Introduction
Globalization and the Internet have made the world a global village and information exchange is becoming simpler.

Automating the current manual system of the hospital enables it to cope up with the current technological world.

Statement of the problem

SAMH mainly uses the manual patient registration and history recording system. The manual patient registration system is time consuming. The manual current system is liable to lose of patient data. The patient data are liable to be accessed by un authorized person.

Objectives of the project

1. General objective
To develop a web based patient registration and recording application software. To develop a dynamic web page for sister

Akelesia memorial hospital.

To minimize time and labor lost for patient registration. To provide pertinent information about the hospital.

2.Specific objective
Study the existing system carefully and identify its problems.

Perform object-oriented analysis and model a new system using UML. And extend this to design.
Develop web page for Sister Akelesia Memorial Hospital. Enable patient register online for 1. medical examination and 2. laboratory examination. Develop a database for Doctor & Lab technician registration.

Readily available semi- manual documents. Visit an internet for related information. Collected data from different stakeholders.

Save the time and labor lost by patients for registration.

Enable the hospital to give fast and reliable service for the patient.
Make the hospital competent in the future. Prevent the lose of patient medical history. Enable the hospital to give consistent and reliable information for patients. Enables Doctors and laboratory technician to use their time efficiently.

Online patient registration for Medical Examination and Laboratory Test. Retrieve lost password and patient serial number. Doctor and laboratory technician registration. Access patient medical history.

Limitations the project

To separate those page which should be visible in the LAN of the hospital and through out the internet. To Connect with other hospitals. The system to automatically calculate the appointment date and time.

Generate an auto alarm message as soon as the patient registered. Due to lack of technology (credit card payment) we couldn't make the online registration complete. However, most of the above limitation would have been done if we got enough time.

Conclusion
The problem of information flow and inconsistency of the hospitals information. Lose of patient medical history.

The time and effort lost by patients for registration and examination.
Difficulty in understanding patients medical history. Unnecessary payment for workers.

Recommendation
To separate those page which should be visible in the LAN and through out the internet. Develop a sub system that handles the payment for medication and registration. Generate an auto alarm message as soon as the patient registered
