O autor apresenta os principais dados experimentais que explicam a ao antidepressiva dos extratos de Hypericum perforatum e discute a eficcia clnica do composto, baseado nos estudos realizados at o momento. St. John's wort has been used as antidepressant in the past decade. This article presents the main experimental findings that explain Hypericum perforatum's antidepressant action. A erva-de-so-joo (Hypericum perforatum) uma planta da famlia Guttiferae, encontrada em toda a Europa, na sia, no norte da frica e na Amrica do Norte. Os principais constituintes do Hypericum perforatum so: naftodiantronas, como as hipericinas; acilfloroglicinis, como a hiperforina; glicosdeos flavonides; biflavonas; proantocianidinas e fenilpropanos (Erdelmeier, 1998). Desde a Idade Mdia, recomenda-se o uso da erva-de-so-joo como medicamento para as mais diversas afeces, inclusive para quadros de depresso. Extratos do Hypericum perforatum tm sido usados na ltima dcada como antidepressivos na Alemanha e esto hoje dentre os antidepressivos mais utilizados naquele pas. Em 1997, com mais de 3,7 milhes de prescries, o Hypericum foi responsvel por 25% das prescries de antidepressivos na Alemanha (Laakmann et al., 1998). Alguns estudos, comparando o Hypericum perforatum com placebo e com antidepressivos tricclicos, tm demonstrado que a erva-de-so-joo realmente tem eficcia clnica no tratamento de depresses leves e moderadas (Laakmann et al., 1998). Apesar disso, no se conhece o exato mecanismo de ao, nem se tem certeza de qual o componente relacionado com a ao antidepressiva. Inicialmente, acreditava-se que a ao antidepressiva da erva-de-so-joo era devida inibio da monoaminaoxidase (MAO) pelos componentes flavonides. Atualmente, sabe-se que tal explicao insuficiente, tanto pela baixa concentrao dos flavonides no Hypericum quanto pelos estudos experimentais realizados que no comprovaram essa hiptese (Schellenberg et al., 1998). Por algum tempo, atribuiu-se a ao antidepressiva da erva s hipericinas por suas concentraes mais elevadas, mas, hoje, o componente mais cotado para explicar a ao antidepressiva do Hypericum perforatum a hiperforina, antes desconsiderada por ser um composto instvel ao calor e luz.
