 INTRODUCTION

Ichthyosis Vulgaris is the most common ichthyosis

Manifests the first year
There is reduced or absent profilaggrin
Autosomal dominant inheritance
The scaly lesions usually apppear in the first few
months
The mainstays of topical therapy are removing scale
 DEFINITION

Ichthyosis vulgaris is characterized by mild,

generalized hyperkeratosis with xerosis (dry
skin) most pronounced on the lower legs.

IV is autosomal-dominantly
Characterized by onset in early childhood (between 3
12 months of age, fine scales over the entire body)
EPIDEMILOGY

 ETIOPATHOGENESIS

Autosomal dominant inheritance

The defect in IV lies in the profilaggrin molecule,
which is missing or reduced
Profilaggrin filaggrin increased
transepidermall water loss and compensatory
hyperproliferation hidration and dehidration
scaling
 CLINICAL MANIFESTATION

Scaly lesion (symmetrically distributed light-gray scale)

Xerosis (dry skin) fish scale pattern

Predilection are the extensor surface of extremities & trunk

Atopic manifestation

hyperkeratosis

Follicular hyperkeratosis
 DIAGNOSIS

Commonly associated with atopy,

anamnesis xerosis and pruritus worse in
winter months
childhood

Physical Skin lesions with fine, powdery

scaling

examination Dry skin

Fish scale pattern

Mild to moderate hyperkeratosis

Epidermis may normal thickness
histopathology or slightly thinned with some loss
of rete ridges
Dimunation or absence granular

 DIFFERENTIAL DIAGNOSIS

X-linked ichthyosis Lamellar ichthyosis Acquired ichthyosis

 THERAPY

Hydration of stratum corneum

Keratolytic agents

Systemic Retinoids
 COMPLICATION

Fissure at ekstremities can caused secondary

infections
Complication depend on systemic disease
 PROGNOSIS

May show improvement in the summer

usually worse in winter season
