 95% confidence interval but within the 99% confidence interval of the normal distribution (.01<p<.05), indicating a borderline classification, and the red areas represent values outside the 99% confidence interval of the normal distribution,indicating outside normal limits. For follow-up examinations, the values of the reference exam are indicated as gray line. The lower section comprises a diagram presenting asymmetry values between eyes as well as four pie charts indicating average RNFL height measurements for the different sectors and the global values (G). If the average height measurement falls outside the 95% confidence interval of the normal distribution (p>.05), the respective sector is colored yellow or red. The overall classification result for each eye is presented as a green, yellow or red bar on the bottom of the page.

3D-View Report Open the Thickness Map tab and use the Quick Print button or the menu option Image / Print Report.

RNFL Basic Report OU This printout format is designed to provide a comprehensive analysis of the RNFL in an easy to read format for clinical interpretation. It is only available on circle scans. To print both sides, select a circle scan of each eye and exam date in the patient file, then select the option Print.from the context menu (right mouse button). Asymmetry comparisons between both eyes are given in the two gray pie charts in the upper section. The numbers indicate the difference of both thickness

Printing IV, Expanding Series, Extracting Images


Additional OCT Printing Features OCT images can also be printed from the Analysis window using the option Print Report from the Image menu. Depending on the currently opened tab in the Analysis window, the Print Report button either prints a single image or single visit report, the current 3D-View Report or Thickness Map Report. Please note that 3D views or Thickness maps can only be printed when the respective tab is open. Expanding Image Series Fundus and OCT images from image series* can be viewed as individual images using the Expand option from the context menu. The images from the series will now appear as individual thumbnails. A new tab will open, displaying the images from the series as individual thumbnails. This tab is marked with a yellow folder symbol in the top left corner. Extracting Single OCT Images from Image Series Single OCT images can be extracted from a volume or star pattern scan and saved individually. These single scans can then, for example, serve as baseline (reference) image for follow-up examinations. From the Patient File, select a volume or star pattern scan and select Expand (see above). A new tab will open, displaying the single images from the scan series. This tab is marked with a yellow folder symbol. Select a single image and choose the option Extract Image from the Context Menu in order to save this OCT image separately Alternatively, select the menu item Extract as Single Image from the Image menu in the Analysis window (only available in the Display tab). .

To return to the Patient File, click on the yellow folder symbol.

Monitor Setup, Image Information, Export I


Monitor Setup Tool This tool offers a gray scale target for optimal monitor setup, which is especially important for LCD screens. In the Analysis window, the Monitor Setup Tool window can either be opened via the shortcut button, or by choosing the respective option from the Image menu. In order to optimize the monitor settings, follow the instructions in the Monitor Setup Tool window. Export of Reports Reports can be saved as .jpg or .bmp image files. In the Analysis window, use the Quick Print button or the option Print Report from the Image menu. In the Print Spectralis Report window, select the report layout and options and press the Preview button. In the Print Preview window, use the Save as button to save the report. The Save as bitmap window gives the opportunity to select the range of pages to be saved.

Image Information Scan parameters of the OCT and fundus images are available in a separate window. It is opened via the Info shortcut button or the option Image Information of the Image menu in the Analysis window. E2E File Export1 To export images and other data in an examination as an E2E file, select the desired thumbnail image(s) from the Patient File window, and select the item Export as E2E from the Context Menu in the Patient File. The Batch Export E2E feature in the Database window enables export of multiple patient records at once.

Export II (Graphics Files)


Graphics File Export Images can be exported as graphics files. Use the Export as Picture option from the Context Menu or the menu item Export as picture in the Image menu of the Analysis window. 1. Context Menu or Display: The fundus and OCT images are displayed next to each other in full size, without further control elements. The y-scale of the OCT scan can be selected (1:1 pixel or 1:1 m ), and it is possible to down-size the fundus image to the same height as the OCT image (496 x 496 pixels). 2. 3D View: The content of the 3D window is exported without control elements. 3. Thickness Profile: The displayed . window is exported including the diagram. 4. Thickness Map: The displayed window is exported including the thickness map.
Save as

File Formats Images can be exported in .bmp, .jpg., .tif or .png format. Fundus image movies can be exported as AVI files. For export of images as TIF files, it is possible to select a compression method (None / LZW / Packbits). This reduces the size of the exported image file without loss of image information. Additional Information Below the image, the following information can be displayed optionally: Patient Name, ID, date of birth, ancestry Examination Date, OS/OD, diagnosis Image Image type, time In the Save as... window, select the option Add Info Block and choose the Displayed Patient Info by ticking the respective boxes.

General Settings, Archiving, Software Versions


General Settings In the Database window or in the Patient file, use the menu item Options from the Setup menu to open the Options window. In the caption line of the Analysis window, the image title is displayed in addition to patient name and acquisition date. If image numbers are activated via the menu option Setup / Options, this image number is also displayed. Icons: On the left side of this tab, the icon size and spacing in the Patient File can be chosen. Settings: Closing the window of the HEYEX will shut down the program. As standard setting, you will be asked to confirm that you wish to close the program. This confirmation window can be switched off. Archiving Archiving creates a copy of all of your data on a FireWire drive. This protects your data from a hard drive crash. In the Database menu of the Database window, the function Archive Images starts the process to archive the data to an external media.

Software Versions To check the versions of the Heidelberg Eye Explorer as well as the acquisition and viewing modules, please open the About box using the Help / About menu item in the Database window. The software release 5.0 includes Spectralis Acquisition Module 5.0, Spectralis Viewing Module 5.0 and Heidelberg Eye Explorer software version 1.6.1.
