»KELUHAN UTAMA

Bercak kehitaman pada hampir seluruh tubuh

sejak 3 minggu yang lalu.
»RIWAYAT PENYAKIT SEKARANG

⋄Pasien dikonsulkan ke bagian Kulit dan

Kelamin tanggal 4 agustus dengan keluhan
utama timbul bercak kemerahan pada
hampir seluruh tubuh sejak 3 minggu yang
lalu
⋄Awalnya bintil kemerahan di tangan yang
menjalar hingga ke seluruh tubuh
 Lanjutan

⋄Gatal (+) yang terutama dirasakan

pada malam hari
⋄Luka di kaki (+), nanah (+) 2 minggu
yang lalu. Namun saat ini sudah
mengering
⋄Pasien saat ini dirawat di irina F
neuro dengan penurunan kesadaran
ec SOL intrakranial
»Riwayat Penyakit Dahulu
Pasien pernah mengalami penyakit yang
seperti ini sebelumnya kurang lebih 1 tahun
yang lalu dan diobati dengan scabimite.

»Riwayat Alergi
Pasien tidak memiliki riwayat alergi makanan,
obat-obatan maupun debu.

»Riwayat Atopi
Pasien tidak memiliki riwayat asma dan
riwayat bersin-bersin pada pagi hari.
»Riwayat Penyakit Keluarga
Sebelumnya anak pasien menderita penyakit
yang serupa dan saat ini seluruh anggota
keluarga dalam rumah juga menderita
penyakit yang seperti ini.

»Riwayat Sosial Ekonomi

Kehidupan sehari-hari pasien sebagai ibu
rumah tangga. Rumah pasien beratap seng,
berdinding beton, berlantai keramik dan
memiliki ventilasi yang baik
»Riwayat Kebiasaan
Pasien tinggal dirumah orang tuanya
dan tinggal 6 orang dalam rumah.
Pasien mengaku jarang mandi dan
jarang menjemur kasur. Pasien tidur
bersama suami dan anaknya di ruangan
yang sama.

Pada pemeriksaan mikroskopis
kerokan kulit tidak ditemukan parasit,
telur ataupun kotoran (skibala)

DIAGNOSIS
Skabies + Infeksi Sekunder

»Dermatitis

PENATALAKSANAAN
»Permethrin 5% 1x oles selama 10 jam
»Asam fusidat cream 2x oles
»Cetirizine 10 mg 1x1 PO
 EDUKASI
»Mandi dengan air hangat dan keringkan
badan
»Pengobatan skabisid topikal yang dioleskan
di seluruh kulit, kecuali wajah, sebaiknya
dilakukan pada malam hari sebelum tidur
»Ganti pakaian, handuk, sprei yang digunakan,
dan cuci teratur, bila perlu direndam dengan
air panas karena tungau akan mati pada suhu
130o C;
»Hindari penggunaan pakaian, handuk, seprai
bersama anggota keluarga serumah
»Setelah periode waktu yang
dianjurkan, segera bersihkan skabisid
dan tidak boleh mengulangi
penggunaan skabisid yang berlebihan
setelah seminggu sampai dengan 4
minggu yang akan datang; dan
»Setiap anggota keluarga serumah
sebaiknya mendapatkan pengobatan
yang sama dan ikut menjaga
kebersihan
BAB IV
PENUTUP
Seorang perempuan berumur 36 tahun, dikonsulkan
dengan keluhan utama bercak kehitaman di hampir
seluruh tubuh sejak 3 minggu yang lalu. Awalnya
timbul bintil kemerahan dulu di seluruh tubuh serta
adanya keluhan gatal yang terutama pada malam
hari. Sebelumnya anak Pasien menderita penyakit
yang serupa dan saat ini seluruh anggota keluarga
dalam rumah juga menderita seperti ini.
Pada pemeriksaan st. dermatologis → makula hiperpigmentasi,
multiple, berbatas difus generalisata. Pada regio dorsum pedis
dekstra erosi (+), krusta (+). Pemeriksaan mikroskopis kerokan
kulit tidak ditemukan tungau.
Berdasarkan anamnesis dan pemeriksaan fisik pasien di
diagnosis dengan skabies + infeksi sekunder. Terapi yang
diberikan krim permethrin 5%, krim asam fusidat dan cetirizine
1x 10 mg (PO). Penatalaksanaan juga diberikan pada orang-
orang yang kontak langsung dengan penderita.
