Information on Application for Admission to Kindergarten


School Year 2013-2014

I. POLICY 1. All applicants to Xavier Kindergarten are required to go through the admissions screening process. The purpose of the admissions screening process is to determine whether or not the Early Education Program of Xavier will be suitable and beneficial to the applicant. 2. All applicants are required to undergo Vision and Hearing Screening Tests. 3. Parents will be informed of application results, but details of actual test scores will remain confidential.

II. GENERAL NORMS


1. A boy is evaluated primarily based on his own merits. However, a weighted system gives due consideration to an applicant who is a son of an alumnus or who has brother in Xavier School. 2. The following areas will be considered in screening the applicant : a) Attention Span ("Can the boy sit through a 15-30 min. task?") b) Interpersonal Skills ("Can the boy get along with others?") c) Communication Skills in English and Filipino: Comprehension ("Can the boy understand what is asked of him?") Expression ("Can he articulate his needs?") Competence ("Can the boy understand and follow directions/instructions?") d) Self-Discipline ("Can the boy finish a given task?") e) Stress Tolerance ("Can the boy participate actively in group activities even when under the normal pressure of multiple instructions and activities?") f) Does the boy know his letters, numbers, basic shapes? 3 The school also takes the following into account: a. the willingness of the parents to collaborate actively with the school in the education and formation of the student (which includes the parents acceptance of the school policies, especially those found in the student handbook); b. the willingness of the applicant and his parents to be involved in important school activities; c. the extent to which the applicants family environment is open to and supportive of of the Catholic values espoused by the school.

III. AGE REQUIREMENT


Applicants who were born from July 01, 2007 to June 01, 2008 (Between ages 5 years to 6 years old by June 2013)

IV. REQUIRED DOCUMENTS


Letter of Intent to Apply Birth Certificate from NSO or Certified True Copy (original & photocopy) Baptismal Certificate if baptized in the Roman Catholic Church (original & photocopy) Three (3) copies of recent identical 1.5 x 1.5 Photos in white background (scanned & computerized pictures not allowed) to be pasted on the following: Application Form (1 copy) Kinder Admissions Test Schedule Slip Health Record Form Two (2) copies of recent identical Family Picture in size 4R (scanned & computerized pictures not allowed) to be pasted on the application forms. (Only immediate family members should be in the picture.) Vision and Hearing Screening test results Original copy of the Report Card in the previous year and/or photocopy of the grades in the First Quarter of the current school year.

Additional Requirements for Naturalized Filipino, Dual Citizen and Non-Filipino Applicants: For Naturalized Filipinos: Naturalization Identification Certificate (NIC) (Original & Photocopy) For Resident Foreign Applicants: (Original & Photocopy) Alien Certificate of Registration (ACR) Valid Foreign Passport Special Resident Retirees Visa (SRRV) Special Investors Resident Visa (SIRV) For Dual Citizens or applicants who were born abroad: (original & photocopy) Identification Certificate of Recognition (ICR) Bureau of Immigration Order Report of Birth from the Foreign Services of the Philippines Philippine Consulate Valid Foreign Passport Valid Philippine Passport

V. APPLICATION PERIOD
: August 01 - 31, 2012 September 10, 2012 Application Fee and Vision & Hearing Screening Test
Note:

Monday Friday: 7:45 AM - 3:00 PM (No lunch break) Saturday: 8:15 - 11:00 AM Deadline of Submission of Accomplished Forms P 1000.00 (Non-Refundable)

Application for XS Nuvali will be entertained in both the San Juan and Nuvali campuses starting in September 2012.

VI. APPLICATION PROCEDURE


1. Present the applicants Birth Certificate (original/certified true copy) to the Registrars Office and secure a Payment Slip. 2. Pay the Application Fee and Vision & Hearing Screening Testing Fee of P1000.00 in cash to the ACCOUNTING OFFICE ( non-refundable). 3. Present the receipt of payment to the REGISTRARS OFFICE, and claim the following documents: a) Application Form b) Health Record Form c) Recommendation Form 4. Submit to the REGISTRARS OFFICE the following for checking: a) Duly accomplished Application Form b) Duly accomplished Health Record Form c) Required Documents d) Vision & Hearing Screening Test Results Upon completion of the required documents you will be issued a proof of completion of the requirements. 5. Proceed to the TESTING AND RESEARCH CENTER, present proof of completion and claim the Kinder Admissions Test Schedule Slip. 6. Bring the applicant to the TESTING AND RESEARCH CENTER (TRC) on the scheduled testing date. Make sure to bring the Kinder Admissions Test Schedule Slip.

VII. INDIVIUAL TESTING


1. Applicants are expected report to the TESTING AND RESEARCH CENTER (TRC) at least (10) minutes before the scheduled Individual Testing Session. Due to the tight schedule and the nature of the Individual Testing, latecomers will not be entertained. Requests for the postponement of Individual Testing will not be granted. Likewise, tardiness due to heavy traffic, missed schedule, etc. will not be considered as valid reasons for postponement. Individual Testing may be reset only if: a) the applicant is sick ( A medical certificate from the attending physician is required); b) there is death in the family; c) the school calls off classes/offices due to inclement weather or other unforeseen events. In case the applicant refuses to enter the examination room, rescheduling of the Individual Testing will be accommodated. However if the applicant fails to enter the examination room after being given three (3) chances, his application will be automatically cancelled.

VIII. APPLICATION RESULTS


1. Application results should be claimed at the Registrars Office according to the schedule given since results are released in batches. Only parents or their representatives (with Authorization letter) may claim application results. Results will not be released over the phone. 2. If the results are not picked up 4 days after the release date, we will assume that you are no longer interested in your childs admission to Xavier School San Juan. His application will be automatically cancelled.

IX. ENROLLMENT RESERVATION (FOR ACCEPTED APPLICANTS)


For applicants who have been accepted, the parents are required to: 1. Make a reservation to express their intention to enroll their son in Xavier. N. B. The acceptance letter will include instructions on how to make this reservation and to pay the required Reservation Fee. For other concerns regarding the Reservation Fee, please feel free to see the School Treasurer.

X. SESSION / SECTION ASSIGNMENTS FOR ACCEPTED APPLICANTS


1. The school reserves the right to assign accepted applicants to either the morning or afternoon session. 2. Class lists will be posted during the scheduled Parents Orientation Day.
