DELEGATION Process of transferring a selected nursing task in a situation to an individual who is competent to perform the specific task Professional

ional nurses delegate tasks to unlicensed assistive personnel (UAP) and licensed practical nurse (LPN) Nurse Practice Acts, by the Boards of Nursing, define what can be delegated and what can be performed by RN, LPN, UAP Even if task is delegated, the RN maintains accountability for overall care for client 5 rights of delegation: o Right task o under Right circumstances o to the Right person o with the Right direction and communication o Right supervision and evaluation

Guidelines:
ALWAYS maintain client safety Validate the persons understanding of the directions or task that is delegated RNs are responsible for assessing, evaluation, teaching, making decision about client care- these tasks cannot be delegated Delegate care for stable patients with expected outcomes; tasks that involve standard, unchanging procedures Determine which tasks can be delegated to UAP o Assist with ADLs (Bathing, feeding, turning, transferring, ambulating, grooming), maintaining safety, I&O, CPR, apply clean dressings, taking VS, weighing; with additional training: glucometer, phlebotomy Determining which tasks cannot be delegated to UAP o Administering medications or oxygen, assessment, care of invasive lines, client education, creating care plan, evaluation of care effectiveness, interpretation of information and data, making nsg diagnosis, performing sterile procedures, administering NG feedings, performing triage Determine tasks to be delegated to LPN o Delegate and supervise other LPNs in the delivery of care within their legal scope of practice and level of competency, can supervise (UAP), data collection, administer IV saline and heparin flushes, calculate/ adjust IV flow rate Determining which tasks cannot be delegated to LPN o Independently develop the nursing care plan, administer IV chemotherapy, administer any direct IV push medications, administer an IV fluid bolus for plasma volume expansion, except in the outpatient, access any form of central line, do triage, case management

Factors to consider when deciding to delegate or assign activities: Potential for harm: The greater the potential for harm, the more the RN must provide care Complexity of task: Activities involving more complex psychomotor skills and requiring expert nursing assessment and judgment should only be performed by RN Problem solving and innovation required: If a problem is suspected, does it require individualized problem solving to achieve a successful outcome? Unpredictability of outcome: If patients response is unpredictable/ status is unstable, care should be provided by RN
