Leading cause of meningitis, bacteremia and


pneumonia, sinusitis, otitis media worldwide
in children below 5 yo
WHO estimates( 2008) : 476,000/ 8.8 M
deaths
Developing countries : CFR 20%
pneumococcal septicemia, 50% meningitis
Philippines (2005) pneumonia is no. 1 cause
of morbidity
Philippines (2006) pneumonia is 3rd leading
cause of infant mortality, 1st in <5 y/o
morbidity

May reach lungs through different routes


Contaminated

air droplets
Blood born infections
In contact with organisms in the birth canal or
with contaminated substance during birth
Transmitted through respiratory droplets from
nasopharynx

Exclusive breastfeeding up to 6 months

Adequate nutrition

Vaccination for infants and children


Hib
Measles
Pertussis
PCV

All infants 6 weeks 11 months old in priority


areas

Dose (0.5 ml, IM, upper thigh)

Minimum Age to be given

1st dose

6 weeks ( 1 months old)

2nd dose

10 weeks ( 2 months old)

3rd dose

14 weeks (3 months old)

Safely administered with other vaccines


(OPV, RV, Pentavalent {Hep B, Hib,DPT} )

Separate syringe

Separate injection site

NOT to be given : severe allergic reactions


to prior dose or any component of the
vaccine

NOT to be given : moderate to severe illness


( Temp : > 39) until condition
improved

Mild infection such as URTI is NOT a


contradindication.

Liquid adsorbed vaccine in suspension for


injection
1 dose vial, with VVM
Stored in + 2 - + 8 , never frozen

Before administering

Screen the infants

Check age and immunization status ( 6 weeks 11


months old)
Establish if infant is healthy (no fever, diarrhea or
vomiting)
Screen of contraindications, steroid use

PCV 13 is safe and well tolerated


Severe adverse reactions attributable to the
vaccines are extremely rare.
Mild side effects reported in some cases
Other vaccines which were co-administered
may cause the reactions
Although safe, side effects may occur.

Common in about 1 in 2 (50%) : swelling and
tenderness at injection site
About 1 in 2o : transient fever 39

allergic reactions : dermatitis 1 in 1000

Adverse events after receipt of the vaccine,


even if it is not clear that it is caused by the
vaccine, should be reported following the
DOH Issuance AO 2010-0017 Guidelines in
Surveillance and Response to Adverse Events
