Cardiac Risk Factors

by: Dr. JP Saleeby, MD
The leading cause of death in the United States is from cardiac (heart) disease. It is crucial that Americans of all ages be concerned about and identify their Cardiac Risks. In this article I will outline some modifiable and non-modifiable risk factors, certain tests which are important to obtain as a part of routine health screening & lastly some measures to take to reduce the risks. To define Cardiac Disease, we mean to say, atherosclerosis of the coronary arteries or the hardening of the arteries that feed the heart muscle. In general there are several established risk factors that include: Family History, Advanced Age, Male Gender, Postmenopausal Status in Women, Tobacco Use, Diabetes & Kidney Failure, Thyroid dysfunction, Hypertension, Elevated Cholesterol (LDL), Low HDLC, Elevated Homocysteine, Lp (a), Sedentary Lifestyle, Increased States of Inflammation, Poor Nutrition, Elevated Fibrinogen, Low Socioeconomic status, and Low circulating Antioxidants. Traditionally physicians have focused on family history, hypertension, tobacco use, gender, cholesterol, sedentary lifestyle & diabetes. It is becoming increasingly clear that there are other independent risk factors that should be assessed for the simple reason that coronary artery disease (CAD) can be found in people with negative family history and normal cholesterol levels. There is probably too much emphasis today focused exclusively on lowering total cholesterol and LDL-C when it is apparent from research and population studies that these risk factors are not impacting many of the patients with CAD. Checking the oxidized-LDL may actually be more important as well as total oxidative load and total oxidative protection index, than just a standard lipid profile. Assessing Vitamin A, Beta-carotene levels, Vitamin C and Tocopherol (Vit. E) levels may also play an important role. Vitamins B-6, B12 and Folate should also be measured since they play a major role in metabolism and elimination of homocysteine. Another vitamin like substance known as ubiquinone or coenzyme Q10 has a tremendous role in cardiac health. Studies mostly out of Japan have shown that deficiencies in coenzyme Q10 can throw a patient into congestive heart failure. Certainly assessing serum levels of coenzyme Q10 may be important in certain individuals. Those folks with elevated Lipoprotein (a) or Lp(a) have an increased risk of coronary artery disease, again that is something that should be checked as part of a comprehensive cardiac risk assessment. Elevated Fibrinogen, which can cause blood to clot abnormally, is also another testable independent risk factor. Homocysteine has come to

the forefront lately after the publishing of studies and a text by Dr. Kilmer McCully as yet another important independent risk factor for heart disease. While this is now just becoming more routine in cardiac testing it certainly is a treatable risk factor that patients should be aware. Unfortunately there are some non-modifiable risk factors that defy treatment such as family history (probably the strongest of all risk factors), advanced age (hormonal considerations), the male gender, and low socioeconomic status. But identifying modifiable risk factors and implementing changes will drastically lessen a persons risk for coronary disease and heart attack, especially if a person has one or more non-modifiable risk factors. These modifiable factors are the postmenopausal state in women, tobacco usage, diabetes (managing high glucose levels), maintaining proper thyroid function, controlling high blood pressures, decreasing elevated levels of cholesterol, decreasing elevated homocysteine levels, lowering circulating inflammatory mediators, improving nutrition, increasing circulating antioxidants and implementing an exercise program. For your own good cardiac health it is important to seek out those proactive physicians who will order both the esoteric as well as the routine cardiac risk profiles. Also it should be pointed out that the endpoint of all therapies to lower risk of coronary artery disease is the reduction of plaques on the arteries. Therefore, studies should be performed as a baseline and to determine if therapy is effective via noninvasive Electron Beam (or Siral) CT scan of the heart for calcium score.

Were you aware that coronary heart disease is the leading cause of death in the U.S. and the leading cause of death caused by smoking? Its true. According to the U.S. Department of Health and Human Services smoking and heart disease are strongly related. Smoking is hard on the heart, and the toxins in cigarette smoke cause plaques to form in your arteries, which leads to atherosclerosis, otherwise known as hardening of the arteries. Atherosclerosis is a progressive hardening of the arteries caused by the deposit of fatty plaques and the scarring and thickening of the artery wall. Inflammation of the artery wall and the development of blood clots can obstruct the flow of blood to the heart and cause heart attacks or strokes. Cigarette smoking has been associated with sudden cardiac death of all types in both men and women. Smoking-related coronary heart disease may contribute to congestive heart failure.. Smoking and heart disease do go hand in hand. If you are thinking of quitting and need help call today to schedule an appointment with Dr. Saleeby. WellnessOne is here to support you!

