Main aim and major elements
“The main aim of this article is to examine the causal factors that lead to
the human error in the helicopter accidents which occurred in Brazil
between 2006 and 2015 usingHFACS”.
• The proportion of accidents associated with perceptual errors was
relatively low
• Inadequate supervision was the most common preconditions for
unsafe acts
• Decision errors accounted for the highest percentage of the helicopter
accidents
