PER : ANUGERAH WANITA DAN SUKAN MAJLIS OLIMPIK MALAYSIA (MOM) 2007 Sukacita dimaklumkan bahawa Majlis Olimpik Malaysia (MOM) melalui Jawatankuasa Wanita Dan Sukan telah mewujudkan anugerah yang dinamakan Anugerah Wanita Dan Sukan. Anugerah ini diadakan setiap tahun untuk menghargai dan mengiktiraf individu atau organisasi yang telah banyak membantu membangun, mempromosi serta mempertingkatkan peranan Wanita dalam Sukan di Malaysia. Malaysia mempelawa Persatuan/Jabatan/Organisasi Y.Bhg. Dato/Datin/Tuan/Puan mencalonkan individu yang layak untuk dipertimbangkan bagi anugerah tersebut mengikut kriteria-kriteria yang telah ditetapkan di dalam borang yang dilampirkan bersama-sama surat ini. Oleh itu, Majlis Olimpik Sehubungan itu, diharap Y.Bhg. WOMEN & SPORTS AWARD 2007

(Achievement award for contributions to the development of Women & Sports in Malaysia) 1. Objective To give recognition to any person, whether individual or body corporate, who has contributed significantly outstandingly and positively to the promotion, development and advancement of women in any sport and or any sport-related activities in Malaysia. 2. The Award 2.1. 2.2. The award shall be known as the Women & Sports Award. The Award shall consist of: (i) (ii) 2.3. a challenge trophy (with a replica); and a certificate

The Award shall be on an annual basis BUT the Committee reserves the right not to give the award in any particular year if it is of the view, at its discretion, that there is no suitable recipient.

Nominations 3.1. Nominations shall be in the standard form issued by the Women & Sports Committee of the Olympic Council of Malaysia, and may be made by any of the following: (i) (ii) (iii) 3.2. any school, college, university or any private or public institute of learning; any duly registered club or organization; or any other individual or body corporate or organization, private or public

Nominations must be with the consent of the nominee, and must be accompanied by a curricula vitae or brief write-up of the nominee in support of the nomination, together with relevant supporting certificates and documents of achievements and or contributions, and two (2) 2 x 2 photographs of the nominee. All nominations must reach the Olympic Council of Malaysia Secretariat by 20th April 2007.

Selection Criteria 4.1. Candidates must satisfy the following criteria: (i) active involvement in sports or sports-related activities for at least five (5) continuous years, immediately preceding the year of the award (2006) and inclusive of the year of award; AND has shown significant achievements for, and contributions to the advancement of women in any sport or sport-related activities in the country; OR has shown significant contribution of time and effort towards the promotion, development and advancement of women in any sport or sport-related activities in the country, through leadership and or administration.

Selection Committee 5.1. The Selection Committee for the Award shall be chaired by the Chairman of the Women & Sports Committee, and shall comprise of such number and such members as shall be endorsed by the Board of the Olympic Council of Malaysia. Deliberations of the Selection Committee shall remain confidential. The decision of the Selection Committee shall be subject to the endorsement of the Board of the Olympic Council of Malaysia.

QUALITY OF INFORMATION It is important that the information on the Nomination Form is clear and concise. It will be used by the Selection Committee to draw up the shortlist for interview. This section may include comments on additional activities, supporting Malaysian activities e.g. skills and experience in conducting/organizing, lectures, forums, seminars, conferences involving young people or associations/corporate bodies, to create and increase awareness and development of women in sports and sports related activities.
