Atrial Septal Defect


 The most common congenital heart ds in
adult

 Three types:
1. Sinus venosus
2. Ostium secundum
3. Oastium primum
 Systolic murmur and wide,fixed, split S2
without cyanosis

 Incomplete RBBB on ECG

 Color Doppler and contrast Echo show


inter-atrial shunting

 Cardiac chateterization with oxygen step-


up whitin the right atrium: rigt –sided
catheter may pass into the left atrium
across the defect
Valvular Septal dcefect

 Four types:
1. Perimembranous (75 %0
2. Supracristal or Outlet septum
3. Atriovetricular canal defect or
inlet septum
4. Muscular (low septal) or
trabecular (15 %)
 History of murmur appearing shortly after birth

 Holosystolic murmur at left sternal border

 Left ventricular enlargement or biventricular


enlargement by ECG, chest X-ray and Echo

 Doppler and contrast Echo shows inter-


ventricular shunting

 Cardiac catheterization with oxygen step-up


within the right ventricel; right-sided catheter
may pass into the left ventricle across the
defect
