PENDAHULUAN
PTM berkontribusi 60% kematian dan
43% dari seluruh penyakit. 79%
terjadi di negara berkembang.
Pada tahun 2020, menyebabkan
73% kematian dan 60% dari beban
Sebagian adalah CVD

– Mempunyai efek protektif: anti mikroba, anti
virus
• Pada masyarakat tropis:
– kelapa sumber lemak satu-satunya
– Bagian dari food culture
• Kelapa dikonsumsi dengan bms protektif
lain: ikan, bumbu
Asam lemak laurat
• Masyarakat tropis:
• Tak terdapat perbedaan konsumsi kelapa
pada penderita PJK dan orang sehat
• Penderita PJK:
– mengkonsumsi lebih lemak hewani,
– kurang sayuran,
– tinggi makanan mengandung kolesterol
Monounsaturated fatty Acids
• Oleic acid/as.oleat (C18:1) dan as erusik
• Nama lain: omega 9
• Sumber utama: olive oil (minyak
zaitun),alfukat, myk kedele
• Efek terhadap profil lipid: netral
• Lemak primadona
• Masyarakat Mediteranian: rendah PJK,
konsumsi olive oil tinggi
PUFA
• Omega 6:
– menurunkan LDL
– Menurunkan HDL
– Gampang teroksidasi, sangat aterogenik
– Dapat menjadi asam lemak trans
– Asam lemak trans: sangat aterogenik
– Sudah ditinggalkan
PUFA
• Omega 3:
– Penghambat trombosis
– Protektif
– menghambat produksi tromboksan,
meningkatkan sintesa prostaksilin, menurunkan
viskositas darah dan trombosis
– Sumber utama: ikan,brokoli
KOLESTEROL
• Kolesterol
– Lipid sederhana
– TDD 4 cincin siklopentanoperhidrofenantren
– Sintesis kolesterol
• Pembentukan HMG-KoA (-hidroksi--
metilglutaril-koA)
• Perubahan HMG-koA menjadi skualen, melalui
reduksi menjadi asam mevalonat oleh enzim HMG-
koA reduktase
• Pembentukan kolesterol dari skualen
LEMAK
• Lipoprotein
– Kilomikron
• Dibentuk sistem limfa usus
• Fs : transpor TG & kolesterol mkn dan usus
– VLDL
• Dibentuk oleh sel hati
• Fs : transpor TG dan Kolesterol dari gati
– IDL
• Konversi VLDL ke LDL
• tdd TG & ester kolesterol dlm jml sama
LEMAK
– LDL
• Produk katabolik IDL
• Paling kaya kolesterol
• Fs : transpor kolesterol ke jaringan
– HDL
• Disintesis oleh hati & usus
• Tdd : HDL 2 & HDL 3
• Fs : transpor ester kolesterol ke hepar
Hubungan aterosklerosis dengan
profil lipid, serat dan AO
• Aterosklerosis : diawali oleh pembentukan
plak tdd timbunan lemak, trombosit,
magrofag dan sel-sel darah putih.
• Plak permukaan tunika intima mjd kasar
trombosit ditarik aterom
• Hiperkolesterolemia patogenesis

ANTIOKSIDAN
• Radikal bebas
– gugusan atom atau molekul dengan satu atau
lebih elektron tanpa pasangan di orbit luarnya.
• Rx. Rad. Bebas
– fase inisiasi
– fase propagasi
– fase terminasi
ANTIOKSIDAN
• Antioksidan (AO) :
– Senyawa dengan kadar yang sangat rendah
– secara signifikan dapat mencegah/menghambat
proses oksidasi substratnya.
• Peran AO :
– Menghambat rx. rad. bebas
– Scavenger dan memutus rantai rx. rad. bebas
– Sequester ion logam yang prooksidan
ANTIOKSIDAN
• Vitamin C
– AO larut air
– AKG :
• non perokok : 60 mg/hari
• perokok : + 30 - 40 mg/hari
• Ausman (1999) : 120 mg/hari peny. Kronis
– Mudah diserap : asupan 20 - 120 mg/hari
abs 90 %
VITAMIN C
• Fungsi vitamin C :
– Antioksidan
– sintesis kolagen
– Metabolisme obat & steroid
– Fungsi Imun
– Meningkatkan abs. Fe
– Met. Histamin & neurotransmiter
VITAMIN C
• Peran sbg AO
– Scavenger thd ROS, OH & RNS
– Memulihkan rad.tokoferol quinon
• Peran terhadap dislipidemia
– Belum pasti
– Ko faktor enzim kolesterol 7-monooksigenase
– Aktivitas enzim HMG-KoA reduktase
ANTIOKSIDAN VIT E
• Vitamin E
– tdd : tokoferol & tokotrienol
– tokoferol
• derivat fenol
• larut lemak
• berkonsentrasi dalam membran lipid &
lipoprotein
• BMS : gandum, myk tumbuhan, kacang-
kacangan, sayuran berwarna hijau
VITAMIN E
• Potasium
– ratio sodium/potasium = 1 dapat menurunkan
TD
– Potasium mernyebabkan dilatasi arteriol,
pengeluaran sodium dan air, menekan renin
angiotensin dan me aktivitas adrenergik
Nutrisi & hipertensi
• Mg
– menghambat kontraksi otot polos dan
vasodilator

Nutrigenomic
Definition : The study of how different foods
may interact with specific genes to increase the
risk of common chronic diseases such as type 2
diabetes, obesity, heart diseases , stroke and
certain cancers.
Nutrigenomic : provide a molecular
understanding of how common chemicals in the
diet affect health by altering the expression of
genes and the structure of and individual ‘s
genome

SALT INTAKE (con’t)
• BP response to changes in dietary
sodium intake heterogenous.
• Salt sensitive and salt resistant.
• Effect of sodium reduction on BP greater
in black, middle age and older person,
DM, CKD. This group less responsive to
RAAS.
• Conclusion: ACE (I/D) polymorphism is associated with
increased serum ACE activity and consequently with
increased risk for essential hypertension and its
complications. The frequency of different ACE genotypes
were; 41.7% homozygous (DD), 45.2% heterozygous (ID)
and 13.1% homozygous (II) indicating that (D) allele is
significantly associated with essential hypertension. Also
serum ACE level was significantly correlated with ACE
(I/D) polymorphism (p=0.001). Patients with DD genotype
had the highest serum ACE level followed by patients with
ID genotype and patients with II genotype had the lowest
serum ACE level. Conclusion: ACE (I/D) polymorphism is
associated with increased serum ACE activity and
consequently with increased risk for essential hypertension
and its complications.
