suatu peradangan otak,
dan diagnosanya dapat
ditegakkan hanya melalui
pemeriksaan mikroskopis
jaringan otak yang
disebabkan oleh berbagai
macam mikroorganisme.

Radang otak primer

Ensefalitis primer terjadi ketika virus atau
pun agen penyebab penyakit lainnya, langsung Radang otak sekunder
menginfeksi jaringan otak. Infeksi ini bisa berpusat di
satu area saja, atau meluas ke bagian otak lainnya.
Jenis ensefalitis yang satu ini biasanya Tidak seperti radang otak primer
menyerang area lobus temporal, yakni bagian otak yang langsung menyerang jaringan otak,
yang mengatur fungsi memori dan berbicara. radang otak sekunder berbeda. Jenis
Jenis virus penyebab radang otak primer bisa dibagi ensefalitis ini merupakan hasil respon dari
menjadi 3 kelompok utama, yaitu: sistem kekebalan tubuh yang salah

 04 TANDA DAN GEJALA

Umumnya, manifestasi- manifestasi awal penyakit ini

menyerupai penyakit sistematis akut yang sukar dibedakan.
Misalnya
- Demam
- Sakit kepala
- Pada bayi disertai jeritan- jeritan
- Perasaan tidak nyaman pada perut
- Mual serta muntah
- Tanda- tada yang berhubungan dengan nasofaringitis
ringan dapat menunjukkan infeksi pernafasan
- Suhu tubuh meningkat
- Kelemahan mental yang dengan stupor
- Kejang- kejang

 06 PEMERIKSAAN DIAGNOSTIK

Pemeriksaan Diagnostik menurut (Victor, 2001) yaitu :

1. Biakan :
Dari darah : viremia berlangsung hanya sebentar saja sehingga sukar untuk mendapatkan
hasil yang positif.
Dari likuor serebrospinalis atau jaringan otak (hasil nekropsi), akan didapat gambaran jenis
kuman dan sensitivitas terhadap antibiotika.
Dari feses, untuk jenis enterovirus sering didapat hasil yang positif.
Dari swap hidung dan tenggorokan, akan didapat hasil kultur positif.

2. Pemeriksaan serologis : uji fiksasi komplemen, uji inhibisi hemaglutinasi dan uji
neutralisasi. Pada pemeriksaan serologis dapat diketahui reaksi antibodi tubuh, IgM dapat
dijumpai pada awal gejala penyakit timbul.

3. Pemeriksaan darah : terjadi peningkatan angka leukosit.

4. Punksi lumbal Likuor serebospinalis sering dalam batas normal, kadang-kadang

ditemukan sedikit peningkatan jumlah sel, kadar protein atau glukosa.

5. EEG/ Electroencephalography EEG sering menunjukkan aktifitas listrik yang merendah

sesuai dengan kesadaran yang menurun. Adanya kejang, koma, tumor, infeksi sistem saraf,
bekuan darah, abses, jaringan parut otak, dapat menyebabkan aktivitas listrik berbeda dari
pola normal irama dan kecepatan. (Smeltzer, 2002).

6.CT scan Pemeriksaan CT scan otak seringkali didapat hasil normal, tetapi bisa pula
didapat hasil edema diffuse, dan pada kasus khusus seperti Ensefalitis herpes simplex, ada
kerusakan selektif pada lobus inferomedial temporal dan lobus frontal
 07 PENATALAKSANAAN

Penatalaksanaan yang dilakukan pada ensefalitis menurut (Victor, 2001) antara lain :
 Isolasi : bertujuan mengurangi stimuli/rangsangan dari luar dan sebagai tindakan pencegahan.
 Terapi antimikroba, sesuai hasil kultur. Obat yang mungkin dianjurkan oleh dokter :
Ampicillin : 200 mg/kgBB/24 jam, dibagi 4 dosis.
Kemicetin : 100 mg/kgBB/24 jam, dibagi 4 dosis.
• Bila encephalitis disebabkan oleh virus (HSV), agen antiviral acyclovir secara signifikan dapat

menurunkan mortalitas dan morbiditas HSV encephalitis. Acyclovir diberikan secara intravena
dengan dosis 30 mg/kgBB per hari dan dilanjutkan selama 10-14 hari untuk mencegah
kekambuhan.

