In 1860, Nightingale laid the


foundation of professional nursing
when the first school of nursing was
established.
Entry into Practice: Diploma
Programs for Registered
Nursing
Diploma programs are the oldest and most
traditional type of nursing education in the
United States. These programs are two to
three years in duration and provide nursing
education primarily in the hospital setting.
Graduates of these programs receive a
diploma as opposed to a college degree.
Most diploma programs are now affiliated
with colleges or universities that grant
college credit for certain courses.
History of Diploma Nursing
In the beginning nursing education
consisted of on-the-job training based
mainly on the specific needs of the hospital
in which the school of nursing was housed,
not on the learning needs of the individual
student. In other words, nursing students
provided cheap or free labor.
The apprenticeship differed in quality and
length. The size of the hospital influenced
the experience of the nursing student.
Larger hospital offered more exposure to a
variety of clinical experience.
Standardization of nursing program was not
considered until the 1940s.

The theory classes taught in these early nursing


schools were generally laid out in blocks of study that
included medical nursing, surgical nursing, psychiatric
nursing, obstetrics, pediatrics, operating room and
emergency department, in addition to classes in
anatomy, physiology, nutrition, pharmacology and
history of nursing.

Nursing students would work additional hours in a


hospital unit. A typical day could be 12 hours or more
in length. Nursing students covered all three shifts.
What is the Educational Preparation
of the Diploma Graduate?
The current preparation of a diploma nurse
varies in length from 1-3 years. The type of
program may be under the direction of the
hospital or incorporated independently.
Graduates of diploma programs are
prepared to function as beginning
practitioners in acute, intermediate, long-
term and ambulatory health care facilities.
Graduates of diploma program are also
prepared for beginning supervisory or
management positions in their hospital
setting.
The advantages of diploma programs include
having nursing instructors who are actively
involved in clinical practice.
Graduates of diploma program have the
greatest amount of clinical experience in a
hospital setting. Because there is a close
relationship between the nursing school and
the hospital, graduates are well prepared to
function in that institution; on graduation
many students are employed by that
hospital and therefore experience an easier
role transition.
In diploma schools, theory and clinical
practice are introduced concurrently.
Diploma nursing students are offered
nursing theory equivalent to that offered in
the RN preparatory schools.

Furthermore nursing courses in


baccalaureate program usually are limited
to the last two academic years or
approximately 18months , while diploma
schools offer 24-36 months of nursing
theory and practice.
