Tinjauan Pustaka
 Keratitis merupakan infiltrasi
sel radang pada kornea yang akan
mengakibatkan kornea menjadi keru.
Akibatnya terjadi kekeruhan yang terj
adi pada kornea, maka tajampengliha
tan akan menurun.

Superfisialis dan Interstisial atau profunda
Keratitis pungtata
Merupakan keratitis yang terkumpul di daer
ah membrane bowman dengan infiltrate ber
bentuk bercak-bercak halus.

Keratitis herpes simpleks

Pada yang epithelial kerusakan terjadi akibat
pembelahan virus di dalam sel epitel yang aka

Keratitis herpes simpleks

Gejala yang terlihat pada mata adalah rasa sa

sesuai dengan dermatom yang dipersarafi sar

Keratitis herpes simpleks
Keratitis subepithelial,
ilfiltrat yang bulat atau lonjong di dalam jaringan

kornea. Biasanya merupakan keratitis profunda
superfisial, terjadi akibat infeksi virus herpes si
mpleks. Sering diduga keratitis disiformis merup
akan reaksi alergi ataupun imunologik terhadap
infeksi virus herpes simpleks pada permukaan k
ornea.
 Klasifikasi
Superfisialis dan Interstisial atau profunda
Keratitis lagoftalmos
Keratitis yang terjadi akibat adanya kelopak
mata tidak dapat menutup dengan sempurn
a sehingga terdapat kekeringan kornea. Lag
oftalmos akan mengakibatkan mata terpapa

Keratitis Interstisial
Keratitis yang ditemukan pada jaringan kornea
Keratitis Profunda, yang lebih dalam pada kedua mata, merupakan
tes fluoresin (-) keratitis nonsupuratif profunda disertai dengan
neovaskularisasi. Keratitis ini disebut juga
dengan keratitis parenkimatosa.

Manifestasi Klinik
Gejala yang ditimbulkan keratitis sangat bervariasi dari ringan hingga berat, dimana pasien akan mengeluhkan
• nyeri mata,
• mata berair
• rasa mengganjal disertai dengan penurunan tajam penglihatan.

Pada pemeriksaan menggunakan slit lamp keratitis pungtata superfisialis ditandai dengan kekeruhan epitel
yang meninggi berbentuk lonjong, dan tegas yang menampakkan bintik-bintik pada pemulasan dengan fluoresen, terutama
pada daerah sentral.

hijau pada pemeriksaan slit lamp disebut fluore

 Penatalaksanaan
Pengobatan diberikan tergantung organisme penyebab, misalnya antibiotik, antivirus, antijamur.

Virus Bakteri
virus dapat digunakan trifluridin, .
Gram (+) : Cafazolin, penisilin G atau vancomisin
idoxuridine, dan acyclovir. Gram (-) : Tobramisin, gentamisin, atau polimixin B

Medikamentosa lainnya yang dapat diberikan

dengan tujuan mengatasi gejala yang ditimbul
kan oleh penyulit lainnya misalnya untuk melin

Natamisin, amfoterisin, atau fluconazole enggunakan kacamata.
Prognosis
Prognosis bergantung pada virulensi organisme, lokasi dan
perluasan ulkus kornea, vaskularisasi dan deposit kolagen,
diagnosis awal dan terapi tepat dapat membantu mengurangi
komplikasi. Keratitis pungtata superfisial penyembuhan biasa
nya berlangsung baik meskipun tanpa pengobatan. Imunitas
tubuh merupakan hal yang peting dalam kasus ini karena
diketahui reaksi imunologik tubuh pasien sendiri yang
memberikan respon terhadap virus maupun bakteri.

 Keluhan Sekarang

Pasien datang ke poli klinik mata RSU

D Tengku Rafi’an Siak dengan keluhan kedua m
ata merah sejak ± 2 minggu yang lalu sebelum
masuk rumah sakit. Pasien juga mengeluhkan m
ata terasa perih, berair, pandangan kabur, dan t
erdapat secret warna putih encer. Pasien menga
ku tidak ada riwayat trauma sebelumnya dan riw
ayat penggunaan lensa kontak, pasien sering m
engetik dalam jangka waktu yang lama sehingga
sering menggunakan obat tetes mata insto jika
mata lagi perih tetapi keluhan yang ada hilang ti
mbul

Kedua mata merah sejak ± 2 min Sudah pernah berobat dengan Pasien mengaku belum pernah
ggu yang lalu sebelum masuk ru obat tetes mata cendo cenfresh mengalami keluhan seperti ini se
mah sakit dan insto beli di apotik belumnya.