• Untuk kemungkinan infeksi 02 sekunder diberikan antibiotika secara polifragmasi.
• Mengurangi meningkatnya tekanan intrakranial : manajemen edema otak
• Mempertahankan
01 hidrasi, monitor balans cairan : jenis dan jumlah cairan yang diberikan tergantung
keadaan anak.
• Glukosa 20%, 10 ml intravena beberapa kali sehari disuntikkan dalam pipa giving set untuk
menghilangkan edema otak.

07 PENATALAKSANAAN
PENATALAKSANAAN

• Kortikosteroid intramuscular atau intravena dapat juga digunakan untuk menghilangkan edema otak.
• Mengontrol kejang : Obat antikonvulsif diberikan segera untuk memberantas kejang. Obat yang diberikan
ialah valium dan atau luminal.
• Valium dapat diberikan dengan dosis 0,3-0,5 mg/kgBB/kali.
• Bila 15 menit belum teratasi/kejang lagi bia diulang dengan dosis yang sama.
Jika sudah diberikan 2 kali dan 15 menit lagi masih kejang, berikan valium drip dengan dosis 5 mg/kgBB/24
jam.
• Mempertahankan ventilasi : Bebaskan jalan nafas, berikan O2 sesuai kebutuhan (2-3l/menit).
Penatalaksanaan shock septik.
• Mengontrol perubahan suhu lingkungan.
• Untuk mengatasi hiperpireksia, diberikan kompres pada permukaan tubuh yang mempunyai pembuluh
besar, misalnya pada kiri dan kanan leher, ketiak, selangkangan, daerah proksimal betis dan di atas
kepala. Sebagai hibernasi dapat diberikan largaktil 2 mg/kgBB/hari dan phenergan 4 mg/kgBB/hari secara

intravena atau intramuscular dibagi dalam 3 kali pemberian. Dapat juga diberikan antipiretikum seperti
asetosal atau parasetamol bila keadaan telah memungkinkan pemberian obat per oral.

08 ASKEP ENSEFALITIS
A. Pengkajian

1. Biodata
2. Keluhan utama
keluhan utama pada penderita encephalitis yaitu sakit kepala,
kaku kuduk, gangguan kesadaran, demam dan kejang.
3. Riwayat penyakit sekarang.
Merupakan riwayat klien saat ini yang meliputi keluhan, sifat dan
hebatnya keluhan, mulai timbul atau kekambuhan dari penyakit yang
pernah dialami sebelumnya. Biasanya pada masa prodromal berlangsung
antara 1-4 hari ditandai dengan demam, sakit kepala, pusing, muntah, nyeri
tenggorokan, malaise, nyeri ekstrimitas dan pucat. Kemudian diikuti tanda
ensefalitis yang berat ringannya tergantung dari distribusi dan luas lesi
pada neuron. Gejala terebut berupa gelisah, irritable, screaning attack,
perubahan perilaku, gangguan kesadaran dan kejang kadang-kadang
disertai tanda neurologis fokal berupa afasia, hemiparesis, hemiplegia,
ataksia dan paralisi saraf otak.
4. Riwayat kehamilan dan kelahiran.
Dalam hal ini yang dikaji meliputi riwayat prenatal, natal dan post natal.
Dalam riwayat prenatal perlu diketahui penyakit apa saja yang pernah diderita oleh ibu terutama penyakit infeksi.
Riwayat natal perlu diketahui apakah bayi lahi dalam usia kehamilan aterm atau tidak karena mempengaruhi
system kekebalan terhadap penyakit pada anak. Trauma persalinan juga mempengaruhi timbulnya penyakit
contohnya aspirasi ketuban untuk anak. Riwayat post natal diperlukan untuk mengetahui keadaan anak setelah
lahir.
Contoh : BBLR, apgar score, yang mempengaruhi pertumbuhan dan perkembangan selanjutnya.

