SKILL STEPS: 1. Label the container with residents name and room number. Record the time and date the specimen is collected. 2. If the resident has eaten recently, rinse mouth or assist with oral hygiene. 3. Instruct resident to take several deep breaths, and cough up the sputum. Ask the resident to cover the nose and mouth with tissue while coughing. 4. Try to bring up fluid or sputum from chest. 5. Have the resident spit directly into the specimen container. Only the outside of the container should be touched. Collect 1 to 2 tablespoons of sputum.

Clean and return equipment and supplies, if applicable. Remove gloves, if wearing. Position resident comfortably. Place call light within reach. Lower bed to safe position for the resident. Leave room neat. Wash hands. Document. Report abnormal findings to licensed nurse. Take specimen in a proper bag and the requisition slip to the collection station or specific area as directed by laboratory. Under optimal conditions the specimen should be collected directly into the container or use clean/unused bedpan urinal or specimen pan. Assist the resident to a comfortable position to urinate. Instruct the resident not to put toilet tissue in bedpan, but in trash. Fill in label with residents name, room number, date, and time of specimen collection. Take bedpan/urinal into bathroom. Pour urine into new or unused graduate and measure if resident on intake & output. Pour the urine from graduate into specimen container, filling it three fourths full if possible. Place lid and label on container. Pour leftover urine into toilet and flush.

MANUAL SKILL: Collecting and Identifying Specimens (Sputum, Urine and Stool)

SKILL STEPS: 1. 2. 3. 4. 5. 6. 7. 8. 9. Label the container with residents name and room number. Record the time and date the specimen is collected. Assist the resident with toileting. Use a clean or unused bedpan, bedside commode or specimen pan. Ask the resident to discard toilet tissue in the trash. Give the resident an option of privacy. Give resident the call light and wash hands if leaving the room. Return to the room when the resident signals or needs a safety check. Remove pan and take to the bathroom. Use tongue blade and collect about 2 tablespoons of feces, put in specimen container. Put the lid on the container. Place the tongue blade in paper towel and discard in trash.
