Rules and Guidelines

1. The BRA-VO Fundraiser is designed to encourage participants to create and decorate a bra. The Peoples Choice Award will be chosen by the raiser general public by contributing $1.00 per vote earmarked to a designated entry. Unlimited votes are permitted. The Fund-raiser is sponsored by Fund Artisans of Mount Dora, Inc., d/b/a/ Artisans on fifth, a Florida nonprofit corporation and is intended as a benefit as described below. s 2. In order to be part of the Fundraiser, you must agree to these Rules and Guidelines which consist of ( I ) the terms and conditions on these raiser, conditions on these pages; and (ii) the Entry Form. If you are a minor, you must obtain the prior permission of your parent and/or legal guardian, who must agree to these Rules and Guidelines on his/her and your behalf before you can enter this Fundraiser. Because these Rules and Guidelines raiser. form a legally binding agreement with respect to this F Fundraiser, please read them carefully. If you do not agree to the Rules and Guidelines you are not eligible to participate in the Fundraiser. Bra design should completely cover the bra, including straps, sides, back and inside. There is no Theme. If the design includes any mechanical devices such as battery packs, this must be accessible to the curators of the exhibit. Additional batteries must be provided. Entry fee of $20.00 per entry must accompany each entry. Multiple entries are allowed. All entries meeting these terms and conditions will be accepted. Artisans on fifth accepts no responsibility for submissions lost, delayed, damaged, defaced, or mislaid, howsoever caused. 5. All entries are due no later than September 22 at 12 Noon. Submission may be delivered in person to Artisans on fifth, or may be sent by mail or by courier service (Fed Ex, UPS, etc.) to the address indicated below:

6. Entries must be accompanied by a short paragraph explaining your creative process, inspiration for the design and how the piece was created. You must include your name, address and contact information. Title of the creation is most important. This information will be displayed with your bra. 7. All Designers should attend the Designers Reception, Saturday, October 6 and the BRA-VO Auction, Saturday October 20, 2012, at the Artisans ception, on fifth. 8. By participating in the Fundraiser, you agree and hereby grant Artisans on fifth permission to use, copy, modify, distribute, and publicly display your submissions for any purpose, such as, but not limited to, press and media communications, without any compensation or attribution to you. You also agree to participate in any media or promotional activity regarding the Fundraiser. If you are an entrant you agree that the sponsor may use your name and likeness to administer and promote the Fund undraiser and to conduct media interviews and promotional events. 9. This Fundraiser is governed by the laws of the State of Florida. By participating in this Fundraiser, you agree to be bound by the terms of these raiser Rules, Guidelines and by Artisans decisions, which are final and binding on all matters pertaining to this Fundraiser. Each entrant agrees not to raiser. participate in the Fundraiser in any manner that might: (I infringe any third-party proprietary, intellectual property, industrial property, personal rights (I) or other rights, including without limitation, copyright, trademark, patent, trade secret or confidentiality obligation. To the extent permitted by law, the permit right to litigate, to seek injunctive relief, or to any other recourse to judicial or any other procedure in case of disputes or claims resulting from or in connection this Fund-raiser are hereby excluded, and you expressly waive any and all such rights. raiser 10. TO THE MAXIMUM EXTENT PERMITTED BY LAW, YOU INDEMNIFY AND AGREE TO KEEP INDEMNIFIED ARTISANS ON FIFTH AT ALL TIMES FROM AND AGAINST ANY LIABILITY, ACTIONS, CLAIMS, DEMANDS, LOSSES, DAMAGES, COSTS AND EXPENSES FOR OR IN RESPECT OF WHICH ARTISANS ON S, FIFTH WILL OR MAY BECOME LIABLE BY REASON OF OR RELATED OR INCIDENTAL TO ANY ACT, DEFAULT OR OMISSION BY YOU AND UNDER THESE RULES INCLUDING WITHOUT LIMITATION RESULTING FROM OR IN RELATION TO ANY BREACH, NONOBSERVANCE, ACT OR OMISSION WHETHER NEGLIGENT OR OTHERWISE, PURSUANT TO THESE RULES BY YOU. TO THE MAXIMUM EXTENT PERMITTED BY LAW, YOU AGREE TO HOLD ARTISANS ON FIFTH, ITS RESPECTIVE DIRECTORS, OFFICERS, EMPLOYEES AND ASSIGNS HARMLESS GREE AN FOR ANY INJURY OR DAMAGE CAUSE OR CLAIMED TO BE CAUSED BY PARTICIPATION IN THE FUND-RAISER AND/OR USE OR FUND ACCEPTANCE OF ANY PRIZE WON. By entering the Fund Fundraiser and/or submitting an entry you agree explicitly that all components of your artwork and supporting statement are indeed original creations. Any intellectual property infringement, misuse or plagiarism of anothers work in any form or another state will result in immediate disqualification and forfeiture of eligibility t receive all awards and recognition. You also agree to allow Artisans on fifth to to make public statements regarding any disqualifications and any ground for disqualification. All intellectual property and industrial property right in any entries that belonged to the entrants will remain with the Entrants, but the submissions will otherwise become the property of Artisans on fifth, ged and will not be returned after the Fund-raiser. You grant Artisans on fifth permission to use, copy, modify, distribute and publicly display your entry for raiser. publ any purpose without any attribution or compensation to you. 11. Proceeds from this Fundraiser will be contributed to Waterman Hospital Foundation benefiting women in the immediate area of Lake County for raiser free and low cost breast diagnostics, and for the benefit of Artisans of Mount Dora, Inc.
