Classification Antibiotic

Dosage 8-14 g/day

Action Interferes with cell wall replication of susceptible organisms

Uses Skin and structure infections Intra abdominal infections Pneumonia Gynecologic infections Soft tissue infections Otitis media Sinusitis Meningitis Septicemia

Contraindications Do not use solution in infants, children less than 4 years of age, pregnancy, in liver or kidney failure, and in those treated with disulfiram or metronidazole

Adverse effect CNS: lethargy, hallucinations, anxiety, depression, twitching, coma, seizures GI: nausea, vomiting, diarrhea, increased AST, ALT, abdominal pain, glositis, colitis, pseudomembranous colitis GU: oliguria, proteinuria, hematuria, vaginitis, moniliasis, glomerulonephritis, dysuria HEMA: anemia, increased bleeding time, bone marrow depression, granulocytopenia, leukopenia, eosinophilia SYST: anaphylaxis, serum sickness, toxic epidermal necrolysis, StevensJohnson syndrome

Nursing management Assess patient of previous sensitivity reaction to penicillins or cephalosporins Assess patient for signs and symptoms of infection Complete C&S before beginning product therapy to identify if correct treatment has been initiated Assess for allergic reactions Monitor blood studies Monitor electrolytes Assess bowel pattern daily Monitor for bleeding Assess for superinfection