5. Riwayat penyakit dahulu.

Kontak atau hubungan dengan kasus-kasus meningitis akan meningkatkan kemungkinan terjdinya peradangan
atau infeksi pada jaringan otak (J.G. Chusid, 1993). Imunisasi perlu dikaji untuk mengetahui bagaimana
kekebalan tubuh anak. Alergi pada anak perlu diketahui untuk dihindarkan karena dapat memperburuk keadaan.
6. Riwayat kesehatan keluarga.
Merupakan gambaran kesehatan keluarga, apakah ada kaitannya dengan penyakit
yang dideritanya.

7. Riwayat social.

8. Kebutuhan dasar (aktfitas sehari-hari).

Pada penderita ensepalitis sering terjadi gangguan pada kebiasaan sehari-hari
antara lain : gangguan pemenuahan kebutuhan nutrisi karena mual muntah,
hipermetabolik akibat proses infeksi dan peningkatan tekanan intrakranial. Pola
istirahat pada penderita sering kejang, hal ini sangat mempengaruhi penderita.
Pola kebersihan diri harus dilakukan di atas tempat tidur karena penderita lemah
atau tidak sadar dan cenderung tergantung pada orang lain perilaku bermain perlu
diketahui jika ada perubahan untuk mengetahui akibat hospitalisasi pada anak.
9. Pemeriksaan fisik.
Pada klien ensephalistis pemeriksaan fisik lebih difokuskan pad apemeriksaan
neurologis. Ruang lingkup pengkajian fisik keperawatan secara umum meliputi :
a) Keadaan umum.
Penderita biasanya keadaan umumnya lemah karena mengalami perubahan atau
penurunan tingkat kesadaran. Gangguan tingkat kesadaran dapat disebabkan oleh
gangguan metabolisme dan difusi serebral yang berkaitan dengan kegagalan neural akibat
prosses peradangan otak.
b) Gangguan system pernafasan.
Perubahan-perubahan akibat peningkatan tekanan intra cranial menyebabakan kompresi
pada batang otak yang menyebabkan pernafasan tidak teratur. Apabila tekanan intrakranial
sampai pada batas fatal akan terjadi paralisa otot pernafasan (F. Sri Susilaningsih, 1994).
c) Gangguan system kardiovaskuler.
Adanya kompresi pada pusat vasomotor menyebabkan terjadi iskemik pada daerah
tersebut, hal ini akan merangsang vasokonstriktor dan menyebabkan tekanan darah
meningkat. Tekanan pada pusat vasomotor menyebabkan meningkatnya transmitter
rangsang parasimpatis ke jantung.
d) Gangguan system gastrointestinal.
Penderita akan merasa mual dan muntah karena peningkatan tekanan
intrakranial yang menstimulasi hipotalamus anterior dan nervus vagus sehingga
meningkatkan sekresi asam lambung. Dapat pula terjadi diare akibat terjadi
peradangan sehingga terjadi hipermetabolisme (F. Sri Susilanigsih, 1994).

10. Pertumbuhan dan perkembangan.

Pada setiap anak yang mengalami penyakit yang sifatnya kronis atau
mengalami hospitalisasi yang lama, kemungkinan terjadinya gangguan
pertumbuhan dan perkembangan sangat besar. Hal ini disebabkan pada
keadaan sakit fungsi tubuh menurun termasuk fungsi social anak. Tahun-tahun
pertama pada anak merupakan “tahun emas” untuk kehidupannya. Gangguan
atau keterlambatan yang terjadi saat ini harus diatasi untuk mencapai tugas –
tugas pertumbuhan selanjutnya.
 B. Diagnosa Keperawatan

