A. FINAL BID FORM

B. CHANGES TO THE BID FORM


1. TOPSOIL Revised description to:
ITEM 161.COMPOST MANUFACTURED TOPSOIL (BIP) (4")

2. STREETLIGHTS Bulbs are to be 150 watt (120\240 VOLTAGE):

3. TRAFFIC RAILING Railing is now separated by railing on bridge and railing on top of MSE Walls:

MAINTAIN TEMPORARY RR CROSSING MATERIALS FOR INSTALLATION BY UPRR Revised description to: MAINTAIN TEMPORARY RAILROAD CROSSING. Also, see answers on CivCast. 7. ALTERNATE BID ITEM QUANTITIES Quantities and descriptions for the railing have been revised.

C. CHANGES TO THE SPECIAL PROVISIONS


1. CORRECTION TO THE COMPLETION DATE FROM FEBRUARY 14, 2012 TO 2013:
26. WORKING DAYS\CALENDAR DAYS: This will be a calendar day contract and the number of calendar days (380) is the schedule for this Project and will be specified in the Agreement. The OWNER intends to start construction on January 31, 2012 with the successful BIDDER, and have the bridge completed and the road open for traffic from US 377 to Blythe Bridge Drive by February 14, 2012. If the Project is not started on January 31, 2012 due to delay by the OWNER, the completion date will be adjusted to reflect the actual starting date. If the CONTRACTOR is not completed by February 14, 2013, liquidated damages will be deducted from the Contract payment amount at the rate of $800.00 per calendar day. The ENGINEER will perform the final acceptance inspection and notify the CONTRACTOR of acceptance. Final acceptance is made when all work is complete and the ENGINEER, in writing, accepts all work specified in the Contract.

2. BID GUARANTY: All bids must include a bid bond or guaranty check. The guaranty check must be payable to the MERITAGE HOMES OF TEXAS, L.L.C. and must be a cashiers check, money order, or tellers check drawn by or on a state or national bank, a savings and loan association, or a state or federally chartered credit union. The check must be dated on or before the date of the bid opening. Post dated checks will not be accepted. The type of check or money order must be indicated on the face of the instrument and the instrument must be no more than 30 days old. The Owner will not accept personal checks, certified checks, or other types of money orders as a bid guaranty. The sample Bid Bond form has been posted to CivCast.

3. SAW-CUT GROOVING OF CONCRETE BRIDGE SLAB AND APPROACH SLAB: PER TXDOT ITEM 420.4, PAGE 508: Saw-cut grooves in the hardened concrete of bridge slabs, bridge approach slabs, and direct-traffic culverts to produce the final texturing after completion of the required curing period. SAW-CUT GROOVING OF THESE ITEMS WILL BE REQUIRED.
