The electricians received information on how to change the handle.
The handle was replaced but still did not work

1 and 2 degree burn (on hand)/(DAFWC) serious injury
What happened
One of the technicians opened the door and used the old handle directly to the breaker operation axel and
turned the locking arm
The locking arm came too close to the live part of the fuse and created an electrical short circuit
Direct causes
1-10 Short circuit
3-1 Lack of knowledge of hazards present

Failed meggar test were found on 2 of the 3 phases
Repairs were made to the 2 phases
Following repairs, one phase still failed the meggar testing
Action
It was determined that the linemen would be required to assist in investigating the failure
Since it was break time and it appeared as if it might rain, the transformer cover was put back with two screws.

What happened
The wireman crew foreman thought that the work was completed, so he told them to install the cutouts and
energize the transformer.
Immediately after being energized, one of the splice connections failed and the 4160 volt feeder tripped out on
ground fault.