1. Gangguan perfusi jaringan serebri yang berhubungan dengan peningkatan tekanan intracranial
2. Ketidakefektifan bersihan jalan nafas yang berhubungan dengan akumulasi sekret, kemampuan batuk menurun akibat
penurunan kesadaran.
3. Risiko tinggi defisit cairan dan hipovolemik
4. Risiko tinggi gangguan nutrisi: kurang dari kebutuhan tubuh yang berhubungan dengan ketidakmampuan menelan, keadaan
hipermetabolik
5. Resiko tinggi cedera yang berhubungan dengan kejang, perubahan status mental, dan penurunan tingkat kesadaran
6. Resiko kejang berulang
7. Nyeri yang berhubungan dengan adanya iritasi lapisan otak
8. Gangguan mobilitas fisik yang berhubungan dengan kerusakan neuromuskular, penurunan kekuatan otot, penurunan
kesadaran, kerusakan persepsi/kognitif
9. Gangguan persepsi sensorik yang berhubungan dengan kerusakan penerima rangsang sensorik, tranmisi sensorik, dan
integrasi sensori.
10. Koping individu tidak efektif yang berhubungan dengan prognosis penyakit, perubahan psikososial, perubahan persepsi
kognitif, perubahan aktual dalam struktur dan fungsi, ketidakberdayaan dan merasa tidak ada harapan.
11. Cemas yang berhubungan ancaman, kondisi sakit, dan perubahan kesehatan.
 3. INTERVENSI KEPERAWATAN

1. Gangguan perfusi jaringan serebri yang berhubungan dengan peningkatan tekanan

intracranial.
Tujuan : perfusi jaringan otak meningkat
Kriteria Hasil : tingkat kesadaran meningkat lebih sadar, disorientasi
negatif, konsentrasi baik, perfusi jaringan dan oksigenasi baik, tanda
tanda vital dalam batas normal dan syok dapat dihindari.

-Monitor klien dengan ketat terutama setelah lumbal pungsi. Anjurkan klien berbaring minimal 4- 6
jam setelah lumbal pungsi.
-Monitor tanda-tanda peningkatan intrakranial selama perjalanan penyakit (nadi lambat, tekanan
darah meningkat, kesadaran menurun, napas irreguler, refleks pupil menurun, kelemahan)
-Monitor tanda-tanda vital dan neurologis tiap 5-30 menit. Catat dan laporkan segera perubahan-
perubahan tekanan intrakranial ke dokter.
-Hindari posisi tungkai ditekuk atau gerakan-gerakan klien, anjurkan untuk tirah baring.
-Tinggikan sedikit kepala klien dengan hati-hati, cegah gerakan yang tiba-tiba dan tidak perlu dari
kepala dan leher, hindari fleksi leher
-Bantu seluruh aktivitas dan gerakan-gerakan klien.
-Beri penjelasan keadaan lingkungan pada klien
-Evaluasi selama masa penyembuhan terhadap gangguan motorik, sensorik, dan intelektual
-Kolaborasi pemberian steroid osmotik.
2. Ketidakefektifan bersihan jalan napas yang berhubungan dengan akumulasi sekret,
kemampuan batuk menurun akibat penurunan kesadaran.
Tujuan : jalan napas kembali efektif
Kriteria Hasil : sesak napas negatif, frekuensi napas 16-20x/menit tidak menggunakan otot bantu
napas, dapat mendemontrasikan cara batuk efektif.

-Kaji fungsi paru, adanya bunyi napas tambahan, perubahan irama dan kedalaman, penggunaan otot-
otot aksesori, warna dan kekentalan sputum.
-Atur posisi fowler dan semifowler
-Ajarkan cara batuk efektif
-Lakukan fisioterapi dada: vibrasi dada
-Penuhi hidrasi cairan via oral seperti minum air putih dan pertahankan asupan cairan 2500 ml/hari
-Lakukan pengisapan lendir dijalan napas
3. Resiko tinggi gangguan nutrisi: kurang dari kebutuhan yang berhubungan dengan ketidakmampuan
menelan, keadaan hipermetabolik.
Tujuan : kebutuhan nutrisi terpenuhi dalam waktu 5x24 jam.
Kriteria hasil : turgor baik, asupan dapat masuk sesuai kebutuhan, terdapat kemampuan menelan, sonde
dilepas, berat badan meningkat 1 kg, Hb dan albumin dalam batas normal.

-Observasi tekstur dan turgo kulit.

