Goals of the Programme

The goal of the programme is to garget 5,600 Hampshire families


with multiple, complex issues, delivering solutions to persistent
problems that lead to lasting, positive change for families and
communities.

How;

early help tailored and intensive support

single family plans putting the whole family at the centre of the
planning

co-ordinated multi-agency approach local services working more


effectively together

transforming the way we work local innovation, system changes,


dedicated, assertive and persistent approach

service

*Families with 2 or more of the above family problems can be referred/nominated to the
Programme and at present, nominations can be made through the Early Help Hub.
