 SKENARIO
Seorang laki-laki umur 67 tahun dibawa keruang gawat darurat dengan keluhan sesak napas berat. Sejak
lama penderita berobat dengan hipertensi tapi tidak teratur dan telah pernah mengalami infark miokard
sebelumnya. Sekitar seminggu sebelum masuk rumah sakit dia mengeluh nyeri dada substernal lebih dari
30 menit, dan sejak itu dia mengeluh sering sesak napas yang makin berat. Penderita hanya bisa tidur
dengan 3 bantal kepala dan sering terbangun tengah malam akibat sesak napas. Pada pemeriksaan fisik :
tidak demam, tekanan darah 160/100 mmHg, denyut jantung 110 kali/menit, pernapasan 22 kali/menit, dan
saturasi O2 88%. Penderita pucat dan berkeringat dingin. Pada pemeriksaan auskultasi ditemukan adanya
ronchi basah pada kedua basal-medial paru, terdengar S3 dan S4, tidak terdengar bising jantung. Pada
pemeriksaan EKG salah sau kelainan yang ditemukan adalah adanya gelombang Q patologis di sendapan
V1-V4.