P atient Stories
What is the deal with this WellnessOne all about? I had driven by it several times and never took the time to stop and find out. Several issues were going on in my life and no one, mainly doctors, wanted to hear about all of them. Separation of my body parts and my needs had to be farmed out to different specialists and modalities(treatments-sometimes my nursing background sneaks in). Sick of that mentality and being in pain led me to WellnessOne. Opening their door was by far the most helpful thing I have for myself in a long time. Greeted by a most delightful and caring receptionist was to be only the beginning! An appointment was made for the next day for a massage since I was having muscle pain everywhere. I have fibromyalgia and was having a flare-up. I, also, was looking for a friendly, compassionate and professional massage therapist. Not only was Darin a fine and knowledgeable massage therapist he became a major force in my life. He not only greatly exceeded all of my expectations as far as medical needs but also filled my inspirational needs. As time passed, WellnessOne became my home away from home-I threatened to bring my pillow and take over a room repeatedly. Expansion has been a welcome site as it brings more friends to meet-both clients and staff. Small informational chats have been beneficial to clients and family members. I have loved going to these meetings as it makes any problems easy to discuss and I get answers that are highly helpful. Dr. Saleeby is a most welcomed addition to the staff. He was concerned with ME and not my chart. A plan was prescribed for my health needs and not my illness. The diet, massage, infared booth and eventually chiropractic intervention helped me to find myself. Dr. Ed Thomas, the chiropractor with best southern accent I ever heard, has given me more mobility than I thought possible. No medication I took ever gave me pain-free days like I now have. Having to have a MRI, see a pain management doctor and a physical therapist for severe arthritis in my back I, now, have all my needs met at WellnessOne. Oh, lest I forget, colonic hydrotherapy was in my future treatment. All, OK most, of the toxins in my digestive system have been cleared out in a few treatments. Never felt so clean and empty in my life! The newest equipment and the professionalism shown to me made this experience pleasant(HAHA) instead of embarrassing. You should try it sometime. My muscle pains, headaches and digestive problems have cleared entirely or been lessened to a great degree.

My attitude is back-I am from Upstate NY, and the real me has been released! Activity was absent and now is a welcome addition back in my life. Aches and pains are not my lead in with conversation. What can we go do and when are we going is where I am coming from now. Of course, WellnessOne, is highly recommended to my family, friends, acquaintances and healthcare providers. My needs have been met, both physical and emotional. I have a new understanding of my needs and have learned how to control them and not have them control me. Heartfelt thanks to all I have met and now count as friends at WellnessOne -the most interested in You place to be! -Linda Varga, WellnessOne Patient

YOUR LIQUID ASSETS.WATER!
No matter what..DRINK WATER. And then when you are done, drink some more. And then some more. But water is boring right? Not reallynot if you look at how absolutely essential it is to your health, and ultimately your happiness! It took me a while to understand just how important water is to my every day. I went a LONG time not drinking enough water, or just drinking the 8 glasses or so a day you hear you need growing up. No one told me when I was younger how important water was. That was because when you are young you can stand to live off of Capri Sun and Mello Yello. That is how I remember my formative years. I can actually remember drinking a Mello Yello every single morning before schoolfor years. I would explode if I did that now. Makes me sick just thinking about it. As you get older, and if you choose to live a healthy lifestyle, you must drink lots and lots and lots of H2O! If I go a day without drinking enough water, it is the first thing I feel the next day. I am usually stiff, with a bad headache, and a zapped energy level. Drinking a healthy amount of water is vital to your health. You gain tremendous health benefits and sometimes can even throw away your migraine medicine or pain killer. Water not only helps your body to absorb all the essential nutrients that it needs but it also helps to convert your food into energy! How cool is that? This is magnified when you lead an active lifestyle and get plenty of exercise. While exercising you should drink water before, during and after. The more vigorous your exercise, the more water youll need. Water also cushions your joints, protects your vital organs, regu

younger when your skin is properly hydrated. Water helps to replenish tissues, moisturizes skin and increases skin elasticity. Helps in Digestion and Constipation: Drinking water raises your metabolism because it helps in digestion. Water and fiber goes hand in hand so that you can have your daily bowel movement. Reduces the Risk of Cancer: Related to the digestive system, some studies show that drinking a healthy amount of water may reduce the risks of bladder cancer and colon cancer. Water dilutes the concentration of cancer-causing agents in the urine and shortens the time in which they are in contact with the bladder lining. If you are intrigued but feel like you need to ease into drinking this boring beverage in bulk, start small. Try drinking a flavored water, a coconut water or aloe water. These are good but only in moderation. The reason these drinks taste so yummy is because they are flavored with lots of sugar. Mineral water is good too. Are you reaching for that nice tall glass yet? I AM! Trust meYour body will end up thanking you in the long run!
