Summary of the Lesson Plan


The summary should be a snapshot of the lesson: 1 sentence giving a short description of the lesson and 1 sentence about the general materials & timeframe used to teach the lesson.

Student Population
Grade Level: Grade level for teaching lesson.
Skill Level: Choose one: on-level, above-level, below-level, all levels. Grouping: Identify how students will be grouped during different parts of your lesson: whole group direct instruction, small group direct instruction or guided practice, partners- guided practice, independent practice.

Materials
List everything you and your students will use during this lesson.
List book titles, authors, page numbers, teacher edition materials, etc. List the names of all practice sheets and handouts. (Attach to lesson plan) List the assessment tool or rubric. (Attach to lesson plan)

Procedure
(numbered steps)
Write out the steps of the lesson. Think of this as a recipe. What specifically will students do during this lesson? Number the steps so its easy for you to glance at your lesson plan and know where you left off. In the procedure, include information a teacher would need to know to implement this plan. It should be easy to find book titles, page numbers, chapter titles, workbook information, etc. Be specific but dont get bogged down in little details. Do not write things like Students will sit at their desk and take out a pencil.

Closure
How you will wrap up the lesson? Describe what you will do to ensure students have an opportunity to explain what they learned and how to apply the concepts.

Assessment
Assessment Tool State what you are using as your assessment tool. Explain how you will know students understand the concepts from the lesson.
What Is Being Assessed? Restate the lessons objective (you can copy and paste it into this section) and identify how your assessment measures student understanding. There should be an obvious connection between the standard, the lesson, and the assessment.
