(1) INTRODUCTION The National Programme for Control of Blindness (NPCB) is in its thirty fifth (35th) year, initiated as a Centrally Sponsored scheme (CSS) and in the Eleventh Five Year Plan (2007-12) under the flagship Health Programme of National Rural Health Mission (NRHM); designed as a Health Sector Reform (HSR) Programme. The NPCB is considered as a flagship model of Public Private Partnership (PPP). NGOs and Private Practitioners play a key role in NPCB with over 73% of the cataract operations being performed by them.

(2) XIth FIVE YEAR PLAN (2007-8 to 2011-12) 1. Main Objectives a. The six main objectives of the NPCB to reduce the prevalence of blindness include b. Reduce the backlog of blindness c. Develop comprehensive eye health facilities in the 626 districts d. Strengthen Human Resources e. Enhance the quality of eye health services to the population f. To secure the participations of the Non-Governmental Organizations i. The four key expectations3 included 1. Adopt area for eye-care services (Block or group of villages), 2. Identify the blind, organise screening camps and transport patients to base hospitals; 3. Provide medical/laser/surgical rehabilitative treatment; and 4. Ensure follow-up g. To enhance community awareness 2. Strategies Five key strategies to achieve the objectives, included a. Decentralise the implementation of the programme and manage though the State and District Health Societies b. Increase participation of the voluntary organizations, including the Private sector c. Train personnel (eye health team), provide support for equipment, monitor and follow-up for Quality Assurance, and

d. Ensure convergence with other Sectors (multi-sectoral action at district level) and schemes, i.e., with leadership, supervision and monitoring from the District Collector to ensure convergence with the i. District Rural Development Authority (DRDA), Social Welfare, Media, District Education Officer (DEO). ii. A focus on the illiterate women in convergence with other schemes, and iii. Participation, including that of Community and the Panchayati Raj Institutions (PRI). e. Assistance for Other Eye Diseases (OED), besides Cataract. 3. Activities The activities included a. Develop eye care services at the Secondary level (District Hospital and NGO Eye Hospital) and at the Primary level (Sub-district level hospitals/Community Health Centre (CHC), Mobile Ophthalmic Units, Vision Centres/Primary Health Centres (PHC) through developing/strengthening them. Funding was specifically allocated for this as well as for training of human resources. The need for greater participation of the Non-Governmental organizations / Voluntary Organizations is recognised both as objectives and strategies. (3) VISION 2020 VISION 2020 represents both Non-Governmental agencies and international Governmental agencies that are the major Non-Government source for the blindness program. It efforts complements that of the Government and other stakeholder to eliminate the main causes of avoidable blindness in India by facilitating the planning, development and implementation of a sustainable national eye care programme (NPCB) based on the three core strategies of disease control, human resource development and infrastructure and technology, incorporating the principles of Primary Health Care (PHC), integrated within the NRHM (National Rural Health Mission) in the National Health System. Plan Formulation VISION 2020 INDIA, at the request of MOHFW, Government of India, worked closely with the department (NPCB) to formulate the XIth Five Year Plan (2007-8 to 2011-12). (Annexure 4) 4

Plan Implementation Subsequent to its approval by the Cabinet Committee for Economic Affairs (CCEA), VISION 2020 INDIA worked with the department on the following key issues.

5.2 VISION 2020 INDIA perceives the five major challenges which need to be overcome in the XIIth Five Year Plan and are: 1) NGO / VOLUNTARY SECTOR PARTICIPATION a. Further increasing and strengthening the participation of the NGO Sector / Voluntary Organizations for other eye diseases possibly because of the difficulties with the Grant-in-aid to the District Health Societies (DHS) for release to the NGOs need for greater transparency, strengthening the Management Information System (MIS) and enhancing support both for cataract and other eye diseases. 2) STRENGTHENED PUBLIC SECTOR a. We are not in a position to assess the strengthening of the Public Sector (eye wards and eye theatres at district and sub district level, Vision Centres in the Public System). This needs to be assessed as (......% of XIth plan expenditure was earmarked for public sector and ....% for NGO sector through grant-in-aid (recurring and non-recurring). We suggest that an evaluation needs to take place both as to review the implementation of the plan as to the strengthening of Medical College, District and sub-district level infrastructure and the outputs of the strengthened Public Health System. 3) CONVERGENCE a. Systems have not been developed for convergence and involvement in Panchayati Raj and other institutions, and involvement in other schemes. The improvement of Primary Eye Care and Refraction Services is the area of greatest need which needs to be strengthened in the XIIth Plan.

