Transfers

Step by Step
Initiate Transfer
Obtain required information
Determine appropriate Moffitt
Specialist
Communication
Disposition
Bed Assignment

Initiate Transfer
Request for transfer is initiated by calling
the Patient Placement & Transfer Center
(PPTC) @ 813-745-8404
Request is made by
Sending facility representative contacts PPTC
Sending facility Physician contacts Moffitt Provider
directly. Please ensure that the sending facility
Physician contacts the PPTC

*Patients, family or friends cannot initiate a transfer & should be


instructed to contact the physician at Outside Sending Hospital (OSH)
to formally request / write an order for transfer to Moffitt Cancer Center

Obtain Required Info


Moffitts PPTC will obtain the following
information from the sending facility:
Requesting Physicians name, specialty and
cell phone number
Most recent 48 hrs of Progress Notes & Lab
Results
H & P, Consults
Current Medication List
All diagnostics & Pathology- CT Scans,
MRIs, X-Rays, slides
Demographics, insurance information

Determine Appropriate
Specialist
PPTC will determine the appropriate
Moffitt Specialist
Established Patients
Seen < 6 Mos- Most recent Moffitt Attending
Seen > 6 Mos- Moffitt On-Call Physician
Info will be sent to this Moffitt Specialist

New Patient Info sent to Med/ Onc Attending On-Call, Heme A


& B Inpatient Team or appropriate On-Call Surgery
Attending

Communication
Email communication will be sent to the
appropriate Moffitt Physician & include:
Name of sending facility
Name of requesting physician, specialty and
cell phone # in order to perform a peer-topeer discussion and/or hand-off
Attachments- Clinical information

Response
Email response- response should be sent
to PPTC within 2 hours between the
hours of 0900-1600:
Acceptance- Upon accepting the transfer,
the Moffitt Physician should include a brief
plan of care within the email
Decline- Upon declining the transfer, please
include reason
If another Physician is more appropriate to
accept the patient, please forward the email
with the attached clinicals and copy PPTC @
hospitaltransfers@moffitt.org

Disposition
Admitting to IHM- PPTC will forward the
entire transfer request/ plan of care via
email to IHM for review/ acceptance.
If IHM Physician has questions; they will
contact accepting physician
IHM Physician will reply to PPTC email
(accept/ decline)

New Patients/ Outpatient Clinic- If a new


pt can be seen in the Outpatient Clinic in
lieu of transfer, the sending physician/ pt
will be provided a New Patient Appt
*Overbooks require MCC Physician approval- pls include in email

ER Transfers
PPTC will page the appropriate MCC
Attending (not email) when an
established patient is in outside facilitys
ER
Consultation- It is not uncommon for the ER
Physician to request consultation from the
MCC specialist
Transfer- If ER is unable to discharge the
transfer will be affected immediately
If Moffitt lacks capacity, the sending facility will
admit to their hospital and the transfer placed @
MCC as soon as possible

Bed Assignment
Upon acceptance, the PPTC will:
Assign the first appropriate & available bed
(including level of care)
Send the original email packet to all involved
physicians including the bed # & time the bed
was given to the accepting facility

Important Points
PPTC is not able to initiate a transfer request directly
from patients, family or friends
All transfers must be initiated via a Physician Order
from the sending facility physician
The following MCC team members are not able to
accept transfers:
IHM Physicians, Residents, APPs, RNs
Non-Clinical Staff- Liaison staff
Updates on the status of transfers should be
communicated via the PPTC staff
PPTC will always call the official accepting/ admitting
Physician in the event that email approval is sent via
another party

Review
Initiate transfer through Moffitt Patient Placement &
Transfer Center
PPTC gathers clinical information from sending hospital
Email sent with clinical and contact information to
appropriate MCC Physician
Deciding MCC Physician emails PPTC back with decision,
instructions and brief plan of care
The entire email thread, to include your plan and clinicals
will be forwarded to the appropriate IHM Attending for
acceptance/ admission
Upon bed assignment, all physicians involved in the care
of the patient will be emailed including the existing email
thread, Unit, bed # and time assignment provided to
sending facility
