Pendahuluan
 Pendahuluan
DIC (disseminated intravascular coagulations) merupakan
komplikasi dari sepsis atau hematologic malignancy dan
menunjukan tanda adanya Hemorrhage. Koagulasi yang
abnormal dan trombositopenia berhubungan dengan
penyakit yang berat. Jumlah platelet yang rendah dapat
memprediksi outcomes yang buruk.

 Patofisiologi DIC, melibatkan Interaksi kaskade
koagulasi, host imunne dan inflammatory system

sepsis-associated DIC -> respon agregrasi ->

menyebabkan-> kegagalan organ multipel, shock
dan kematian.
Tingkat keparahan disfungsi organ dapat
menentukan prognosis pasien

Metode dan pasien
 Metode

level biomarker plasma termasuk sitokin, kemokin,

selektin larut, PDMPs (platelet-derived microparticles),
soluble vascular adhesion molecule 1, dan kelompok
protein 1 dengan mobilitas tinggi diukur dengan enzyme-
linked immunosorbent assay pada awal dan setelah hari
ke 4,7,14 dan 21 pengobatan

 Pasien

Studi kohort (April 2013- Agustus 2016)

Grup a: 25 pasien sepsis ; 18 dengan DIC, 7 tanpa DIC

Grup b: 15 pasien dengan Hematologic Malignancy ;

akut leukemia, limpoma maligna atau multipel
myeloma.
3 pasien diterapi dengan angiotensin II receptor bloker

Kontrol grup: 12 relawan sehat

 Pengumpulan data
1. Coagulations marker; fibrin, fibrinogen degradations
products(FDP), D-dimers, prothrombin time ratios,
platelet count, antitrhrombin (AT). Pengambilan data
dilakukan sebelum pengobatan
2. Diukur setelah pengobatan
-APACHE II (The Acute Physiology & Chronic Health
Evaluation II)
-SOFA Scores (the Sequential Organ Failure Assesment)
-Diagnosis DIC -> JAAM (Japanese Assosiation for Acute
Medicine) dan JMHW (Japanese Ministry of Health and
Welfare)

 Pengumpulan data
-Pengukuran PDMPs menggunakan ->metode ELISA
-Pengukuran sitokin dan molekul terlarut ->metode
ELISA
-Nilaicutoff dari marker: pada hari ke 28 kematian

dihitung menggunakan penanda yang telah dipilih pada

analisis multivariat
-studi tentang aktivasi trombosit-> agonis, adenosin
difosfat(ADP), dan Kolagen.
-platelet-rich plasma diinkubasi dengan sitoki

Kesimpulan
﹡ Perbedaan yang ditemukan antara pasien yang menderita
sepsis dengan DIC dan HM dengan DIC di aktivasi trombosit
dan di ketinggian diaktifkan PDMPs terkait trombosit dan
sPselectin. Selain itu, IL-6 dan TPO secara signifikan
meningkat pada pasien sepsis dengan DIC. IL-6 dan TPO
dipromosikan aktivasi trombosit in vitro. Penilaian biomarker
ini mungkin bermanfaat dalam pencegahan primer kegagalan
multi-organ pada pasien sepsis dengan DIC. Namun, uji klinis
yang lebih besar diperlukan untuk menguji hipotesis ini.