4) REHABILITATION SERVICES a. Rehabilitation services for those with visual impairment or blindness need to be strengthened and cross sectoral programmes such as Sarva Shiksha Abhiyan (SSA) for eye care of children as well as education of those with visual impairment or blindness. 5) HUMAN RESOURCES a. Human Resources particularly at sub-district level NGO partnership could be the answer to this also and mechanisms for productive and equal partnerships (in the real sense) can be included in the XIIth Five Year Plan. (6) STRATEGIES CRITICAL FOR THE XIIth FIVE YEAR PLAN Of the above, the following three strategies are critical for the XIIth Five year plan, i.e., 1. Strengthening partnership with the Non-Governmental Sector (NGOs) through enhanced Grant-in-aid and transparent Management Information Systems. 2. Strategies for cross-sectoral programs with Panchayati Raj Institutions (PRI), women and childrens programs, etc. 3. Strengthening of Rehabilitation Services. (7) KEY ISSUES IDENTIFIED AND SUGGESTIONS
THE VISION 2020 INDIA team believes that incorporation of the following suggestions in the National Programme (NRHM and NPCB) in the XIIth Five Year will significantly contribute to the rights of our Indian citizens residing in our 626 districts (especially the vulnerable5 populations, and those residing in rural and tribal areas) to their right to health, including their right to sight to enhance the Quality of their lives. subsidy and not a cost-reimbursement. The actual cost is about 2500. Enhance this subsidy from Rs 750 per surgery to Rs 1500 per surgery taking into consideration the fixed costs, the equipment costs, depreciation, HR costs, etc. RSBY remits Rs. 2500 per patient. Govt. should take cue from this scheme. Clearing the backlog of arrears from 2004 onwards by Direct payment can be made through bank accounts by utilizing the Central Plan Scheme Monitoring System (CPSMS). If the District Health Society does not clear the payment within 60 days then it should be cleared centrally after 60 days but within 90 days of submission of claims. Convergence with SSA

SSA is a good platform for disseminating eye care knowledge. A centralized curriculum focusing on eye care, sanitation and education may be developed to be used in primary and secondary schools. SSA may participate actively in World Sight Day. Make Vision screening compulsory for children before school entry (We have shared these points with Dr Amarjit Singh, JS, MHRD) 1. Early identification of children with eye problems, low vision and blindness. 2. Functional assessment and ophthalmic examination of all children 3. Provision of low vision devices and other educational devices to children with low vision. 4. Provision of Braille kit, Braille books and mobility devices to children with blindness. 5. Providing treatment, surgical intervention, medicines and spectacles to children. Dr Bhushan Punani informs us that in Gujarat, SSA even pays for Surgery in case of congenital cataract. We should consider offering the same facility for amblyopia, squint and other eye ailments and conditions resulting into progressive loss of vision. 6. Training of regular teachers as regards the specific needs of children with low vision and blindness and eye conditions. 7. Appointment of special educators for ensuring education of children with low vision and blindness. 8

Enabling Legislation for the NGOs/Private Sector Create an enabling The GOI should exempt Eye Health Organizations from the following environment for the three legislations NGP/Private sector 1. Duties imposed by the Customs on Eye Health Equipments including low vision aids. 2. Central excise on Corporate Income. 3. 1:3 ratio governments approved paramedical appointment against bed strength. 4. Copies of the VISION 2020 Publications - (1) THE VISION 2020 HANDBOOK ON Equipping a secondary eye hospital, and (2) Guidelines for the Comprehensive Management of Diabetic Retinopathy in India were given, including the Advocacy Roadmap for Eye Health Report.

(2) As suggested by the Additional Secretary, submit the Course Curriculum for Diploma in Ophthalmic Technology and take it forward for recognition from Ministry of Health. Decisions / Actions:

Training of Eye Health Team ( Eye Surgeons Medical Council / DNM, Nurses Nursing Council, PMOA) 1. Getting a common curriculum finalized by forming a committee by the ministry 2. Focus on Human Resources Development, especially in the Public Sector (Government Sector) and also NGOs in order to identify the current status, need in the 12th plan and gaps 4.
