Predictors of Difficult Facemask
Ventilation
Older than 55 years old
Body mass index > 26 kg/m2
Beard
Lack of teeth
History of snoring
11/02/17 44
Tehnik
Facemask ditahan ke wajah pasien dengan
jari tangan kiri anestesiolog mengangkat
mandibula (chin lift, jaw thrust)
Tekanan pada jaringan lunak
submandibular harus dihindari
Jempol dan jari telunjuk anestesiolog
melakukan tahanan balik pada facemask
Tekanan ventilasi harus < 20 cm H2O
11/02/17 45
Air Adjuncts
Digunakan bila ada kesulitan dalam
menghasilkan tekanan positif yang
cukup dalam sirkuit pernafasan
anestesia
Oral dan nasal airways diciptakan
untuk memberikan jalur udara
dengan menyingkirkan lidah dari
dinding faringeal posterior
Oral airway dapat menyebabkan gag
reflex atau laryngospasm pada pasien
sadar atau terbius ringan. Nasal
airway lebih ditoleransi pada pasien
dengan anestesi ringan
11/02/17 46
11/02/17 47
11/02/17 48
11/02/17 49
Indikasi untuk Endotracheal tubes
Memberikan jalur pernafasan pasien
Mencegah inhalasi (aspirasi) isi lambung
Memerlukan suctioning yang sering
Menyediakan ventilasi tekanan positif
pada paru
Posisi operasi selain supine
Daerah operasi dekat / pada upper airway
Maintenance dengan masker sulit
11/02/17 51
Cricoid Pressure (Sellick maneuver)
Dilakukan oleh asisten dengan
melakukan tekanan oleh jari jempol
dan telunjuk pada kartilago krikoid
untuk memindahkan cincin
krikotiroid kartilago secara
posterior sehingga menutup jalur
esofagus yang terletak dibawahnya.
11/02/17 52
Laryngoscope
Curved (Macintosh) Blade
Straight (Miller) Blade
11/02/17 53
Tehnik
ETT dipegang seperti pensil pada
tangan kanan anestesiolog.
Dimajukan ke glottis dari sisi kanan
mulut. ETT di masukkan sampai
akhir proksimal dari ujung cuff 1
sampai 2 cm melewati vocal cord.
Cuff ETT diinflasikan dengan udara
11/02/17 54
Indikasi
Pasien yang diduga sulit dilakukan
intubasi dengan direct laryngoscope,
dapat diberikan sebelum anestesi umum
Dianjurkan pada pasien dengan cervical
spine yang tidak stabil
Pasien dengan luka pada upper airway
akibat trauma tumpul atau tajam
11/02/17 55
Tehnik
Dapat diberikan melalui nasal maupun oral,
pasa pasien yang sadar maupun teranestesi
Secara umum, pendekatan nasal lebih mudah
pendekatan nasal
11/02/17 56
Persiapan pasien
Penjelasan yang baik
Antisialagogue untuk mencegah
timbulnya sekresi air liur.
Obat sedasi yang baik

Laryngeal
Mask
Airways
(LMA),
Indications and Use
for the
Pre-Hospital Provider
Throat soreness
Dryness of the throat and/or mucosa
Side effects due to improper placement
vary based on the nature of the placement
Equipment for
LMA Insertion
Body Substance Isolation equipment
Appropriate size LMA
Syringe with appropriate volume for
LMA cuff inflation
Water soluble lubricant
Ventilation equipment
Stethoscope
Tape or other device(s) to secure LMA
Preparation of the
LMA for Insertion
Step 1: Size selection
Step 2: Examination of the LMA
Step 3: Check deflation and
inflation of the cuff
Step 4: Lubrication of the LMA
Step 5: Position the Airway
Step 1: Size Selection
6
Inflate the mask with the
recommended volume of air.
Do not over-inflate the LMA.
Do not touch the LMA tube
while it is being inflated
unless the position is
obviously unstable.
Normally the mask should be
allowed to rise up slightly out
of the hypopharynx as it is
inflated to find its correct
position.
Verify Placement of the
LMA
Connect the LMA to a Bag-Valve Mask
device or low pressure ventilator
Ventilate the patient while confirming
equal breath sounds over both lungs in
all fields and the absence of ventilatory
sounds over the epigastrium
Securing the LMA