-Lakukan oral hygene
-Observasi asupan dan pengeluaran.
-Observasi posisi dan keberhasilan sonde
-Tentukan kemampuan klien dalam mengunyah, menelan, dan refleks batuk.
-Kaji kememuan klien dalam menelan, batuk, dan adanya sekret.
-Auskultrasi bising usus, amati penurunan atau hiperaktivitas bising usus.
-Timbang berat badan sesuai indikasi.
-Beri makan dengan cara meninggikan kepala.
-Letakkan posis kepala lebih tinggi pada waktu, selama dan sesudah makan
-Stimulasi bibir untuk menutup dan membuka mulut secara manual dengan menekan ringan di atas bibir/ di bawah
dagu jika dibutuhkan.
-Letakkan makanan pada area mulut tang tidak terganggu.
-Beri makan dengan perlahan pada lingkungan yang tenang.
-Mulailah untuk memberi makan per oral setengah cair dan makanan lunak ketika klien dapat menelan air.
4. Resiko terjadi cidera yang berhubungan dengan kejang, perubahan
status mental, dan penurunan tingkat kesadaran.
Tujuan : Dalam waktu 3x24 jam perawatan, klien bebas dari cedera
yang disebabkan oleh kejang dan penurunan kesadaran.
Kriteri hasil : klien tidak mengalami cedera apabila ada kejang berulang.

-Monitor kejang pada tangan, kaki, mulut, dan otot-otot muka lainnya.
-Persiapkan lingkungan yang aman seperti batasan ranjang, papan pengaman,
dan alat suction selalu berada dekat klien
-Pertahankan bedrest total selama fase akut
-Kolaborasi pemberian terapi: diazepam, fenobarbital
5. Nyeri kepala berhubungan dengan iritasi lapisan otak
Tujuan : keluahan nyeri berkurang/rasa sakit terkendali
Kriteria hasil : klien dapat tidur dengan tenaang, wajah rileks, dan klien
memverbalisasikan penurunan rasa sakit

-Usahakan membuat lingkungan yang aman dan tenang.

-Kompres dingin (es) pada kepala
-Lakukan penatalaksanaan nyeri dengan metode distraksi dan relaksasi napas dalam
-Lakukan latihan gerak aktif atau pasif sesuai kondisi dengan lembut dan hati-hati
-Kolaborasi pemberian analgesik
6. Gangguan mobilitas fisik berhubungan dengan kerusakan neuromuskular, penurunan kekuatan
otot, penurunan kesadaran, kerusakan persepsi/kognitif.
Tujuan : tidak terjadi kontraktur, footdrop, gangguan integritas kulit, fungsi pencernaan dan kandung
kemih optimal, serta peningkatan kemampuan fisik
Kriteria Hasil: skala ketergantungan klien meningkat menjadi bantuan minimal

-Tinjau kemampuan fisik dan kerusakan yang terjadi

-Kaji tingkat imobilisasi, gunakan skala ketergantungan
-Berikan perubahan posisi yang teratur pada klien
-Pertahankan kesejajaran tubuh yang adekuat, berikan latihan ROM pasif jika klien sudah bebas panas
dan kejang
-Berikan perawatan kulit secara adekuat, lakukan masase, ganti pakaian klien dengan bahan linen dan
pertahankan tempat tidur dalam keadaan kering
-Berikan perawatan mata, bersihkan mata, dan tutup dengan kapas yang basah sesekali
-Kaji adanya nyeri, kemerahan, bengkak pada area kulit
7. Cemas yang berhubungan ancaman, kondisi sakit, dan perubahan kesehatan.
Tujuan : mengakui dan mendiskusikan rasa takut. Mengungkapkan keakuratan pengetahuan
tentang situasi. Tampak rileks dan melaporkan ansietas berkurang sampai pada tingkat dapat
diatasi.

-Kaji status mental dan tingkat ansietas dari pasien/keluarga. Catat adanya tanda-tanda verbal
atau non verbal.
-Berikan penjelasan hubungan antara proses penyakit dan gejalanya.
-Jawab setiap pertanyaan dengan penuh perhatian dan berikan informasi tentang prognosa
penyakit
-Jelaskan dan persiapkan untuk tindakan prosedur sebelum duilakukan.
-Berikan kesempatan pasien/keluarga untuik mengumgkapkan isi pikiran dan perasaan takutnya.
-Libatkan pasien/keluarga dalam perawatan.
-Berikan petunjuk mengenai sumber-sumbner penyokong yang ada, seperti keluarga, konselor
professional dan sebagainya
