Appearance and Behavior

a. Attitude Mr. J. J is a friendly man and he gets along well to the other patient in the institution. As we have our interaction he says that sometimes if there is misunderstanding inside the institution he prefer to talk with the person involve in a nice manner rather than shouting and making some issues about it. I have also noticed that Mr. J.J is an industrious man because he helps the staff and other patient in the institution in his little way like watering the plants, gardening, fixing and cleaning stiff at the institution. b. General appearance and Grooming Mr. J.J is aware that hygiene is very important in everyday life. I have observed that he is neat and clean all day. His hair is fixed and he wears clean clothes. His nails are trimmed well. He has a very good appearance. c. Facial expression Mr. J.J is a happy man so I noticed him smiling all the time and laughing with other patient. During our interaction I become more comfortable to him because of his non verbal expression which shows that he is interested and willing to cooperate to the interaction. He seems to be listening well in my statements and questions because he is attentive and he can answers it correctly and immediately. d. Posture Mr. J.J walks straight and shoulders are leveled. His posture is good. When he sits, he doesnt slouch and he sits straight. e. Gait Carriage Mr. J.J can move freely without any discomfort. His balance is good and he is responsive during morning exercises. He can follow steps and he can perform it well. f. Body built Mr. J.J is a small man but his body built is classified as Mesomorph with wide broad shoulder, muscled arms and legs, narrow hips and a minimum amount of fats.

g. Psychomotor activity Mr. J.J is an active man. During morning exercises he can follow directions and steps accurately. He follows order and command well. h. Voice and speech Mr. J.J has a moderate volume of voice. He can communicate well and he talks clearly. He response to questions correctly and clearly. II. Mood and Affect Mr. J.J is friendly to all the people he can encounter in the institution. He is not a high tempered man, he prefer to be quiet sometimes. He loves to play and exchange of jokes to other patient. Perception As we have our interaction, Mr. J.J perceived things positively and has a purpose. He stated that change should start within ourselves if we want to. He has a good outlook towards the future and to his situation now. Thinking He has a good initiative in helping others inside the institution. He thinks well and fast on what to do especially in helping the staff in some things. he can also follows order.

Judgment Mr. J.J has a very Good judgment, he sees his future to be a good one this time by changing his alcohol habit and having a work again, by this I can say that he has a good judgment because he know what is the right thing to do to be more productive someday. Insights Mr. J.J has clear insights towards his situation now. He clearly stated that this event in his life has no one to be blamed but him; he also added that change should start in to his self for his benefits and for his family. His insight to change and to be more productive someday can help him forget his habit in drinking.
