MHC My primary position is out- patient Social Worker in the Behavioral Health Servi ce, Central Intake Unit (CIU) and the Emergency Department. Triaged numerous pat ientas daily to determine their need for Addiction Recovery and Mental Health Se rvices. At times patients needed immediate admission through the Emergency Room , detox from alcohol and drugs, had minor to major psychiatric issues, family pr oblems, economic problems, social problems, issues of abuse and so on. Many of o ur vets had prolonged PTSD that had never been treated. Others are young OEF/OIF vets with PTSD and/or Poly Trauma issues. It is also common to treat vets with Psychotic Disorders, Bipolar/ Mood Disorders, Depression, Personality Disorders, Addictions and Adjustment Disorders. This position has been one that includes a great deal of crisis intervention, multi-tasking and quick decision making. T his has been a very fast paced work environment which requires cooperation from a Multidisciplinary team, particularly the psychiatrists. October 2001 to February 2004: San Francisco Veteranas Medical Center San Francisco, CA Clinical Social Worker- Out-Pt. Primary Care Out-patient Social Worker In Ambulatory Care Clinics, including the Womenas Clinic, assessment, treatment, referral and case management of veterans with social work needs. Some of the n eeds included assistance with advance directives, income/benefits/financial issu es, care giver support, community resources, and home support. I provided indiv idual, group and family sessions with patients according to their needs. I also co-facilitated a transgender support group for vets and provided individual psyc hotherapy for some patients. I participated on several committees focusing on h omelessness and affordable housing. It was called aLinkagesa, a quarterly meetin g for veterans between the ages of 45 and 55 years of age. The goals of this po sition have been to decrease length of stay, increase longevity in the community , and to avoid unnecessary costs for socially driven admissions. Medicine This was my first social work position that was purely in a medical setting (no psyche.). I was responsible for 34 surgical beds. This included the Surgical I ntensive Care Unit. For the first four months I was the only social worker on a unit that normally had two social workers. I worked with in a"patients, outpat ients and their families from the following services: General, Thoracic, Vascul ar, Plastics, ENT, Ophthalmology, Orthopedics, Urology, Podiatry, Neurosurgery, GYN. This position included the skills of education, assessment, discharge plan ning, adjustment planning, family counseling, bereavement counseling and end of life planning. This was a very fast paced and stimulating position. August 2000 a" April 2001 San Francisco Veteranas Hospital San Francisco, CA Clinical Psychiatric Social Worker- In-Pt. Psyc. My position as Clinical Psychiatric Social Worker at the SFVAMC was much like my other experiences on an in-patient psychiatric ward except it was working stric tly with Veterans. I was the social worker for a 12 bed adult unit where my dut ies included: psychosocial assessments, case management, individual and family counseling discharge planning, and interdisciplinary team consultations. Conser vatorships were also a large part of my job description. This was my first posi tion working strictly with Veterans and their families. I gained a deeper under standing for their particular issues and needs. I found that working with Veter ans was very interesting and fulfilling. They were all related to mental health, add ictions, counseling and social work. They took place at Francis Scott Key Medic al Center (now known as Johns Hopkins Bay view Medical Center), Wyman Park Healt h System, Baltimore City Health Department and Sheppard and Enoch Pratt Psychiat ric Hospital.
